This paper describes a single 44-year-old nulliparous woman with long-standing pelvic endometriosis who developed high-grade endometrioid adenocarcinoma arising from deep infiltrating endometriosis (DIE), presenting with elevated tumor markers and pelvic-only FDG-avid disease that later became visible on the posterior vaginal fornix. Using MRI/CT and surgical exploration, the authors found extensive deep invasion beneath the peritoneum into the rectovaginal septum and adjacent organs, including the uterus, vagina, rectum, bladder, ureters, and parametrial tissues, with renal failure attributed to ureteral involvement and incomplete resection leaving substantial tumor. The tumor histology confirmed both endometriosis lesions and carcinoma, with pathology concluding the cancer originated from DIE rather than ovarian endometriotic cyst continuity, and adjuvant concurrent chemoradiation with cisplatin and 5-fluorouracil after incomplete surgery was followed by survival without recurrence at 24 months.
