Association Between Myopia and Dental Caries of 385844 School-Aged Children and Adolescents in China: A large scale Cross-Sectional Study

Abstract

Abstract Background: Prevalence of dental caries is increasing among children and adolescents, which can delay growth and development. This study aimed to determine the association between dental caries severity and myopia in a large-scale school-based population in China. Methods: The data from the School-Based Chinese Adolescents Health Survey (SCAHS) were collected for all children and adolescents aged from 6 to 18 years students in Zhejiang Province of China from 2020 to 2023. Multinomial logistic regression models were used to estimate the association between dental caries severity and myopia. Results: All children and adolescents with severe dental caries in primary school (OR = 0.72, 95% CI: 0.50-0.94), middle school (OR = 0.86, 95% CI: 0.79-0.94), and high school (OR = 0.80, 95% CI: 0.67-0.95) were less likely to have high myopia compared to those with no dental caries after adjusting for age, gender, family structure, academic pressure, regional classification, regional economic level, environmental support, frequency of eating sweets, frequency of eating fried foods, alcohol consumption, smoking status, internet usage time, sleep duration, weight status, parental myopia, outdoor activity time, eye usage habits. Conclusion: Our study found a significant inverse relationship between severe dental caries and high myopia among chinese students, with the strength of the association varying across different educational stages. These results highlight the potential for integrated public health interventions targeting students with low levels of dental caries but at high risk of myopia.

Introduction In recent years, myopia has become a rapidly growing public health issue worldwide. It is projected that by 2050, approximately 49.8% of the global population will be affected by myopia[ 1 ], with China expected to experience a particularly high prevalence rate—51.9% among children and adolescents in 2022[ 2 ]. Myopia not only impacts children's learning and quality of life, but it can also lead to serious complications such as retinal detachment, glaucoma, and even permanent blindness[ 3 ]. Concurrently, dental caries are among the most common oral diseases in school-aged children, affecting approximately 60%-90% of children globally[ 4 ]. In China, the prevalence of dental caries among children and adolescents was 48.11%[ 5 ]. Dental caries not only impact oral health but can also result in pain, malnutrition, decreased school performance and delayed growth and development[ 6 ]. Therefore, the prevention of both myopia and dental caries has become a critical public health challenge in children and adolescents. Although there is no clear evidence that myopia and dental caries are dependent in their pathological mechanisms, they may share certain environmental and behavioral risk factors, such as high-sugar diets, lack of outdoor activities, and prolonged screen time[ 7 , 8 ]. These common risk factors may promote occurrence of both myopia and dental caries through physiological pathways such as glucose metabolism[ 9 ] and oral microbiota metabolism[ 10 , 11 ]. However, existing studies on the direct relationship between myopia and dental caries remain inconclusive. Studying the relationship between dental caries and myopia can provide insights into common behavioral or lifestyle factors influencing both conditions, aiding in the development of more precise prevention strategies. Several investigations[ 12 , 13 ] demonstrated significantly elevated caries risk in myopic children, while other studies [ 14 – 16 ] reported no statistically significant associations. Nevertheless, there was still a lack of large-scale population studies on the link between myopia and dental caries in school-aged children. This study aims to examine the association between myopia and dental caries in a large dataset from the School-Based Chinese Children and Adolescents Health Survey (SCCAHS), which includes all children and adolescents aged 6 to 18 years in the Zhejiang province, China from 2020 to 2023, and to provideevidence basis for the integrated prevention and control of myopia and dental caries in children and adolescents. Methods Study Design and Participants This study was based on data from the School-Based Chinese Adolescents Health Survey (SCAHS), which investigated common diseases and health-related behaviors among adolescents in age 5 to 21years old from 2020 to 2023 in Zhejiang Province of China. A total of 482,650 children and adolescents from 177 schools across 11 cities and regions participated in the SCAHS. In this analysis, we excluded 96,806 participants with missing data on dental caries, myopia, height and weight. The final sample consisted of 385,844 participants. All participants’ data wereanonymized before analyses. In accordance with the principle of voluntary participation, all participants and their legal guardians in this study provided written informed consent. Ethical approval was obtained from the Ethics Review Committee of the Zhejiang Provincial Center for Disease Control and Prevention. This study adhered to