Materials and Methods A survey of 200 patients collected questions about clear aligner treatment, identifying 29 frequently asked ones. Researchers added 14 additional questions, resulting in 43, categorized into usage, side effects/complications, treatment limitations, and others. These questions were posed to chatbot simultaneously. The chatbot’s responses were analyzed for narrative level, readability, informational accuracy, and plagiarism using scoring systems. The data underwent statistical analysis with SPSS V.22. Results The highest FKGL score was observed in usage-related questions (14.53 ± 8.88), while the highest FRES score was in the other group (54.30 ± 0.89). DISCERN, EQIP, and GQS scores were highest in the usage and side effect/complication groups. Correlation analysis showed the strongest correlation between similarity and FKGL (r= -0.391, p = 0.010). For the top 10 questions, the average FKGL was 13.37 ± 4.37, and the average FRES was 47.18 ± 23.69. Regression analysis identified no significant relationships. Conclusion ChatGPT 4o demonstrates potential as a supplementary tool in patient education under professional supervision. However, further research is required to optimize its application and ensure its reliability in clinical settings. Artificial intelligence ChatGPT Patient education Health informatics Clear aligner treatment Introduction Artificial intelligence (AI) refers to the capability of computers or computer-controlled robots to execute tasks that are characteristic of intelligent beings. Utilizing algorithms and computer programs, AI performs complex tasks, analyzes data, and resolves intricate problems. While AI has the potential to enhance system efficiency, it raises significant concerns regarding ethics and safety [ 1 ]. Recently, chatbots equipped with language learning capabilities have been developed to process extensive datasets and generate meaningful outputs on a variety of topics. These features allow chatbots to facilitate easy access to information, making them increasingly popular among users [ 2 ]. ChatGPT, an AI-driven language model developed by OpenAI, synthesizes written sources from the internet to generate coherent texts on specific queries. Its data sources include books, articles, and websites, and the model continuously updates its database to improve response quality [ 3 ]. AI-based systems, such as ChatGPT, are introducing innovative perspectives in healthcare. These systems efficiently address patients’ questions about treatment processes, simplify complex medical information, and reach a broad audience [ 4 , 5 ]. The integration of AI into healthcare has the potential to strengthen doctor-patient communication, facilitate access to accurate information, enhance healthcare infrastructure, reduce the workload of healthcare professionals, and deliver high-quality care to patients [ 5 , 6 ]. However, as patients increasingly rely on internet-based information before, during, and after treatments, it becomes imperative for physicians to evaluate the reliability of such information and educate patients about the risks of misinformation [ 7 , 8 ]. While chatbots are often preferred for their accessibility and conversational style, they pose ethical and safety concerns due to their occasional provision of incorrect or incomplete answers and fabricated references [ 9 ]. Clear aligners have gained significant popularity due to increasing aesthetic concerns and the demand for a more comfortable treatment experience. Despite some disadvantages compared to traditional brackets, clear aligners are favored for their discreet appearance and ease of use. They are custom-made for each patient and replaced at regular intervals [ 10 , 11 ]. However, as a technologically advanced and innovative treatment option, clear aligners frequently raise questions among patients. Similarly, ChatGPT, as an innovative tool requiring cautious use, warrants evaluation for its ability to address such patient queries [ 12 ]. Studies have explored chatbot-generated information on various orthodontic applications, including cleft lip and palate, orthognathic surgery, and temporary anchorage devices [ 13 – 15 ]. Patients seeking clear aligner treatment often turn to internet-based sources like Google, YouTube, and Instagram for information. However, these platforms present challenges in verifying the reliability of their content. Chatbots, offering refined and controllable responses, may serve as a reliable alternative information source. Encouraging patients to use chatbot under medical supervision has the potential to improve treatment management, reduce physicians’ workloads, and provide easy access to accurate information. This study aims to evaluate the responses provided by chatbot to questions related to clear aligner treatment. The analysis focuses on the quality and validity of the information, readability, plagiarism, and narrative complexity. The hypothesis of the study was that chatbot’s responses may have limitations in accuracy and reliability, they exhibit high readability and narrative quality. Material and Method This study was approved by the Ethics Committee of Ege University Faculty of Medicine and the study was conducted in accordance with the Declaration of Helsinki. This study utilized ChatGPT-4o, selected for its extensive and general-purpose database as well as its ease of access for users. The topic of clear aligner treatment was chosen due to its representation as a modern, technology-integrated treatment and its appeal to a patient population that is enthusiastic about utilizing the internet for information. Identification of Questions Related to Clear Aligners The questions were identified using a face-to-face survey technique. Preliminary interviews were conducted with patients visiting the Orthodontics Department of Ege University Faculty of Dentistry. A total of 200 patients (105 female, 95 male; mean age: 18.5 years; age range: 15–28), who were either interested in undergoing clear aligner treatment, currently receiving treatment, or