While preventive strategies are well documented, patient compliance with post-extraction instructions plays a critical role in reducing their incidence. In Pakistan, especially in Khyber Pakhtunkhwa, limited data exists on patient compliance to post-extraction instructions, making it difficult to assess their impact on postoperative outcomes. The study aims to assess Patient Compliance with post-extraction instructions in the prevention of Alveolar Osteitis and to determine the factors associated with it. Methods We included 303 patients who underwent extractions, either surgical or nonsurgical, at the maxillofacial department of Khyber College of Dentistry, using a non-probability consecutive sampling technique. The post-extraction instructions were provided verbally in accordance with guidelines in the local language. One week later, patients were contacted to gather information about their adherence to post-operative instructions through a questionnaire to assess the level of compliance. Using SPSS v 22, descriptive statistics were reported for all variables. Regression analysis was applied to find the strength of association between the independent variables and the level of compliance and dry socket. Results 51.2% were male and 48.8% were female. A total of 161 (53.1%) patients showed compliance (score 8 and above) with the instructions while 142 (46.9%) showed partial compliance (score 5–7). The regression analysis revealed that higher compliance with post-extraction instructions was associated with a lower odds ratio of developing a dry socket. The higher compliance variables are higher education and above, surgical extraction, impacted tooth and posterior tooth extraction, while the non-compliance variables are employed and self-employed. Regarding compliance with post-extraction instructions in the prevention of dry socket, results are statistically significant with P < 0.05. Conclusion The study found a strong correlation between compliance and factors such as education level, employed, operator status, posterior tooth extraction, impacted tooth and extraction technique. Higher compliance significantly reduced the risk of alveolar osteitis. Enhancing patient education and emphasizing adherence to post extraction instructions can effectively minimize post extraction complication. compliance non-compliance post-extraction instructions dry socket Alveolar osteitis Background Tooth extractions, i-e surgical or non-surgical, is one of the common procedures done in dentistry.( 1 ). It is a stressful event for patients before, during and after treatments( 2 ). Post-extraction instructions are given to each patient to relieve post-operative complications. One of the most common complications is alveolar osteitis, also called Dry Socket.( 3 )( 4 ), the inflammation of alveolar bone( 5 ) develops after 2 to 4 days, characterised by severe pain( 6 ), bad smell (halitosis)( 3 , 7 ), swelling and redness of the gingiva, open socket, not covered by a blood clot and exposed bone. Its occurrence is 1 to 5% in all extraction while in mandibular molar, 38%( 8 ). The reported incidence of dry socket in closed extraction is 0.6% to 5.6% while in open extraction is 24.7%( 9 ). According to dental arch, the incidence of dry socket in Mandible is more common than in the maxilla because of food debris accumulation into socket and presence of dense bone( 10 ). Most cases of dry socket occur in impacted teeth that are mainly mandibular, 3rd molars which are removed surgically ( 4 ). The post-extraction instructions act as a guideline to prevent post-operative complications that is given after surgical procedures ( 11 ). The healing process of the socket is disrupted ( 12 ) When there’s noncompliance towards post-extraction instructions. The primary purpose is to facilitate a more effective healing process of the socket after extraction ( 13 ). When these instructions are followed correctly, patients will have early recovery ( 14 ) and help in promoting wound healing after extractions( 9 , 15 ). To ensure compliance, post-operative instructions should be presented in a clear and understandable manner. Multiple studies indicate that non-compliant patients have a higher incidence of complications such as dry socket, which emphasises the importance of adherence to these instructions for successful recovery and healing ( 14 ). Patients who fully comply with the instructions during follow-up tend to have a satisfactory wound-healing process ( 11 ). Knowledge about post-extraction instructions is also one of the reasons to help in the prevention and management of dry socket( 13 ). There is negligible evidence regarding compliance in Pakistan, especially in Khyber Pakhtunkhwa, and inconsistency in following post-extraction instructions. With the above background, this study aims to generate evidence on compliance with post-extraction instructions. The objective of the study was to assess the patient compliance with post-extraction instructions and also determine the factors associated with non-compliance. Methods A cross-sectional study was conducted on patients who underwent tooth extraction at the Oral and Maxillofacial Department of Khyber College of Dentistry from September 2022 to February 2023. A non-probability (consecutive) sampling technique was used for a total of 303 patients, with an anticipated frequency of 73.1%, a 95% confidence level, and a 5% margin of error. Approval was obtained from the IPH&SS ethics committee No.KMU/IPHSS/Ethics/2022/PC/034 of KMU, as well as permission from the local administration. Ethical approval was also granted by the research committee at Khyber College of Dentistry for the collection of data under No. 8152/RRB/KCD. The study included adult males and females aged 18 years and older who underwent surgical or non-surgical extraction of a permanent tooth. The patient