However, the prognostic value of peripheral B lymphocyte counts on MM before and after treatment in the novel agent era was rare reported. Herein, we conducted a retrospective study in our center to detect peripheral B lymphocyte counts by flow cytometry in 110 patients with MM at our center and explore the relation with survival. The B lymphocyte count was significantly lower in MM patients than healthy controls (p < 0.005). The cutoff value of B lymphocyte count at diagnosis was 49/µl in MM and 94 patients divided into in high B lymphocyte group. Patients with low B lymphocyte count had a significant shorter progression-free survival (PFS) (p = 0.025) and a trend of unfavorable overall survival (OS) (p = 0.053) at diagnosis and after 4 cycles’ induction treatments. Furthermore, Multivariate analysis showed that low B lymphocyte count at diagnosis independent of ISS stage was a significantly inferior marker for predicting PFS (p = 0.027, hazard ratio(HR) 2.281, 95% confidence interval (CI) 1.098–4.741) and a trend for OS (p = 0.083, HR 2.394, 95% CI 0.896–6.160). In summary, these results suggested the low B lymphocyte count was associated with poor outcome in MM patients at diagnosis and after treatment in the novel agent era. Multiple myeloma B lymphocyte Outcome Dynamic change Immune profiles Figures Figure 1 Figure 2 Figure 3 Figure 4 Figure 5 1 Introduction Multiple myeloma (MM) is the second most common hematological malignant disorder, characterized by the infiltration of monoclonal plasma cells in bone marrow and related organs or tissues impairment. The revised international staging system (R-ISS) is now widely used in clinical practice, which focusing on clinical markers and cytogenetic abnormalities at diagnosis( 1 – 3 ). Immune profiles have been proved to play a crucial role in the pathogenesis and progression of MM( 4 , 5 ).The addition of immune profiles may improve the prediction ability in patients with MM( 6 – 8 ). B lymphocyte is a kind of lymphocytes component. Activated by tumor antigen, B lymphocytes secrete antibodies, which can activate complement system to lyses tumor cells or mediate macrophages, NK cells to exert antibody-dependent cell-mediated cytotoxicity (ADCC) effect( 9 , 10 ).Previous studies indicated that higher circulating blood B cells at diagnosis were a favorable prognostic marker without apparent relationship to circulating plasma cells in patients with multiple myeloma( 11 – 13 ).The depleted B cells in patients with monoclonal gammopathy of undetermined significance and MM were finally recovered after long-term disease control in both the bone marrow and peripheral blood( 9 , 14 ). These data implied B lymphocyte may involve in the progression and prognosis of multiple myeloma. However, the prognostic value of peripheral blood B lymphocyte on multiple myeloma in the novel agent era was rare reported. Therefore, we detected the B lymphocyte count in peripheral blood at diagnosis and after treatment by multiparameter flow cytometry (MFC) to explore the prognostic value and dynamic change of B lymphocyte count in patients with MM. 2 Methods 2.1 Patients Newly diagnosed multiple myeloma (NDMM) patients in Nanfang Hospital, Southern Medical University from January 2020 to January 2023 were enrolled in this study. All patients were diagnosed according to the International Myeloma Working Group (IMWG) criteria( 15 ) and treated with proteasome inhibitor (PI) and/or immunomodulatory (IMiDs) based regimen as first-line treatment after diagnosis. Patients with other malignant tumors, immune deficiency diseases, autoimmune diseases, rheumatism diseases and missing B lymphocyte count were excluded. Patients with good response after four cycles of induction treatment would undergo autologous peripheral blood stem cell transplantation (ASCT) or continue the same regimen as consolidation therapy. Besides, 63 individuals for health physical examination were also recruited in our center as normal control. This study was approved by the Ethics Committee of Southern Medical University affiliated Nanfang Hospital. 