Abstract

Abstract Purpose To evaluate functional internal rotation following reverse shoulder arthroplasty (RSA) for complex fractures in elderly patients using the 100-point Activities of Daily Living requiring Internal Rotation (ADLIR) score, and to explore the lesser-studied 30-point Activities of Daily Living requiring active External Rotation (ADLER) score. Methods Forty patients treated with RSA for complex fractures were prospectively followed for a minimum of two years. ADLIR and ADLER were correlated with range of motion, pain (VAS), Oxford Shoulder Score (OSS), Subjective Shoulder Value (SSV), QuickDASH, and tuberosity union. Results Median age at surgery was 72 years with a median follow-up of 27 months. Median ADLIR was 85 and correlated with internal rotation (p = 0.04). Median ADLER was 28 and correlated with active elevation and OSS (p = 0.011). Tuberosity union had no influence on either score. Conclusions RSA achieved satisfactory internal and external rotation function. ADLIR and ADLER represent reliable, complementary tools for postoperative assessment.