will be willing to listen to post-extraction instructions (takes around 8 minutes & max 10 minutes). Patients who were uncooperative, mentally disabled, or unable to understand and follow instructions were excluded from the study. After the approval from the hospital, patients who fulfilled the inclusion criteria were recruited, and informed consent was taken. A total of 303 participants were recruited. At the time of the extraction, demographic information such as age, gender, tooth to be extracted, and type of extraction (surgical or non-surgical) was recorded. At the end of the procedure, the post-extraction instructions were provided verbally in the local language, according to the standard guidelines for oral and maxillofacial surgery. Consent was obtained from all patients to review these guidelines. After one week post-extraction, patients were called for a reminder to follow up to obtain information. Those who didn’t come for the follow-up had data collected through a phone call using a pre-designed questionnaire. The questionnaire consisted of ten questions about compliance; each question had a corresponding score, which quantitatively assessed the level of compliance with post-extraction instructions. It’s a validated tool taken from a study of Amparo Aloy-Prosper( 16 ). The key outcome variable of interest was the level of compliance score (quantified out of 10) according to the validated questionnaire. The collected data were statistically evaluated using SPSS version 22. Descriptive statistics were reported for all variables. Mean and standard deviation were calculated for the continuous variable, and frequency and percentages were calculated for the categorical variables. Regression analysis was applied to find the strength of association between the independent variables and the level of compliance and dry socket. Results The total study sample was 303, in which 51.2% were male and 48.8% were female shown in Table 1 . Table 2 reported the level of compliance reported was 53.1% highly complied (8 to 10 score) and 46.9% partial compliance (5 to 7 score). Posterior teeth 261 (86.1%) were the main extracted teeth. Caries was one of the main reasons for extraction, with most teeth being nonsurgically extracted (89.1%). The mean and standard deviation of the compliance score recorded were 7.71 ± 0.8. Table 3 showed the highest followed instruction was pressing gauze for 30 mints, avoid rinsing, take a soft diet and maintain proper hygiene. Table 4 reported Patients with a higher education and above (OR: 2.6, 95% CI: 1.5–4.7, p < 0.05), surgical extractions (OR: 7.6, 95% CI: 2.6–22.1, p < 0.05), and posterior tooth extraction (OR: 7.2, 95% CI: 1.6–32.0, p < 0.05) have higher odds of compliance in both univariate and multivariate regression but impacted tooth and those who have extractions done by Consultants have higher odds of compliance in only univariate regression. The employed and self-employed in the occupation category reported lower odds of compliance with post-extraction instructions only in the multivariate regression analysis. Table 5 showed the regression analysis for dry socket, showing that an impacted tooth has higher odds of developing dry socket, with a significant P value of < 0.05 in univariate analysis. In multivariate regression, the odds of developing dry socket are higher in surgical extractions and lower in those who demonstrate higher compliance with post-extraction instructions. Discussion This study assessed patient compliance with post extraction instructions aimed at preventing alveolar osteitis in a sample of 303 patients. The findings showed that more than half of the participants demonstrated compliance, with the mean score of 7.71 ± 0.8. The incidence of alveolar osteitis were 5.6%, which is lower than reported in other studies. Compliance were highest for instructions related to gauze compression, soft diet and maintain oral hygiene while the lowest were observed for mouthwash use, antibiotic intake and application of ice packs. The reason for not applying ice wrapped cloth on the face and taking antibiotics that not every patient were treated by surgical extraction, no need of antibiotics, and has no swelling over the face. Education level were significantly associated with compliance while demographic variables such as age and gender showed no significant effect. In this study, education level played an important role in compliance, with highly educated patients demonstrating significantly better adherence compared to low education level of participants. This align with the findings from Blinder et al, who emphasized the importance of reading and comprehension skills, though they reported no direct link between education and compliance( 2 ). A study from Saudi Arabia also revealed that higher education enhance understanding of postoperative instructions and reduces complications ( 17 ). The mean compliance score reported here (7.71 ± 0.8) is comparable to Spanish study by Aloy-Prosper, which reported slightly a higher compliance (7.9 ± 2.1) with written instructions, suggesting that mode of instruction delivery (verbal vs written) may influence patient adherence ( 16 ). Another Spanish study by Gonzalez et al, demonstrated very high compliance with mouthwash use (98.