Синдром поликистозных яичников (СПКЯ) – одна из наиболее распространенных форм эндокринопатии, встречающаяся у 5–10% женщин репродуктивного возраста. Согласно данным Российского общества акушеров-гинекологов и эндокринологов, распространенность указанного синдрома в общей популяции женского населения репродуктивного возраста составляет от 8 до 21%. Среди причин бесплодного брака СПКЯ находится на 5-м месте, составляя 20–22%.Актуальность проблемы СПКЯ заключается не только в высокой степени распространения этой патологии, но и в ее социальной значимости. Это обусловлено частым сочетанием нарушений менструальной функции, ановуляторного бесплодия, метаболических расстройств и тем, что не менее беспокоит женщин, – проявлениями гиперандрогении: гирсутизмом, алопецией, себореей, акне, которые нередко становятся причиной нарушений психологического статуса (депрессивных, тревожных и биполярных расстройств).Обзор литературы посвящен современным представлениям об этиологии, патогенезе, клинических проявлениях и фенотипах синдрома поликистозных яичников. Несмотря на значительные достижения последних десятилетий, единой концепции этиопатогенеза не существует, а вопросы диагностики указанной патологии продолжают оставаться не окончательно ясными. СПКЯ в определенной степени является диагнозом исключения, поскольку может быть окончательно верифицирован после исключения заболеваний со сходной симптоматикой. В статье отражены анализ основных диагностических критериев синдрома поликистозных яичников, а также современные возможности терапии СПКЯ. Особую сложность представляют вопросы ранней диагностики и тактики ведения таких пациенток в подростковом возрасте. Polycystic Ovarian Syndrome (PCOS) is one of the most common forms of endocrinolopathy, that is observed in 5–10% of women of reproductive age. At the same time, it is noted, that more than 100 million women at the age of 15 to 49 in many countries suffer from this pathology.According to the Russian Society of Obstetricians Gynaecologists and Endocrinologists, the prevalence of the syndrome in the general population of women of reproductive age ranges from 8 to 21%. Among the reasons for infertile (childless) marriage, PCOS is in fifth and sixth place, amounting to 20–22%.The relevance of the problem of the PCOS is not only in the high prevalence of this pathology, but also in its social significance. This is due to the frequent combination of menstrual disfunction, anovulatory infertility, metabolic disorders and what also concerns women – manifestations of hyperandrogeny: hirsutism, alopecia, seborrhea, acne, which cause disorders of psychological status (depressive, anxiety and bipolar disorders).The literature review is devoted to contemporary notions of etiology, pathogenesis, clinical manifestations, and phenotypes of polycystic ovarian syndrome. Despite significant achievements in recent decades, there is no unified of etiopatogenesis, and the diagnosis and treatment of this disorder remain not comletely clear. The PCOS to a cirtain extenti is a diagnosis of the exception, as it can be finally verified after exclusion of diseases with similar symptoms. The article reflects the analysis of the main diagnostic criteria of polycystic ovarian syndrome; current approaches to treatment of PCOS are highlighted. Early diagnosis and management strategies of PCOS patients in adolescence are particularly challenging.
