Infant injuries often occur at home, and many of these injuries are preventable ( 2 ). Caregivers’ education is particularly important considering that children spend most of their time at home as infants. Various interventions have been developed for caregivers, including home visits, hospital-based interventions, and smartphone apps, which have been shown to improve knowledge and preventive behaviors and reduce medical visits due to injuries ( 3 )( 4 ). In Japan, it has been recommended that public health nurses (PHNs) provide injury prevention information through existing maternal and child health services such as infant health checkups ( 5 ). Public maternal and child health services in Japan are widely available, highly accessible, and free of charge. Services, such as interviews when submitting pregnancy notification (pregnancy notification interview), prenatal education, postpartum home visits, and infant health examinations, are conducted by local governments and have high participation rates, providing ideal opportunities for the dissemination of injury prevention education about injuries that can occur to anyone at any time. As a result of the injury prevention education provided in the antenatal class, mothers who received this education were more likely to practice preventative behaviors postpartum and understand injury prevention awareness, severity, and probability of injury two years later ( 6 )( 7 ). Thus, educating caregivers promotes knowledge, awareness, and behavioral change. Utilizing existing maternal and child health services is an effective means of broadly disseminating injury prevention education. However, the COVID-19 pandemic significantly impacted maternal and child health services, and social distancing measures to slow the spread of COVID-19. This resulted in the suspension or reduction of public health services, particularly face-to-face care for mothers and children ( 8 ). Group health checkups and health guidance for infants and toddlers by local governments were canceled or postponed ( 9 ), and the cancellation of “giving birth at one's parent's home” was also necessitated ( 10 ). Public services and support from families were less than usual. PHNs have been diverted to COVID-19 measures, resulting in a shortage of human resources for maternal and child health services. The adverse side effects of withdrawing existing services are alarming ( 11 )( 8 ). Reduced or missed opportunities for health guidance and advice may reduce caregivers' access to injury prevention information and increase the risk of injury. During the COVID-19 pandemic, there were reports of an increase in emergency department visits due to inadequate supervision ( 12 ) and changes in the types of injuries sustained by children ( 13 )( 14 ). Even during the pandemic, it is important to provide injury prevention information to all caregivers and families without delay. The purpose of this study was to obtain suggestions for delivering injury prevention information to the general public in case of future pandemics. We identified the timing, methods of communication, and content of information on injury prevention desired by mothers during the COVID-19 pandemic. Furthermore, we aimed to identify the characteristics of mothers who experienced difficulties obtaining injury prevention information at critical junctures and their correlation with maternal and child health services. Methods A nationwide online survey was conducted from March 24–29, 2022, among registered monitors of Rakuten Research, targeting mothers raising children aged 0–2 years during the COVID-19 pandemic (February 2020-March 2022). The participants consented to the use of their responses in the study and completed an online self-administered questionnaire. The questionnaire included questions on mother/child/household demographics; injuries requiring hospital visits; preferences for the timing, content, and method of injury prevention information provision; utilization of prenatal and postnatal maternal and child health services; the General Health Questionnaire (Japanese version); ( 15 ) and childcare-related resilience ( 16 ). Injuries were identified as the most common cause (falls, accidental ingestion, suffocation, drowning, cuts/concussions, and burns) from the data on fatal injuries and emergency transport data for ages 0–2 years( 17 )( 18 ). The participants were asked whether they had experienced an injury requiring a hospital visit. Those with experience were then asked voluntarily about the age of their most recent medically attended injury and type of injury. Regarding preferences for injury prevention information, respondents were asked 1) when they would like to receive the information, with the following options: during pregnancy, one month after delivery, three to four months after delivery, after one year, or not at all; 2) what information they would like to receive, with the following options: frequent injuries by age and their prevention, general environmental maintenance, or individual environment; and 3) how they would like to receive the information. The options for ( 3 ) were information opportunities to date and non-personal services that have emerged or could be utilized as alternative services after COVID-19. Furthermore, we asked respondents about their preferred means of obtaining information, focusing on non-face-to-face media (LINE, YouTube, e-mail, local government websites, Twitter, Zoom, and Facebook). LINE is popular in East Asian countries and is the most popular chat application in Japan ( 19 ). Prenatal and postnatal maternal and child health services provided by local governments (pregnancy notification interview, regular infant and child health checkups, antenatal classes at public health centers or hospitals, vaccinations, and local workshops such as baby food classes) are important opportunities to provide injury prevention