Actue Isolated Cilioretinal Artery Occlusion Secondary to Percutaneous Coronary Intervention: A Case Report

Abstract
Abstract Background: Actue isolated cilioretinal artery occlusion (CLRAO) is a very rare, urgent ocular disease, which characterized by well-defined retinal infarct in the region of the occlusive cilioretinal artery sparing the central retinal artery. The aim of this article is to describe in detailed regarding the acute isolated CLRAO in a Chinese man related to a complicated therapeutic percutaneous coronary intervention (PCI).Case presentation: A 68-year-old Chinese man presented with a sudden, sharp chest pain that had commenced 2 days prior. He had coronary atherosclerotic heart disease for decades. Coronary angiography showed severe stenosis of the right coronary artery and the left anterior descending coronary artery. Then he was treated with PCI. However, the patient complained of a sudden decrease in visual acuity in the right eye following the PCI. His best-corrected visual acuity (BCVA) was hand motion in the right eye. Funduscopic examination revealed retinal infarct, retinal oedema, and cotton-wool spots in the right eye. Optical coherence tomography angiography (OCTA) showed the decreased flow sign in the superficial and deep retinal layers. Based on the findings, the patient was diagnosed with acute isolated CLRAO definitely.Conclusions: This is the first report to put forward an impressive and detailed description of acute isolated CLRAO secondary to a complicated therapeutic PCI. The visual prognosis of untreated and delayed patients was poor. Suitable management and prevention is essential for the interventional cardiologist to prevent the complications.
