Abstract
Paclitaxel, a potent chemotherapeutic agent, exhibits poor oral bioavailability due to its extensive metabolism by cytochrome P450 3A4 (CYP3A4) and active efflux by P-glycoprotein (P-gp). Dual modulation of these barriers is critical to enhance its systemic absorption via the oral route. Natural bioenhancers, particularly phytochemicals such as piperine, quercetin, curcumin, and resveratrol, have emerged as promising modulators of both CYP3A4 and P-gp. These compounds can inhibit the enzymatic activity of CYP3A4 and reduce P-gp-mediated efflux, thereby improving the pharmacokinetic profile of paclitaxel. This review explores the mechanistic roles of selected natural bioenhancers in modulating drug-metabolizing enzymes and efflux transporters. It also discusses current advancements in formulation strategies integrating these bioenhancers to develop efficient oral delivery systems for paclitaxel. The dual inhibition approach holds significant promise in overcoming the pharmacological barriers associated with oral chemotherapy, potentially reducing the need for intravenous administration and enhancing patient compliance.