2.2 B lymphocytes detection by multiparameter flow cytometry Peripheral blood B lymphocyte was analyzed using fresh peripheral blood samples according to the standard procedure. The following conjugates of monoclonal antibodies were used: anti-CD3-PerCP-Cy5.5 and anti-CD19-PE-Cy7 (BD Biosciences, San Jose, CA, USA). The frequencies of lymphocytes were determined using a FACS Calibur flow cytometer (Becton Dickinson, Franklin Lakes, NJ, USA) and CellQuest software (BD Biosciences) was used for data analysis. At least 5×10 4 events were measured with each sample. B lymphocytes were defined as CD3-CD19 + lymphocytes and recorded as absolute number of cells/µl(normal range: 98.71–364.86 cells/µl) and was detected at diagnosis and the beginning of each cycle of induction therapy: T0, at diagnosis; T1, at the beginning of the second cycle of induction therapy; T2, at the beginning of the third cycle; T3, at the beginning of the fourth cycle; T4, at the beginning of the fifth cycle. 2.3 Statistical analysis Differences between nominal variables were evaluated using the Chi-Square test, and continuous variables were assessed using the Mann-Whitney U test. Optimal cutoff value was detected as a biomarker of survival by surv_cutpoint function. Overall survival (OS) and progression-free survival (PFS) distribution were analyzed by Kaplan-Meier (K-M) curves and log-rank test was applied to estimate differences between different groups. The univariable and multivariable analysis of survival were carried out by Cox regression model. All reported p-values were two-sided and confidence intervals refer to 95% boundaries, and statistical difference was considered as p-value less than 0.05. Statistical analyses were performed using the R4.4.1. 3 Results 3.1 Patients’ characteristics A total of 110 NDMM patient were included in our study. 56 (50.9%) patients were older than 60 years and the male-to-female ratio was 1.75:1. 66 (59.1%) patients were classified as IgG type, and 23 (20.9%) patients belonged to Light Chain type. According to DurieSalmon System (DS), 9 (8.2%) patients were at stage I, 14(12.7%) at stage II, and 87 (79.1%) at stage III. Based on International Staging System (ISS), 18(16.4%) patients were classified as stage I, 36(32.7%) as stage II, and 56 (50.9%) as stage III. Among the 65 (59.1%) patients who received the FISH test, 11 patients (16.9%) were classified as high risk who had 17p-, t (4;14) or t (14;16), while others were classified as standard risk. The most common induction regimen was bortezomib-based regimen in 101 (91.4%) patients, among which 54.5% (60) received a combination with lenalidomide and dexamethasone (VRD) and 37.3% (41) combination with cyclophosphamide and dexamethasone (VCD). Fifty-nine patients (53.6%) underwent ASCT after induction treatment. The baseline characteristics of all patients were summarized in Table 1 . The B lymphocyte count of MM patients was significantly lower than that of the health donors (p < 0.001, Fig. 1 ). The optimal B lymphocyte count cutoff value was determined as 49/µl(p < 0.05, Supplement Fig. 1 ) by using the surv_cutpoint function. All patients were divided into two groups: 94 patients in high B lymphocyte group and the left 16 patients in low B lymphocyte group. No significant differences were found in common clinical parameters including age, gender, hemoglobin, serum creatinine, serum calcium, serum lactate dehydrogenase, serum β2-microglobulin, subtypes and ISS stage between these two groups (p > 0.05), as shown in Table 2 . We also explored the dynamic change of B lymphocyte in MM patients during induction treatment. A total of 74 patients finished 4 cycles’ induction treatment: 42 (56.8%) patients received VRD and 27(36.5%) of them received VCD regimens. The B lymphocyte count was dramatically decreased after the first cycle of induction (P 0.05, Fig. 3 ). No significant difference was found between VCD and VRD regimen at each cycle (P > 0.05, Fig. 2 ). With a median follow-up of 27.77 months (range,0.17–51.83 months), 38 patients (34.5%) suffered relapse or progression and 21 patients (19.1%) died. Among them, 10 patients died of MM, 8 patients died of infection and 3 patients died of unknown cause. Patients with low B lymphocyte count