Analysis on Complications and Related Factors in Gynecological Laparoscopic Surgery

Abstract
Objective:To explore the complications and the related factors in gynecological laparoscopic surgery.Methods:The clinical information of 2160 consecutive patients who received laparoscopic surgery in our department were analyzed retrospectively,including age,BMI,whether with abdominal surgery history or endometriosis.Results:The overall rate of complications is 3.33%(72 of 2160 patients).The complications during insertion of the verres needle and establishment of pneumoperitoneum were noted in 7cases(0.32%).Intra-operative complications occurred in 50 patients(2.31%),the most common complications were excessive blood loss(36 patients),which were treated by intra /post-operative blood transfusion.Postoperative complications occurred in 15 patients,accounting for 0.69% of all compliations,of which 10 were due to post-operative infection(66.67%).The complications occurred with a rate of 1.01%,5.92%,7.57%,24.51% in Ⅰ、Ⅱ、Ⅲ、Ⅳ type of laparoscopic procedure groups respectively.Univariate analysis showed that risk factors of complication were age(OR =2.64),uterine size(OR =3.45),with endometriosis history(OR =11.64),the type of surgery(OR =3.70).Multivariate analysis showed that with endometriosis history(OR =6.46) and the type of surgery(OR =3.55) were the major risk factors.Conclusions:The most commen complications in gynecological laparoscopic surgery are excessive blood loss during operation and post-operative infection.Complications are mainly related with the type of surgery and endometriosis history.The more complex laparoscopic procedure is or the patients with endometriosis history,the greater the rate of operative complications is.