information. Participants were asked whether they could utilize these services without being hindered by COVID-19. Since "giving birth at the mother's parents' home" and "giving birth with a partner" are deemed as important prenatal and postpartum family support for mothers, participants were also asked whether they could implement these services without being impeded by COVID-19. As informal sources of information, participants were asked about whether they had “helpful parents or in-laws in the neighborhood” and “they received information from family or friends (yes: sometimes to very often, no: not often to not at all).” They were also asked about their use of parenting magazines, apps, and social networking sites. Resilience was measured using the Parenting Resilience Scale ( 16 ). Resilience is the ability to successfully adapt to adversity, such as trauma, tragic threats, and significant stress ( 20 ). The COVID-19 pandemic is a health-threatening risk and unprecedented adversity and is thought to have been one of the most important factors in the ability to adapt to it. In this study, we referred to the Childcare-Related Resilience Scale, which was developed for mothers caring for infants and toddlers. Resilience was measured by using a 5-point Likert scale ranging from “totally true” (5 points) to “totally false” (1 point). Descriptive statistics were used to analyze the injury prevention information needed for injury prevention by child’s age. Univariate logistic regression analysis was performed to assess the relationship between the dependent variable of "obtaining or not obtaining injury prevention information" and the independent variables of attributes and service use. In addition, a multivariate logistic regression analysis was performed to identify the variables that were statistically significant in predicting injury prevention information, with variables having a p-value below 0.05. This study was approved by the Research Ethics Committee of the Faculty of Medicine of the University of Tokyo (#2021369NI). The study explanation was displayed immediately before the main part of the questionnaire. Responses to the questionnaire were voluntary, and those who agreed to participate in the study selected "I agree to participate in the study" on the screen to begin completing the online self-administered questionnaire. Results A total of 675 respondents were analyzed, with 75 excluded due to exclusion criteria (lack of spouse/no response, multiple births) and incorrect entry of a second child (birth interval of fewer than 7 months). Descriptive statistics on the need for injury prevention information and experience of medically attended injuries were calculated based on the age of the child (Tables 1 and 2). Table 1. A total of 167 children (24.7%) experienced a medically attended injury. The most common causes of injury, in descending order, were falls, cuts, strikes, and tripping over. When asked when they first wanted to know about injury prevention information, more than half (52.7%) of the respondents answered that they were pregnant. Excluding 39 respondents who did not want to know about injury prevention, we asked 636 respondents what they wanted to know and how they wanted the information to be provided. The results showed that 87.4% of the respondents wanted to know about “Frequent injuries by age and their prevention” regardless of the child's age. The number of respondents who wanted to discern more about individualized environmental care significantly decreased with the age of the child, with only 36.6% of the total respondents expressing an interest in this topic. The most common method of providing information was through antenatal classes (50.7%), followed by magazines (43.6%), and stories from senior mothers (34.7%). When asked only about non-face-to-face tools, 73.2% of the respondents indicated a preference for using the mobile application LINE, with no differences by age. However, only 14.1% of mothers preferred individual consultations on LINE. YouTube (41.2%) was the next most popular choice, but the percentage of those who preferred this method significantly decreased with age. Table 3 shows the relationship between mother/child demographics and the use of maternal and child health services and information sources in relation to the response, “Did you get the injury prevention information you needed when you needed it?" A total of 217 (34.1%) mothers reported that they could not obtain injury prevention information when needed. Over 50% of the mothers could not attend antenatal classes at PHC or HP due to COVID-19. Of those who wished to have their partner present at birth, 40.5% were unable to do so because of COVID-19. A total of 16.7% did not receive parenting information from family or friends during the pregnancy of their first child, and 7.2% did not use parenting apps. Univariate logistic regression analysis revealed that being unemployed, the age of infant/child, infant/child having diseases requiring hospital visits, having a medically attended injury (any injury), medically attended injury (accidental ingestion), not having pregnancy notification interview, delays or unavailability of vaccination, not receiving information from friends or family, and higher GHQ and lower resilience scores were significantly associated with the response "did not receive information on injury prevention.” A multivariate logistic regression analysis with all significant variables was conducted, and the results remained significant for "not employed," "child 0-1 years old," and "child has a disease." Traditional face-to-face group education, such as antenatal classes and infant health checkups, was preferred, even among participants in this online survey. This may be because first-time mothers could not have images of specific injury prevention. Thus, they preferred to obtain an overall understanding through regular maternal and