%), whereas in this study, compliance with mouthwash were only 28.7%, reflecting a lack of awareness about its use in local population ( 18 ). The highest compliance were also seen with the instructions such as pressing gauze for 30 minutes, avoiding rinsing for 24 hours, and consuming soft diet while the lower compliance was reported for using mouthwashes and antibiotics, which was consistent with the studies conducted in Nigeria ( 19 ), and Malaysia( 11 ). Verbal instructions were effective in improving recall, as also reported by ET Adebayo. This suggests that simple, clear, and repeated verbal communication can enhance patient understanding especially in sitting with limited literacy ( 20 ). Dry socket incidence in this study were significantly lower (5.6%) compared to studies conducted in Karachi, Pakistan is 19.7% ( 21 ) and India reported 14.1% ( 1 ), reflecting improved patient awareness and oral health practices in our setting. The lower incidence may also showed better patient education, increased acceptability of dental care, or improvements in postoperative instruction delivery. Dry socket in our study were primarily linked to partial compliance, smoking habits, medical history, surgical extractions and posterior tooth extractions. Similar findings were reported in Nigerian study, where poor adherence to instructions increased postoperative complications( 22 ). A multicentre study in Ethiopia also showed a strong link between verbal instructions noncompliance and postoperative complication, reinforcing the importance of effective communication of instructions( 23 ). Overall, our data suggest that while dry socket is multifactorial, compliance remains a key preventive factor. The study revealed the noteworthy demographics, that male patients underwent more extractions, similar to findings from India( 1 ), which attributed this trend to limited self-care among males. Female (housewives) showed lower compliance consistent with findings from Malaysian study ( 11 ) often due to domestic responsibilities. Employed participants reported better compliance than unemployed individuals, due to more proactive nature to take care of themselves. Those who have impacted tooth, surgical extraction, posterior tooth location, and extraction done by consultants showed good compliance with instructions because of more focused towards their oral health. This study underline the importance of tailoring postoperative care strategies to patients’ lifestyle. The study also found that patients easily recalled initial instructions (e-g gauze compression for 30 minutes, avoid rinsing and soft diet), reflecting the emphasizing of key messages immediate post-surgery. Similar observations from Saudi Arabia, where initial instructions recall were highest( 14 ). A major strength of this study is its relatively large sample size (303 participants) and its focus on verbal instructions, which reflects real world practice in low and middle income countries. The use of validated questionnaire allowed for robust quantification of compliance. However, limitations include reliance on self-reported compliance and potential recall bias during follow-ups. Variations in socioeconomic and cultural contexts also limit generalizability. Despite these limitations, the study provide valuable insight in compliance behaviors and their relationship with post-operative complication. Conclusion The current study concluded that compliance with post-extractions instructions was significantly associated with surgical instructions, operator status (consultants), impacted teeth and posterior teeth extractions. Regression analysis revealed that higher compliance was linked to lower odds of developing dry socket, emphasizing the importance of adherence to postoperative guidance. Partial compliance was influenced by factors such as age, gender, smoking, medical history and occupation. Instruction specific analysis showed the highest compliance with maintaining soft diet, placing a gauze pack, avoid rinsing and practicing oral hygiene, while the lowest compliance was with the use of antibiotics, mouthwash, and ice packs for swelling. While preventive strategies are well documented, patient compliance with post-extraction instructions plays a critical role in reducing their incidence. In Pakistan, especially in Khyber Pakhtunkhwa, limited data exists on patient compliance to post-extraction instructions, making it difficult to assess their impact on postoperative outcomes. The study aims to assess Patient Compliance with post-extraction instructions in the prevention of Alveolar Osteitis and to determine the factors associated with it.\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e\u003cp\u003eWe included 303 patients who underwent extractions, either surgical or nonsurgical, at the maxillofacial department of Khyber College of Dentistry, using a non-probability consecutive sampling technique. The post-extraction instructions were provided verbally in accordance with guidelines in the local language. One week later, patients were contacted to gather information about their adherence to post-operative instructions through a questionnaire to assess the level of compliance. Using SPSS v 22, descriptive statistics were reported for all variables. Regression analysis was applied to find the strength of association between the independent variables and the level of compliance and dry socket.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003e51.2% were male and 48.8% were female. A total of 161 (53.1%) patients showed compliance (score 8 and above) with the instructions while 142 (46.9%) showed partial compliance (score 5\u0026ndash;7). The regression analysis revealed that higher compliance with post-extraction instructions was associated with a lower odds ratio of developing a dry socket. The higher compliance variables are higher education and above, surgical extraction, impacted tooth and posterior tooth extraction, while the non-compliance variables are employed and self-employed. Regarding compliance with post-extraction instructions in the prevention of dry socket, results are statistically significant with P\u0026thinsp;\u0026lt;\u0026thinsp;0.05.\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e\u003cp\u003eThe study found a strong correlation between compliance and factors such as education level, employed, operator status, posterior tooth extraction, impacted tooth and extraction technique.