had completed treatment, were included in the study. Each patient was asked to propose five questions they might search for online regarding clear aligner treatment. From the collected responses, the 29 most frequently asked questions were selected. Additionally, four orthodontists with varying levels of professional experience (13 years, 2.5 years, and 4 years) contributed 14 additional questions that were deemed important but not previously mentioned by patients. These additions enhanced the comprehensiveness of the question set. The identified 43 questions were categorized into four subheadings: 7 related to the usage of the appliances, 16 concerning side effects/complications, 10 addressing treatment limitations, and 10 classified as others (Table 1). The questionnaire used to collect patient-generated questions was specifically developed for this study and has not been previously published. (Suppl. 1) Collection of Questions via Chatbot Questions were posed to ChatGPT 4o by the same author without providing any additional commands. The questions were asked on the 24th November 2024, and the responses were recorded exactly as they were generated, without any further modifications or prompts. Evaluation Criteria Six different evaluation criteria were used to assess the responses to the 43 selected questions from the chatbot. The Flesch-Kincaid Reading Grade Level (FKRGL) index is a scoring system used to measure the readability or complexity level of a text. This index provides a reading level based on criteria such as sentence length and the number of words. The FKRGL index is calculated using the following formula: 0.39 (Total Words/Total Sentences) + 11.8 (Total Syllables/Total Words) − 15.59 The higher the FKRGL index, the more complex and difficult the text is to read. An FKRGL index below 12 corresponds to a 7th-grade reading level or lower, while an index above 16 requires advanced reading skills (Table 2). The Flesch Reading Ease Score (FRES) evaluates the readability of text by analyzing factors such as sentence length and word complexity. This score measures the reading difficulty of English texts and is presented as a numerical value, with higher scores indicating easier readability (Table 2). The Effective and Quality Information for Patients (EQIP) scoring tool was used to assess the quality and validity of the text. This tool consists of 36 questions. The first 14 questions address the general characteristics of the information, while the remaining questions focus on disease, procedure, or medication details. Each question offers four response options: "yes," "partly," "no," and "not applicable." The EQIP score is calculated using the following formula: EQIP Score = (("yes" x 1 + "partly" x 0.5) / (20 - "not applicable" x 1)) x 100 In this formula, each question carries equal weight, resulting in an EQIP score that reflects the document’s quality and effectiveness in healthcare communication. The DISCERN scoring system was employed to evaluate the quality of the text using 15 specific questions. It was designed to assess the accuracy and reliability of health-related information available on the internet. Additionally, the Global Quality Score (GQS) was used to rate the usefulness of ChatGPT's responses on a five-point scale (Table 2). To determine the level of plagiarism in the responses, similarity scores were calculated using a plagiarism-detection tool ( http://www.ithenticate.com ). Each response was evaluated individually, with similarity percentages assessed across four levels: 0–10% (high originality), 10–20% (acceptable similarity), 20–40% (high similarity), and above 40% (very high similarity). The scoring process was conducted by two independent researchers. Initially, the authors calibrated their assessments by agreeing on scores for 10 orthodontics-related questions. These agreed-upon questions were posed to chatbot, and the responses were scored independently by both researchers. Discrepancies between their assessments were identified and resolved through discussions, ensuring consensus was reached for all evaluations. Statistical Analysis The statistical analyses of the study were performed using SPSS V.22. Descriptive statistics of the data were reported as mean, standard deviation, median, minimum, and maximum. Normality distributions of the data were assessed using the Shapiro-Wilk test, which indicated that the data did not follow a normal distribution. Consequently, non-parametric tests were applied. The correlation between index scores was analyzed by calculating Spearman correlation coefficients. Differences between question groups were evaluated using the Kruskal-Wallis test; however, since no significant differences were found, post-hoc analyses were not performed. The top 10 most frequently asked questions by patients were listed, and descriptive statistics of the indexes were calculated. Additionally, descriptive statistics for each question group were determined. The Mann-Whitney U test was used to compare the most frequently asked questions with other questions. Linear regression analysis was performed to evaluate relationships between the indexes. The level of statistical significance was set at p < 0.05. Results Descriptive data for the evaluation scales and scores, grouped by different question categories, are presented in Table 3. The highest FKGL score was observed in the usage-related questions (14.53 ± 8.88), while the highest FRES score was found in the other question group (54.30 ± 0.89); the lowest score was observed in the usage group (40.97 ± 9.98). DISCERN, EQIP, and GQS scores were highest in the usage and side effect/complication groups; in the other group (81.83 ± 7.34) and the usage group (4.43 ± 0.535), respectively. When comparing question groups, differences in all scores were found to be statistically insignificant (p > 0.05). Correlation analysis revealed low correlations between the scores. The highest correlation was found between similarity and FKGL (r = -0.391, p = 0.010), followed by the correlation between DISCERN