had a significant shorter PFS (median PFS 25.8 months vs. not reached, p = 0.025, Fig. 3 A) and a trend of unfavorable OS (both median OS were not reached, p = 0.053, Fig. 3 B) than those with high B lymphocyte level. And then subgroup analysis was conducted based on lymphocyte levels and ISS staging to examine the impact of lymphocyte levels on prognosis in patients at different stages. Consequently, patients in ISS stage III with low B lymphocyte count had a significant shorter PFS (median PFS 19.2 months vs. 42.5 months, p = 0.004, Fig. 4 B), while the difference of outcomes in patients in ISS stage I or II with different B lymphocyte level were non-significant(Fig. 4 A).Furthermore, we explored the prognostic value of B lymphocyte change during induction treatment and found low B lymphocyte count after 4 cycles’ treatment could be a stable unfavorable marker for both PFS (p = 0.001, Fig. 5 A) and OS (p<0.0001, Fig. 5 B). Multivariate analysis showed that low B lymphocyte count at diagnosis independent of ISS stage was a significantly inferior marker for predicting PFS (p = 0.027, hazard ratio(HR) 2.281, 95% confidence interval (CI) 1.098–4.741) and a trend for OS (p = 0.083, HR 2.394, 95% CI 0.896–6.160). In this study, we explored the prognostic value and dynamic changing of B lymphocytes in NDMM and found the low B lymphocyte count remained an adverse immune marker for outcome in NDMM patients in the novel drugs era. B lymphocytes are essential for humoral immunity and play multiple roles in the occurrence and progression of human cancers( 17 , 18 ). In our study, B lymphocyte count was markedly decreased in NDMM compared with healthy controls, which may imply that B lymphocyte involves in the onset and development of MM. However, no significant associations were found in NDMM patients with high and low B lymphocyte on clinical parameters including age, sex, paraprotein isotype, hemoglobin, serum creatinine, serum calcium, serum lactate dehydrogenase, serum β2-microglobulin, and ISS stage. Furthermore, we conducted a longitudinal analysis to explore the changing of B lymphocyte count during induction treatment over time and in relation to outcome. The data showed B lymphocyte count was sharply decreased after the 1–2 cycles induction therapy, which may reflect the impact of therapy on blood immune cells of patients with MM. With the ongoing of induction therapy, the B lymphocyte count kept a slight increase trend. This finding suggests the lymphopoiesis was gradually reconstituted along with the control of MM after initial therapy. Together these data suggest that not only the B lymphocyte involves the causes of MM, but tumor cells also impair the host's immune system in turn. We also observed that patients with low B lymphocyte count at diagnosis had poor survival, especially in patients in ISS stage III. But the impact of B lymphocyte on PFS was more obvious than OS, which may imply the death of our MM patients was probably due to disease progress or relapse. Actually, 21 deaths occurred in our study and 10 of them died of MM, 8 died of infection and 3 died of unknown causes. To some extent, this also reflects infection due to B cell deficiency and humoral immunodeficiency may also contribute to the death in MM, especial at the early stage of induction treatment. It should be noted that this study was a retrospective study in a single center and the treatment regimens were inconsistent in our cohort . The B lymphocyte subsets were not explored in our study. Therefore, these findings need to be confirmed by future prospective study and further explore the clinical and prognostic value of different B cell subsets in MM. In conclusion, our study explored the prognostic value of B lymphocyte count in peripheral blood at diagnosis and found low B lymphocyte count was associated with poor outcome in MM patients in the novel agent era. The positive relation of B lymphocyte counts and survival was also kept after 4 cycles’ induction treatment. And then subgroup analysis was conducted based on lymphocyte levels and ISS staging to examine the impact of lymphocyte levels on prognosis in patients at different stages.
