Prognosis and risk factors of heart failure complicated with severe pneumonia after cardiovascular surgery

Abstract

Abstract Backgrounds Heart failure (HF) is an advanced stage of various heart diseases and are at an elevated risk of pneumonia, which further worsens prognosis. Postoperative pneumonia is a life-threatening complication after cardiovascular surgery, significantly increasing mortality. However, data on risk factors for severe pneumonia in HF patients undergoing cardiovascular surgery remain limited. Methods This study included 194 HF patients who underwent cardiovascular surgery in the cardiovascular intensive care unit (CICU) from September 2024 to January 2025. Eligible participants were required to have undergone bacterial culture, drug sensitivity testing and respiratory pathogen nucleic acid testing during hospitalization. Multivariate logistic regression analyses were used to identify risk factors for severe pneumonia. Results Of the 194 patients, 55.67% had microbiologically positive results, with Klebsiella pneumoniae (36.4%) and Acinetobacter baumannii (21.2%) being the most common pathogens, and multidrug-resistant organisms (MDROs) accounting for 27.3% of isolates. Severe pneumonia occurred in 25.3% of HF patients, and these patients had a significantly higher mortality rate (51% vs. 8.3%, P < 0.001) compared to those without severe pneumonia. MDRO infection (adjusted OR: 5.114, 95% CI: 1.616–16.814) and end-stage HF (adjusted OR: 5.526, 95% CI: 1.397–21.859) as independent risk factors for severe pneumonia. Conclusion This study highlights that MDRO infection and end-stage HF are critical independent risk factors for severe pneumonia in HF patients after cardiovascular surgery, with severe pneumonia significantly increasing mortality. These findings emphasize the need for targeted strategies to improve clinical outcomes.

Previous randomized controlled trials showed that HF patients exhibit a pneumonia incidence roughly three times higher than the general population [ 1 – 3 ] .Among these patients, pneumonia confers a 4.34-fold greater risk of all-cause mortality, 3.62-fold increased risk of cardiovascular mortality, and 2.39-fold higher re-hospitalization rates [ 3 – 5 ] . Postoperative infection poses a serious threat to surgical outcomes, potentially disrupting recovery, prolonging hospitalization, and significantly increasing medical expenses. Cardiac surgery patients experience infectious complications in 5–21% of cases, with associated mortality risks exceeding fivefold baseline rates [ 6 ] . Pneumonia accounts for a substantial proportion of these postoperative infections [ 7 – 9 ] . A propensity-matched analysis revealed markedly reduced five-year survival rates (62% versus 81%) in cardiac surgery patients who developed pneumonia [ 8 ] 。 The impaired physiological reserve of HF patients renders them particularly susceptible to severe postoperative infections. Multiple studies identify HF as an independent predictor of major postoperative infection (HR 1.47, 95% CI 1.11–1.95) [ 10 ] . Greco et al. presented that patients with heart failure, lower ejection fraction, urgent or emergent status, and/or transplantation or ventricular assisted device ex- or implantation all had significantly increased infection rates [ 11 ] . Strobel's group further established preoperative intra-aortic balloon pump use and low ejection fraction as independent predictors of post-coronary bypass pneumonia [ 12 ] . These findings underscore the need to elucidate infection risk factors in this high-risk population to guide clinical decision-making. Current evidence remains insufficient to fully characterize this clinical challenge. Our study systematically evaluates risk factors for severe pneumonia in HF patients undergoing cardiovascular surgery, with the goal of informing preventive strategies and improving outcomes. Methods 2.1 Study Subjects This retrospective study enrolled pre-operative HF patients undergoing cardiovascular surgery in cardiovascular intensive care unit (CICU) from September 2024 to January 2025. Eligible participants were required to have undergone bacterial culture, drug sensitivity testing and respiratory pathogen nucleic acid testing during hospitalization. The diagnosis of HF was confirmed through a comprehensive assessment including medical history, physical examination, laboratory parameters, echocardiography. and other cardiac imaging followed the diagnostic algorithm of Chinese Guidelines for the diagnosis and treatment of HF 2024 [ 13 ] . Exclude criteria are as follows:① age<18 years, ②did not receive surgical treatment, ③ history of severe immunodeficiency diseases or receiving long-term immunosuppressive therapy, ④ presence of active pulmonary tuberculosis or other severe pulmonary diseases that may interfere with the diagnosis of post - operative pneumonia. This study was conducted in accordance with the latest version of the Helsinki Declaration and has been approved by the Medical Science Research Ethics Committee of the First Affiliated Hospital of Xi’an Jiaotong University. Individual consent for this retrospective analysis was waived. 