child health services or familiar parenting magazines. Although some studies have shown no difference in knowledge acquisition between face-to-face and non-face-to-face education( 21 ), it may be important to maintain conventional information provision opportunities with infection control measures in place depending on the target audience and offerings’ content. Subsequently, 34.1% of mothers reported that they could not obtain necessary information on injury prevention when needed. The COVID-19 pandemic reduced the availability of maternal and child healthcare services. The attendance rate of parent/mother classes in Tokyo was halved in 2020 compared to the previous year( 22 ), which may have resulted in the loss of educational opportunities provided by PHNs, which often include injury prevention information. This situation particularly affected mothers who were not employed, had children with illnesses requiring hospital visits and had children in the 0–1 year age group. Unemployed mothers did not go to work or childcare facilities, and with more than 70% of community childcare support programs such as "childcare squares” (kosodate hiroba) ( 23 ), they had nowhere to go. Mothers of children with pre-existing medical conditions and 0–1 year-olds may have been cautious about infection prevention and refrained from going out because their pregnancy and childbirth coincided with the pandemic. In particular, mothers of 1-year-olds born in April-May 2020, when declared a state of emergency and many services were suspended( 24 ), may have been hesitant to leave due to fear of unknown infections. Mothers of 0-year-olds would still have few friends with their children. These mothers likely had limited contact with the outside world and few natural opportunities to obtain information, which may have left them without a safety net when the public services were discontinued. In univariate analysis, mothers with no "information or advice from family or friends" were significantly more likely to report not receiving injury prevention information. To address this issue, a system must be established to provide injury prevention information to households with limited access to the outside world and few informal resources, even during the temporary suspension of face-to-face public services. The public service that should be considered a priority for continuation was the pregnancy notification interview. The pregnancy notification interview is the first contact between a pregnant woman and a PHN (or MW). PHNs or MW assess the support needed through an interview with pregnant women and provide information on available service ( 25 ). In Tokyo, most municipalities provided this service, even during the Covid-19 period. However, the implementation rate varied, with decreased rates in some cities ( 22 ). The disruption of this service might have discouraged mothers at the start of childbirth and childcare and negatively impacted their subsequent expectant life ( 26 ). The pregnancy notification interview should be continued during the pandemic while implementing infection control measures. In contrast, mothers who could obtain information had higher resilience scores. Those who can gather information and respond flexibly under unusual circumstances may have been able to access information. It may be necessary to provide multiple educational opportunities, including face-to-face and non-face-to-face, so that information can be obtained without requiring caregivers to be flexible and to reduce the hurdles to obtaining information. Mothers whose children experienced medically attended injuries were more likely to report that they could not obtain information. It is unclear whether this was due to a lack of information causing injuries or whether they felt that they lacked information because of injuries (reverse causality). In the analysis by type of injury, mothers whose children experienced medically attended injuries due to accidental ingestion were significantly more likely to report not obtaining information. Preventing accidental ingestion may require the most preventive knowledge as it involves understanding the types of objects that can be easily ingested and are within reach. Such information should be provided at an early stage to encourage the creation of a safe environment. In this study, 24.7% of mothers reported that their children had experienced medically attended injuries during the COVID-19 pandemic. While some have reported a decrease in overall pediatric ED visits and traffic accidents in the early years of COVID-19( 27 ), others have reported an increase in ED visits due to injuries caused by improper supervision( 12 ), an increase in stabbing injuries resulting from children playing with sharp objects at home, and injuries from collisions at home( 13 )( 14 ). This study could not address changes in the number of injuries before and after COVID-19. In terms of types of injuries, falls remain the most common cause of injury for children under age 2, followed by cuts and injuries from bumping into objects. A pandemic can also transform children's living environments, creating different risks. Therefore, injury prevention strategies should not be stopped. Finally, thirty-nine mothers indicated that they did not desire information on injury prevention. This number significantly increased with age; mothers of 2-year-olds were also significantly less likely to express interest in "individualized environmental maintenance." Given their expanded range of activities, 2-year-olds are the second most likely age group to be transported to the emergency room after 1-year-olds ( 18 ). Mothers are accustomed to childcare after two years of experience and may need more interest and awareness in prevention. More proactive interventions, such as push interventions, should be considered during this period. Limitations This survey was conducted with a group of people