This review examines the role of Major Histocompatibility Complex Class I chain-related gene A (HLA-G) and its interactions with immune cells during both pregnancy and endometriosis. It highlights how HLA-G, an immunomodulatory molecule, contributes to immune tolerance by interacting with various leukocytes, including natural killer cells and T cells. The paper discusses the differential expression and functional impact of HLA-G isoforms in these two distinct physiological and pathological contexts. Relevance to endometriosis: this paper is centrally about endometriosis, specifically focusing on the immunological mechanisms involving HLA-G that may contribute to disease pathogenesis.

Interactions of Immunomodulatory HLA-G with Immune Cells during Pregnancy and Endometriosis Abstract The Major Histocompatibility Complex (MHC), the genetic region that encodes the proteins responsible for tissue graft rejection,1 encodes MHC Class I (MHC-I) and MHC Class II (MHC-II) glycoproteins. There are two subclasses of MHC-I proteins. Classical MHC-I (MHC-Ia) proteins are membrane-bound isoforms that are expressed in all nucleated cells of the body and present intracellular pathogen-derived peptides or the animal’s own peptides on the cell surface for immune recognition by CD8 T cells.