2.2 Data collection Perioperative clinical data of all participants, including demographic characteristics, laboratory parameters, surgical information are retrospectively retrieved from the electronic medical record. Pre-operative laboratory parameters refer to the first in-hospital test results. Post-treatment parameters were measured one week after the bacterial culture was sent.Post-operative follow-up information was conducted via structured telephone interviews, with data collection continuing until 30 days after surgery. The follow-up information includes the occurrence of severe pneumonia, and 30- ay mortality. The Multiple Organ Dysfunction Syndrome (MODS) Score, Sepsis-related Organ Failure Assessment (SOFA) score and Acute Physiology and Chronic Health Evaluation (APACHE) II score were calculated within the first 48 hours after surgery by two independent physicians to assess the severity of illness. In cases of score discrepancies, a third senior CICU physician made the final determination to ensure inter-rater reliability. The primary outcome is the incidence of severe pneumonia within 30 days after cardiovascular surgery in HF patients. The definition of severe pneumonia adheres to the 2015 Chinese Guidelines for Adult Community-Acquired Pneumonia (CAP) [ 14 ] . Patients meeting 1 major criterion or ≥ 3 minor criteria are diagnosed with severe pneumonia. The diagnosis of severe pneumonia will be confirmed by two senior doctors of CICU. Major Criteria: ①requirement for tracheal intubation and mechanical ventilation; ②septic shock requiring vasoactive agents after aggressive fluid resuscitation. Minor Criteria: ①respiratory rate ≥ 30 breaths/min; ②PaO₂/FiO₂ ≤250 mmHg; ③multilobar infiltrates (imaging shows involvement of ≥ 2 lung lobes); ④altered mental status and/or disorientation; ⑤blood urea nitrogen (BUN) ≥ 7 mmol/L; ⑥hypotension requiring aggressive fluid resuscitation. CURB score, clinical pulmonary infection score (CPIS) and pneumonia severity index (PSI) are used to assess the severity of pneumonia which will be calculated at the time of diagnosis of severe pneumonia. The secondary outcomes including: length of hospital stay, length of ICU stay, 30-day mortality. 2.3 Statistical Analysis Statistical analyses were performed using IBM SPSS Statistics 24.0 (IBM Corp., Chicago, IL, USA). Variable distribution was examined using the Kolmogorov–Smirnov test. All data are expressed as the mean ± standard deviation (SD) for normally distributed variables and as the median with interquartile range (IQR) for skewed distributions. Categorical variables were summarized as counts with percentages. Group differences were evaluated using the Student t test or Mann-Whitney U test for continuous variables and the χ2 test or Fisher’s exact test for categorical variables. Univariate and multivariate logistic regression analyses were used to screen the risk factors for severe pneumonia. Statistical significance was defined as p < 0.05, and all results were two-tailed. Results 1. Clinical characteristics of microbiologically positive and negative HF patients after cardiovascular surgery A total of 194 HF patients after cardiovascular surgery were included. Etiological positivity was defined as either a positive results from respiratory pathogen nucleic acid testing or positive culture, with 55.67% of patients meeting this criterion. Table1 presented the baseline characteristics of participants stratified by etiological status. Patients in the microbiologically positive group demonstrated a greater prevalence of type A aortic dissection (TAAD). Preoperative white blood cell counts, along with post-treatment procalcitonin and interleukin-6 (IL-6) levels, were significantly elevated in this group compared to negative cases. The positive cohort also showed increased neutrophil percentages post-treatment, while lymphocyte, monocyte, eosinophil, and basophil percentages were reduced. These patients exhibited substantially higher MODS, SOFA, and APACHE II scores (all P<0.001). The positive group required longer ICU and hospital stays and showed greater mortality (25.9% vs 10.5%, P =0.006). 2. The distribution of pathogenic microorganisms Sixty-six isolates were obtained. Five patients had polymicrobial infections, while the others had monomicrobial infections. The most common pathogens were Klebsiella pneumoniae (36.4%) and Acinetobacter baumannii (21.2%) (Table 2). Among them, multidrug-resistant bacteria accounted for approximately 27.3%. 3. Characteristics between deceased and non-deceased patients Among the 194 patients, 37 (19.07%) died within 30 days postoperatively (Table 3). The deceased group exhibited a microbiologically positive rate of 75.7%, with 29.7% meeting criteria for severe pneumonia—both significantly higher than the control group's rates of 51% (P=0.006) and 14%( P =0.022), respectively. Baseline characteristics, including age, gender distribution, prevalence of hypertension or diabetes mellitus, and cardiovascular disease profiles, did not differ significantly between group. Preoperative biochemical markers also demonstrated comparable values. However, postoperative levels of PCT, C-Reactive Protein (CRP), IL-6, and white blood cell (WBC) were markedly elevated, while platelet counts were significantly reduced in deceased patients. The deceased group showed substantially higher MODS, SOFA, and APACHE II scores (all P<0.001). No significant intergroup differences emerged for CURB, CPIS, or PIS scores. 