This study aimed to determine the attitudes of Indian medical students toward "AI in medicine," their support for structured AI instruction, and their views on its ethical implications. Methods: This was a cross-sectional study conducted over one month, from March 7th to April 6th, 2025, at ESIC Medical College, Kalaburagi , among medical students. Universal Sampling was used, and the study population was medical students from the college. Out of 550 eligible students, 321 participated . Data was collected using a predesigned, pretested, semi-structured questionnaire with sections covering general information, perception (10 questions, 5-point Likert scale), practices (10 questions), and the future of AI in medical education (6 multiple-choice questions, 5-point Likert scale). Results: The majority of participants were female (52.3%) and aged 18-20 years (54.2%). A large portion of students used technology for educational purposes either Always (40.5%) or Often (37.7%) . Online resources (63.6%) and Textbooks (72.3%) were the primary sources of information. 81% of students reported using AI , with ChatGPT (84.9%) and Meta AI (53.7%) being the most used platforms. Students primarily learned about AI tools through friends (48%) and social media (42%) . In terms of perception , 96.9% agreed that AI can enhance the quality of medical education by providing personalized learning experiences, and 81.6% viewed AI as a powerful tool for teaching-learning . 78.9% felt dedicated AI tools should be created for medical education. For academic purposes , AI was mainly used for assignment writing (66.8%) , solving questions (59.8%) , and summarizing topics (56%) . Concerning the future of AI , 88.7% agreed that ethical training on AI use should be mandatory in the curriculum, and 81.2% believed AI would play a central role in training students to diagnose diseases accurately. Conclusion: The study demonstrates that a large proportion of medical students actively use AI in their studies and have a positive perception of its potential to personalize learning and reduce study time. However, the findings also highlight a need for structured training on AI, as many students felt they were not properly trained and were concerned about the lack of human interaction and the risk of inaccurate information. Therefore, the study recommends the creation of dedicated AI tools for medical education and mandatory ethical training on AI use. Figures Figure 1 Figure 2 1. Introduction Artificial Intelligence is defined as "the science and engineering of making intelligent machines". AI and its associated technologies are increasingly integrated into human life and are beginning to impact the healthcare industry. AI was conceived in the 1950s with the Dartmouth Summer Research Project and has evolved from the rule-based expert systems of the 1980s to the current capabilities driven by machine learning and deep learning in the 21st century. AI is recognized for its role in enhancing medical education and is viewed as a transformative technology that can better prepare students for real-world healthcare challenges. Multiple studies have explored the attitudes of medical students towards AI. For instance, a 2025 study in Pakistan found favourable attitudes (80.3%) toward AI, with the majority considering it an effective (60.8%) and credible (58.4%) learning tool. These participants also found AI helpful for grasping medical concepts compared to traditional tools (65.7%) and expressed a desire for AI integration into the medical field (80%). A 2024 study in Kerala, India, reported that 74.8% of medical students favoured the need for structured training on AI applications. Given the growing recognition of AI's significance, this study's primary objectives were: To assess the perception of AI among medical students in medical education. To assess the practices of AI among medical students. To determine how Indian medical students felt about the ethical implications of AI and their support for structured instruction on AI applications during their undergraduate studies. 2. Methodology This was a cross-sectional study conducted over one month, from 7 March to 6 April 2025. The study area was ESIC Medical College, Kalaburagi , among medical students. The sampling method used was Universal Sampling , and the study population was medical students from ESIC Medical College. The initial sample size was 550, and after exclusions (38 for research group/pilot testing and 191 for non-consent/non-completion), the final sample size was 321 participants . Inclusion criteria included all medical students from 1st year to final year who consented to participate. Data were collected using a predesigned, pretested, semi-structured questionnaire . The questionnaire consisted of four sections : General information of the participants (8 questions), Perception of AI in medical education (10 questions), Practices of AI in medical education (10 questions), and Future of AI in medical education (6 multiple choice questions). The perception questions were evaluated based on a 5-point Likert scale, ranging from 1-strongly disagree to 5-strongly agree 3. Results 3.1. General Characteristics and Technology Use A total of 321 responses were received. The study population consisted of 47.7% male and 52.3% female students. The dominant age group was 18–20 years (54.2%) . The student population was primarily composed of 1st Year (40.2%) and 2nd Year (29.6%) students. A large majority of students used technology for educational purposes, reporting usage as Always (40.5%) or Often (37.7%) . The primary sources of information for learning were Textbooks (72.3%) and Online Resources (63.6%). Only 6% of the study population had taken AI-related training, indicating that 94% had not received prior AI training . Perception of AI Students showed a highly positive perception of AI, with 96.9% (31.2% strongly agree, 65.7% agree) believing AI can enhance the quality of medical education by providing personalized learning experiences . A combined 81.6% felt AI is a powerful tool for teaching-learning. Furthermore, 76.3% agreed that AI tools make it easier to understand complex medical concepts, and 62% felt AI tools will help to cut down study time. However, 68.2% agreed that AI-based education lacks the human interaction necessary for effective learning. Also, 42.1% were concerned that AI