the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. Myopia We defined myopia as a spherical equivalent (SE) of -0.50 diopters (D) or less. Low myopia was defined as an SE between − 0.50 D and − 3.00 D (exclusive of -3.00 D), moderate myopia as an SE between − 3.00 D and − 6.00 D (exclusive of -6.00 D), and high myopia as an SE of -6.00 D or less[ 17 ]. Dental Caries We evaluated decayed, missing, and filled teeth (DMFT) in accordance with World Health Organization (WHO) criteria (WHO, 2013). To quantify the severity of dental caries, the DMFT values were categorized into three levels: low (1–3), moderate (4–6), and high (>7)[ 18 ]. Covariates The covariates included in the model included age, gender, family structure, academic pressure, regional classification, regional economic level, environmental support, frequency of eating sweets, frequency of eating fried foods, alcohol consumption, smoking status, internet usage time, sleep duration, weight status, parental myopia, outdoor activity time, eye usage habits. Statistical analysis We described the baseline characteristics of children and adolescents by each myopia category in Table 1 . The differences in characteristics across normal, low, moderate, and high myopia were assessed using analysis of variance (ANOVA) for continuous variables and \(\:{\chi\:}^{2}\) tests for categorical variables. Prevalence estimates with 95% confidence intervals (CIs) were calculated and adjusted for the complex survey design and nonresponse. To examine the associations between dental caries and myopia, we used a multinomial logistic regression model with dental caries as the independent variable and myopia status as the dependent variable (crude model). Odds ratios (ORs) with 95% CIs were estimated. Model I adjusted for age, gender, family structure, academic pressure, regional classification, regional economic level, environmental support, frequency of sugar-sweetened beverage consumption, frequency of eating fried foods, alcohol consumption, smoking status, internet usage time, sleep duration, weight status, parental myopia, outdoor activity time, and eye usage habits, while Model II further adjusted for frequency of eating sweets. The heterogeneous effect of dental caries category was analyzed by school type. We used the statistical software R (version 4.2.3) to conduct all analyses. Results A total of 385,844 participants (mean age: 12.1 ± 3.5 years; 52.3% male) were included in this study. The prevalence of myopia was 72.9% (95% CI: 72.8–73.1%), with 41.4% classified as low myopia, 24.5% as moderate myopia, and 7.0% as high myopia. The proportion of participants with low, morderate, and severe dental caries was as follows: 37.1%, 14.8%, and 6.9%, respectively (Table 1 ). In the total sample, a graded inverse association was observed between dental caries severity and myopia. After adjusting for sugar-sweetened beverage consumption and other covariates (Model 2), severe dental caries remained significantly associated with reduced odds of high myopia [OR = 0.81, 95% CI: 0.71–0.92]. When stratified by school level, the inverse association between dental caries severity and high myopia persisted but varied in the magnitude across school level. In primary school, only severe caries significantly reduced high myopia risk (OR = 0.72, 95% CI: 0.50–0.94), while mild and moderate caries showed no significant associations. Among middle school students, moderate (OR = 0.84, 95% CI: 0.77–0.92) and severe caries (OR = 0.78, 95% CI: 0.61–0.98) were both significantly associated with a lower odd of high myopia. In high school students, even mild caries was associated with reduced high myopia risk (OR = 0.75, 95% CI: 0.69–0.82), with severe caries showing a similar protective trend.(Table 2 ) . Discussion This cross-sectional study found that greater dental caries severity was significantly associated with lower odds of high myopia among school children, while educational stage notably modified this association. Even after comprehensive adjustment for confounders, including sugar-sweetened beverage intake, severe dental caries was independently associated with a reduced likelihood of high myopia—a finding consistent across all school levels, though with varying magnitudes. Our study demonstrated that the severity of dental caries is inversely proportional to the severity of myopia, which was not consistent with previous reports. Some studies found no strong correlation[ 12 ], whereas a recent analysis in young adults found that active dental caries and advanced periodontitis were positively associated with Table 1 Several potential mechanisms might explain this inverse association. Firstly, dietary factors play a critical role. High sugar intake could increase bloodglucose levels and affect insulin and growth hormone secretion, which may increase Insulin-like Growth Factor-1 (IGF-1) levels[ 20 , 21 ]. When IGF-1 binds to the IGF-1 receptor (IGF-1R) on the cell membrane, it activates signaling pathways such as PI3K/AKT and RAS/MAPK kinases, influencing scleral remodeling and thereby increasing the risk of myopia[ 22 , 23 ]. Children with severe dental caries may adopt dietary changes to alleviate dental discomfort, reducing their sugar