Während insbesondere die ovarielle Endometriose das Risiko eines klarzelligen und endometrioiden Ovarialkarzinoms zweifach erhöht, die maligne Entartung der tiefinfiltrierende Endometriose tritt in seltenen Fällen auf.

Wir berichten über eine 52-jährige Patientin, die sich mit Unterleibsschmerzen, starker Dyschezie und Subileus-Symptomatik mit bekannter Residuum einer tief infiltrierende Endometriose des Spatium Rektovaginale präsentierte. In der Anamnese waren drei Laparoskopien, sowie eine Laparotomie mit unvollständiger Endometriosesanierung im Spatium rektovaginale. Bildgebend ergab sich der Verdacht auf eine ausgeprägte tief infiltrierende Endometriose retrocervikal sowie im Spatium rektovaginale mit Verdacht auf stenosierende Rektuminfiltration bis zur Submukosa von ca. 5cm kraniokaudaler Ausdehnung. Eine laparoskopsiche komplette Endometriosesanierung wurde geplant. Intraoperativ erfolgte beim Verdacht auf Malignität im Spatium rektovaginale eine Schnellschnittuntersuchung, welche das Vorliegen eines malignen Tumors bestätigte. Nach Umstieg auf eine Längslaparotomie erfolgten eine Hysterektomie, Adnektomie bds., partielle Kolpektomie, tiefe anteriore Rektumresektion mit Anlage eines endständigen Descendostomas bei tief infiltrierendem Befund, Zystoskopie mit DJ-Anlage beidseits. Histopathologisch zeigte sich ein Maligner Müllerscher Mischtumor (sog. Karzinosakom) auf dem Boden einer präexistenten Endometriose im Spatium rectovaginale mit Rektuminfiltration. Eine adjuvante Chemotherapie gefolgt von einer lokalen perkutanen Radiatio im Bereich der R1-Resektion wurden durchgeführt. Die bisherigen Verlaufskontrollen zeigten keinen Anhalt auf ein Rezidiv. Die maligne Transformation der Endometriose ist nicht nur in den Ovarien sondern auch in den tief infiltrierenden Lokalisationen möglich und sollte differentialdiagnostisch berücksichtigt werden.