and FKGL (r = -0.354, p = 0.020) (Table 4). For the top 10 most frequently asked questions, the average FKGL score was 13.37 ± 4.37, and the average FRES score was 47.18 ± 23.69. The average EQIP score was 79.89 ± 3.63, while the GQS and DISCERN scores were 3.8 ± 0.91 and 3.5 ± 0.84, respectively. No significant differences were found between the top 10 questions and other questions across all scoring systems (p > 0.05) (Table 5). Regression analysis did not identify any significant relationships between the FRES, FKGL, and EQIP scores (p = 0.957; p = 0.082, respectively). Additionally, no significant relationship was found between EQIP and similarity based on regression analysis. Discussion Is artificial intelligence a friend or a foe? While there is no definitive answer to this question, the undeniable reality is that artificial intelligence is an inevitable part of our future. Understanding it well and being proactive about its effects on medical processes is a responsibility that falls upon healthcare professionals. A total of 43 questions about clear aligner treatment were posed to ChatGPT 4o, an artificial intelligence-based chatbot increasingly used in various aspects of medicine. The responses were evaluated using several scoring systems based on criteria such as readability, level of explanation, and quality of information in medical texts. For readability and explanation level, the FRES and FKGL scoring systems were employed, which quantify text features numerically. Chatbot provided responses with a narrative level suitable for university-level readers, though the readability was classified as 'difficult.' Further evaluation across different quality parameters offered a comprehensive perspective on information quality and examined the relationships between various dimensions of quality standards. Three distinct scoring systems were used for this purpose, designed to evaluate patient information brochures, educational materials, and written communication in healthcare. These systems not only assessed accuracy but also considered the meaningful flow of information, comprehensibility, systematic design, and suitability for patient understanding. Across all scoring systems, chatbot was found to provide high-quality information. The responses exhibited very low plagiarism rates, reflecting the software's ability to synthesize insights from diverse sources. In this study, the questions were categorized into four groups: usage, side effects/complications, treatment limits, and others. The highest FRES score, indicating readability, was observed in the usage group, while the highest FKGL score, reflecting the level of explanation, was noted in the side effects/complications group. These findings suggest an inverse relationship between explanation level and readability, with more detailed explanations being more difficult to read. Notably, no correlation was found between FRES and FKGL scores. Regarding information quality, the highest scores were observed in the usage category. This may reflect the greater availability of data and higher internet search volumes for topics related to appliance usage. Analysis of the top 10 most frequently asked questions revealed no significant differences in comparison to general questions, suggesting that differences in information quality may arise from variations in user preferences and search behaviors. Correlation analysis showed minimal relationships between different indices. An increase in information quality, typically expected to correlate with better explanation levels, was not observed. This lack of correlation may stem from each scoring system evaluating distinct aspects of information quality. Similarly, regression analysis identified no significant relationships among the FRES, FKGL, and EQIP scores or between EQIP scores and plagiarism similarity. Several studies have investigated chatbot’s performance in healthcare contexts. For instance, Sarah Abu Arqub et al. analyzed responses to 111 questions, reporting that 58% of answers were correct, 15% were incorrect, and no scientific references were provided. In contrast, this study emphasizes the comprehensibility, design consistency, and meaningful information flow rather than focusing solely on scientific accuracy [ 16 ]. Another study by Orlando Motohiro Tanaka et al. evaluated ChatGPT responses to orthodontic questions using a Likert scale. They found 71% of answers rated as 'very good' and 15% as 'good.' However, Likert scales rely on subjective evaluation, whereas this study employed objective numerical scoring systems, enhancing reproducibility and consistency [ 15 ]. Duran et al. explored the quality of information related to cleft lip and palate (CLP) and found similar challenges in readability and information quality. Despite differences in target populations, these findings align with the results of this study, reinforcing the importance of evaluating chatbot responses in specialized contexts [ 13 ]. Clear aligners were chosen as the focus of this study due to their increasing relevance, rising demand, and integration with internet-based searches. ChatGPT 4o demonstrated the capability to provide quick access to structured and comprehensible information, though its responses may lack the depth and personal engagement offered by healthcare professionals [ 17 , 18 ]. The potential of AI-based tools to complement clinical care necessitates further exploration to optimize their application in healthcare settings [ 19 ]. Conclusion The responses provided by chatbot to questions about clear aligners were identified as challenging to read but suitable for university and postgraduate-level comprehension. The quality and accuracy of the information were deemed high. Given the growing demand for clear aligners, assessing the quality and reliability of chatbot-generated responses across various clinical topics is crucial. Such evaluations will facilitate the integration of new technologies into doctor-patient collaboration, ultimately enhancing healthcare communication and decision-making.
