Uterine infarction in a patient with uterine adenomyosis following biochemical pregnancy

Clinicians suspected focal uterine infarction after CT showed a wedge-shaped low-attenuation area and pelvic contrast-enhanced MRI demonstrated an irregular non-enhancing wedge-shaped lesion in the anterior uterine wall with preserved myometrium between the endometrial cavity and the inner margin of necrotic myometrium, along with peritoneal inflammation. The patient improved with antibiotic therapy and red blood cell transfusion, and follow-up MRI two months later showed regression of the infarction site and improved blood flow; the authors also reported that thrombophilia-related labs (e.g., antiphospholipid antibodies, protein C/S, plasminogen) were within normal ranges.
