Abstract
Childhood-onset systemic lupus erythematosus (cSLE) often presents with severe disease and frequent lupus nephritis (LN). While standard immunosuppressive therapies are effective for most patients, some develop refractory cutaneous vasculitis, bullous lesions, or neutrophil-driven inflammation. Dapsone, a sulfone with anti-neutrophilic and anti-inflammatory properties, has been used off-label in pediatric SLE with encouraging results. Case reports and small series show improvement in vasculitic and bullous manifestations and allow steroid tapering without worsening renal function. Its benefits in LN are largely indirect, mediated through reduction of neutrophil-mediated vascular injury. Safety concerns—including hemolysis, methemoglobinemia, and hypersensitivity—require careful monitoring. Although evidence remains limited, dapsone may serve as a practical adjunct for selected children with refractory SLE.