intake and, consequently, glycemic load[ 24 , 25 ]. Thus, behavioral adjustments linked to severe caries could inadvertently protect against myopia by limiting hyperglycemia-driven ocular changes. Secondly, the observed inverse relationship between dental caries severity and myopia risk may be partially mediated by socioeconomic and lifestyle-related behavioral patterns. Children from lower socioeconomic backgrounds, who are disproportionately affected by untreated dental caries due to limited access to dental care, may also face reduced exposure to near-vision activities associated with educational engagement [ 26 , 27 ]. Importantly, our study revealed notable age-dependent variations. In primary school children, only severe dental caries significantly lowered the risk of high myopia, whereas in middle and high school students, both moderate and severe caries showed protective effects. This result may be explained by that students in the high school stage might have experienced a longer period of influence from various lifestyle habits. Even in cases of mild or moderate dental caries, the dietary and lifestyle habits they represent (such as higher sugar intake, less outdoor activity, etc.) may have already had a long-term cumulative effect on axial length, ultimately leading to a stronger correlation with myopia. These lifestyle modifications could indirectly lower myopia progression risks, especially in older students who are typically exposed to intensive educational pressures[ 28 , 29 ]. Furthermore, parental differences in health awareness, educational investment, and socioeconomic status may simultaneously influence the occurrence of dental caries and myopia. Severe dental caries might reflect neglect of family oral hygiene and general health management, possibly correlating with lower academic stress, thus reducing the risk of myopia. Several limitations should be noted. Due to the cross-sectional design, causality cannot be inferred. Additionally, unmeasured confounders, such as genetic factors, socioeconomic status, and detailed nutritional intake, could potentially bias the results. Future research, particularly longitudinal studies incorporating objective biomarkers of diet, inflammation, and detailed behavioral assessments, is necessary to clarify the causal nature and underlying mechanisms of the observed associations. In conclusion, our findings highlight a robust inverse relationship between dental caries severity and high myopia in children and adolescents, with significant variations across age groups. Understanding this interplay could provide new insights into integrated public health interventions aimed at students with low levels of dental caries but at high risk of myopia among school-aged populations. When IGF-1 binds to the IGF-1 receptor (IGF-1R) on the cell membrane, it activates signaling pathways such as PI3K/AKT and RAS/MAPK kinases, influencing scleral remodeling and thereby increasing the risk of myopia[\u003cspan class=\"CitationRef\"\u003e22\u003c/span\u003e, \u003cspan class=\"CitationRef\"\u003e23\u003c/span\u003e]. Children with severe dental caries may adopt dietary changes to alleviate dental discomfort, reducing their sugar intake and, consequently, glycemic load[\u003cspan class=\"CitationRef\"\u003e24\u003c/span\u003e, \u003cspan class=\"CitationRef\"\u003e25\u003c/span\u003e]. Thus, behavioral adjustments linked to severe caries could inadvertently protect against myopia by limiting hyperglycemia-driven ocular changes. Secondly, the observed inverse relationship between dental caries severity and myopia risk may be partially mediated by socioeconomic and lifestyle-related behavioral patterns. Children from lower socioeconomic backgrounds, who are disproportionately affected by untreated dental caries due to limited access to dental care, may also face reduced exposure to near-vision activities associated with educational engagement [\u003cspan class=\"CitationRef\"\u003e26\u003c/span\u003e, \u003cspan class=\"CitationRef\"\u003e27\u003c/span\u003e].\u003c/p\u003e\n\u003cp\u003eImportantly, our study revealed notable age-dependent variations. In primary school children, only severe dental caries significantly lowered the risk of high myopia, whereas in middle and high school students, both moderate and severe caries showed protective effects. This result may be explained by that students in the high school stage might have experienced a longer period of influence from various lifestyle habits. Even in cases of mild or moderate dental caries, the dietary and lifestyle habits they represent (such as higher sugar intake, less outdoor activity, etc.) may have already had a long-term cumulative effect on axial length, ultimately leading to a stronger correlation with myopia. These lifestyle modifications could indirectly lower myopia progression risks, especially in older students who are typically exposed to intensive educational pressures[\u003cspan class=\"CitationRef\"\u003e28\u003c/span\u003e, \u003cspan class=\"CitationRef\"\u003e29\u003c/span\u003e]. Furthermore, parental differences in health awareness, educational investment, and socioeconomic status may simultaneously influence the occurrence of dental caries and myopia.
