Abstract
††slugcomment: To be submitted to the Astrophysical Journal Letter Background: Cesarean section (C-section) is a critical obstetric procedure, but optimal rates are challenging to achieve globally. In Somalia’s fragile health system, data on C-section prevalence and associated factors are limited, hindering efforts to improve maternal health. This study investigates C-section rates and determinants using national survey data. Methods: : We analyzed data from 2,081 women aged 15-49 reporting a recent live birth in the 2020 Somalia Demographic and Health Survey (SDHS). Multivariate logistic regression identified factors associated with self-reported C-section delivery, calculating Adjusted Odds Ratios (AORs). Results: : C-section prevalence was 10.0%. Multivariate analysis identified significant associations. Lower odds of C-section were linked to age 25-34 (vs. 15-24), rural residence, higher education, higher wealth status, and parity 2-4. Conversely, attending ≥4 ANC visits was associated with higher odds. Significant regional variations were also present. Media access and breastfeeding status were not significantly associated. Conclusion: Somalia’s 10% C-section rate meets minimum recommendations but likely masks significant inequities influenced by geography, ANC use, and parity. The paradoxical finding of lower C-section odds among women with higher socioeconomic status demands context-specific investigation. Ensuring equitable access to medically indicated C-sections is crucial.