might provide inaccurate information in medical education. Over three-quarters of the students (78.9%) felt that dedicated AI tools should be created for Medical Education . Practices of AI Eighty-one percent (81%) of the study population reported using AI. Among the 259 AI users, the most common platforms were ChatGPT (84.9%) and Meta AI (53.7%) . The source of learning AI tools was primarily from friends (48%) and social media (42%) . For academic purposes, AI was mainly used for Assignment Writing (66.8%) , Solving Questions (59.8%) , and Summarizing Topics (56%) . For non-academic purposes, the main uses were Learning new skills (48.3%) and Fitness and Diet (38.2%). Challenges faced by users included: specific answers are not given (n = 22), lack of pictorial representation (n = 16), and doubt the reliability and accuracy (n = 12). Future of AI and Association A strong majority (81.2%) believed that in the future, AI will play a central role in training medical students to diagnose diseases accurately . Moreover, 88.7% (14.3% strongly agree, 51.7% agree) agreed that Ethical training on AI use should be mandatory in the medical curriculum. Overall, 62.5% of students are optimistic about the future of AI in improving medical education. The association table showed that AI usage was not significantly associated with gender (P = 0.68) or age (P = 0.393). However, there was a statistically significant association with the Year of Study (P = 0.027) . Usage was highest among 2nd-year students (87.3%) and lowest among 1st-year students (75.2%). Discussion The study demonstrates a strong existing practice of AI use (81%) among medical students, with usage concentrated on academic tasks like assignment writing and summarization. The belief that AI provides personalized learning experiences (96.9%) and reduces study time (62%) aligns with favourable attitudes found in similar studies. However, the critical finding is the reliance on informal learning through friends and social media (48%, 42%) despite a near-total lack of formal training (94% reported no training). This gap likely drives the concerns regarding AI's accuracy (42.1%) and the perceived lack of human interaction (68.2%). The significant association between the year of study and AI use (P = 0.027) suggests that familiarity with the curriculum and tools increases adoption, with 2nd-year students showing the highest usage . The overwhelming support for mandatory ethical training (88.7%) confirms that students recognize the necessity for responsible governance and instruction to prepare for AI's future central role in diagnosis (81.2%). Conclusion The study concludes that medical students are actively using AI and have a highly positive perception of its role in enhancing medical education. To address the risks associated with informal learning, the findings underscore the need for structured AI training and mandatory ethical instruction to be incorporated into the medical curriculum. This study aimed to determine the attitudes of Indian medical students toward \"AI in medicine,\" their support for structured AI instruction, and their views on its ethical implications.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods:\u003c/strong\u003e This was a \u003cstrong\u003ecross-sectional study\u003c/strong\u003e conducted over one month, from March 7th to April 6th, 2025, at \u003cstrong\u003eESIC Medical College, Kalaburagi\u003c/strong\u003e, among medical students. \u003cstrong\u003eUniversal Sampling\u003c/strong\u003ewas used, and the study population was medical students from the college. Out of 550 eligible students, \u003cstrong\u003e321 participated\u003c/strong\u003e. Data was collected using a predesigned, pretested, semi-structured questionnaire with sections covering general information, perception (10 questions, 5-point Likert scale), practices (10 questions), and the future of AI in medical education (6 multiple-choice questions, 5-point Likert scale).\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e The majority of participants were female (52.3%) and aged 18-20 years (54.2%). A large portion of students used technology for educational purposes either \u003cstrong\u003eAlways (40.5%)\u003c/strong\u003e or \u003cstrong\u003eOften (37.7%)\u003c/strong\u003e. \u003cstrong\u003eOnline resources (63.6%)\u003c/strong\u003e and \u003cstrong\u003eTextbooks (72.3%)\u003c/strong\u003e were the primary sources of information. \u003cstrong\u003e81% of students reported using AI\u003c/strong\u003e, with \u003cstrong\u003eChatGPT (84.9%)\u003c/strong\u003e and \u003cstrong\u003eMeta AI (53.7%)\u003c/strong\u003e being the most used platforms. Students primarily learned about AI tools through \u003cstrong\u003efriends (48%)\u003c/strong\u003eand \u003cstrong\u003esocial media (42%)\u003c/strong\u003e.\u003c/p\u003e\n\u003cp\u003eIn terms of \u003cstrong\u003eperception\u003c/strong\u003e, 96.9% agreed that AI can \u003cstrong\u003eenhance the quality of medical education\u003c/strong\u003e by providing personalized learning experiences, and 81.6% viewed AI as a \u003cstrong\u003epowerful tool for teaching-learning\u003c/strong\u003e. \u003cstrong\u003e78.9%\u003c/strong\u003e felt \u003cstrong\u003ededicated AI tools should be created\u003c/strong\u003e for medical education. For \u003cstrong\u003eacademic purposes\u003c/strong\u003e, AI was mainly used for \u003cstrong\u003eassignment writing (66.8%)\u003c/strong\u003e, \u003cstrong\u003esolving questions (59.8%)\u003c/strong\u003e, and \u003cstrong\u003esummarizing topics (56%)\u003c/strong\u003e. Concerning the \u003cstrong\u003efuture of AI\u003c/strong\u003e, 88.7% agreed that \u003cstrong\u003eethical training on AI use should be mandatory\u003c/strong\u003e in the curriculum, and 81.2% believed AI would play a central role in training students to diagnose diseases accurately.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion:\u003c/strong\u003e The study demonstrates that a large proportion of medical students actively use AI in their studies and have a positive perception of its potential to personalize learning and reduce study time. However, the findings also highlight a need for \u003cstrong\u003estructured training\u003c/strong\u003e on AI, as many students felt they were not properly trained and were concerned about the lack of human interaction and the risk of inaccurate information.