who had registered as monitors with an Internet research firm; thus, it is possible that this group had a predilection for non-face-to-face tools. While the respondents’ demographics, including age, educational level, and household income, approximated the national average, and the percentage of respondents' residential areas did not deviate from the national population ratio, the characteristic of being a registered monitor may have influenced the response results. Second, the survey was self-reported, which may have introduced a self-report bias regarding the respondents obtaining injury prevention information. Despite some limitations, this study offers suggestions on the use of public maternal and child health services and the needs of mothers to prevent some families from obtaining the necessary information during a pandemic. Conclusion Even during the pandemic, mothers wanted to learn about injury prevention information through traditional face-to-face services. More than 30% of the mothers reported that they could not obtain injury prevention information when needed during the pandemic. Being unemployed and having a child under one year of age or a sick child who needed to go to the hospital were at higher risk. It is also important for maternal and child health services to have a system that maintains as many opportunities as possible for pregnancy notification interview. In preparation for the next pandemic, it is necessary to maintain service provision with infection control measures and to consider alternative means of information and support so as not to create mothers who are cut off from information sources. Excluding 39 respondents who did not want to know about injury prevention, we asked 636 respondents what they wanted to know and how they wanted the information to be provided. The results showed that 87.4% of the respondents wanted to know about \u0026ldquo;Frequent injuries by age and their prevention\u0026rdquo; regardless of the child\u0026apos;s age. The number of respondents who wanted to discern more about individualized environmental care significantly decreased with the age of the child, with only 36.6% of\u0026nbsp;the total respondents expressing an interest in this topic. The most common method of providing information was through antenatal classes (50.7%), followed by magazines (43.6%),\u0026nbsp;and stories from senior mothers (34.7%).\u003c/p\u003e\n\u003cp\u003eWhen asked only about non-face-to-face tools, 73.2% of the respondents indicated a preference for using the mobile application LINE, with no differences by age. However, only 14.1% of mothers preferred individual consultations on LINE. YouTube (41.2%) was the next most popular choice, but the percentage of those who preferred this method significantly decreased with age.\u003c/p\u003e\n\u003cp\u003eTable 3 shows the relationship between mother/child demographics and the use of maternal and child health services and information sources in relation to the response, \u0026ldquo;Did you get the injury prevention information you needed when you needed it?\u0026quot;\u0026nbsp;A total of 217 (34.1%) mothers reported that they could not obtain injury prevention information when\u0026nbsp;needed.\u003c/p\u003e\n\u003cp\u003eOver 50% of the mothers could not attend antenatal classes at PHC or HP due to COVID-19. Of those who wished to have their partner present at\u0026nbsp;birth, 40.5% were unable to do so because of COVID-19.\u0026nbsp;A total of 16.7% did not receive parenting information from family or friends during the pregnancy of their first child, and 7.2% did not use parenting apps.\u003c/p\u003e\n\u003cp\u003eUnivariate logistic regression analysis revealed that being unemployed, the age of infant/child, infant/child having diseases requiring hospital visits, having a medically attended injury (any injury), medically attended injury (accidental ingestion), not having pregnancy notification interview, delays or unavailability of vaccination, not receiving information\u0026nbsp;from friends or family, and higher GHQ and lower resilience scores were significantly associated with the response \u0026quot;did not receive information on injury prevention.\u0026rdquo; A multivariate logistic regression analysis with all significant variables was conducted, and the results Public maternal and child health services provide ideal opportunities for disseminating injury prevention education. However, the COVID-19 pandemic significantly impacted these services, reducing access to injury prevention information. This study aimed to obtain suggestions for delivering injury prevention information to the public during future pandemics.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods: \u003c/strong\u003eIn March 2022, a nationwide Internet survey of mothers raising children ages 0-2 was conducted, asking whether they could obtain injury prevention information when required and when, what, and how they would like to receive it.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults: \u003c/strong\u003eOver half of the mothers wanted injury prevention information throughout their pregnancy. They preferred receiving information through traditional face-to-face services provided by local governments, such as antenatal classes or checkups. However, 34.1% of mothers said they did not obtain the information they needed, especially those unemployed, mothers of children aged 0 to 1, and mothers of children with illnesses requiring hospital visits. In public services, failure to be interviewed when submitting pregnancy notification was associated with failure to obtain injury prevention information.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusions: \u003c/strong\u003eIt is suggested that the interruption of public services prevented mothers, especially those with fewer opportunities to obtain informal information, from obtaining injury prevention information.