4. Clinical characteristics of HF patients stratified by comorbid severe pneumonia Table 4 presents data from 49 HF patients who developed severe pneumonia, with median CURB, CPIS, and PIS score were 4 (4.00–5.00), 8.00 (7.00, 10.00), and 190.00 (164.00, 215.00), respectively. The severe pneumonia cohort contained significantly more cases of end-stage HF and TAAD than the non-severe group. Severe pneumonia patients exhibited lower preoperative neutrophil percentages but higher monocyte, eosinophil, and basophil counts compared to those with non-severe pneumonia. Post-treatment laboratory results demonstrated markedly increased PCT levels, alongside decreased lymphocyte percentages, and platelet counts in these patients. MODS, SOFA, and APACHE II scores all showed significant elevation in the severe pneumonia group compared to controls. Both the respiratory pathogen nucleic acid tests and sputum culture showed significantly higher positive rates in the severe pneumonia group. Multidrug-resistant bacterial infections occurred in 20.4% of severe pneumonia patients versus 5.5% of controls (P=0.003). The severe pneumonia group required prolonged ICU stays and exhibited dramatically higher mortality (51% vs 8.3%, P<0.001). 5. Risk factors of severe pneumonia in HF patients after cardiovascular surgery Multivariate logistic regression analysis revealed that both concomitant multidrug-resistant organism (MDRO) infection and end-stage HF independently predicted severe pneumonia in HF patients after cardiovascular surgery. MDRO infection elevated the risk of severe pneumonia by 5.114-fold (95% CI: 1.616–16.814), while end-stage HF increased the risk by 5.526-fold (95% CI: 1.397–21.859) (Table 5). Discussion This study represents the first investigation into the risk factors for severe pneumonia in HF patients following cardiovascular surgery. This study elucidates the comprehensive impact of severe pneumonia, heart failure, and cardiac surgery on patient prognosis. The analysis reveals etiological positivity (55.67%) and severe pneumonia (25.3%) are prevalent in HF patients after cardiovascular surgery, with MDRO infection and end-stage HF emerging as independent risk factors for severe pneumonia (adjusted OR: 5.114 and 5.526, respectively). Mortality rates reached 51% in severe pneumonia cases compared to 8.3% in non-severe pneumonia patients, demonstrating the critical impact of postoperative severe pneumonia on prognosis. Previous study reported a 2.1–8.2% incidence of postoperative pneumonia following valvular and coronary surgery, substantially lower than our observed rate [ 15 – 17 ] . This discrepancy likely reflects our inclusion of higher-risk heart failure patients with poorer clinical status, including those undergoing left ventricular assist device implantation, or TAAD repair, along with an older overall population. Yao et al. documented a 36.9% pneumonia incidence after aortic dissection surgery [ 18 ] , consistent with our finding that TAAD patients exhibited higher rates of microbiologically positive and severe pneumonia. As reported in prior studies, gram-negative bacilli remain the most prevalent pathogens in MDRO -associated ventilator-associated pneumonia (VAP) following cardiovascular surgery [ 2 , 19 , 20 ] . Klebsiella pneumoniae (36.4%) and Acinetobacter baumannii (21.2%) were the predominant pathogens in our study, with MDROs accounting for 27.3% of isolates. This pattern underscores the substantial risk of nosocomial infections in cardiovascular surgery patients, especially those with immunocompromised conditions such as heart failure.Chen et al. observed a markedly elevated in-hospital mortality rate among cardiac surgery patients with MDRO-VAP compared to non-MDRO cases (29.79% versus 1.61%) (29.79% vs 1.61%) [ 19 ] . MDRO-infected patients also exhibited higher rates of severe pneumonia (19.3% versus 5.51%), correlating with prolonged postoperative hospitalization and greater healthcare expenditures [ 17 ] . These findings emphasize the necessity of tailored antimicrobial protocols and preoperative MDRO surveillance for high-risk populations. End-stage HF was associated with a 5.526-fold increased risk of severe pneumonia, aligning with previous findings that HF independently predicts serious postoperative infections (HR: 1.47) [ 4 ] . Pulmonary congestion, common in end-stage HF[2, 21], promotes bacterial growth by impairing mucociliary clearance through chronic venous stasis. These patients also exhibit immune dysfunction, as chronic inflammation and diminished cardiac output suppress systemic immunity—a pattern reflected in the lower preoperative lymphocyte counts observed in severe pneumonia cases. Surgical intervention and cardiopulmonary bypass further impair pulmonary defenses and amplify inflammatory responses, effects that heart failure and concurrent pneumonia synergistically intensify, prolonging recovery and increasing complication risks [ 22 , 23 ] . The current study has several limitations. As a single-center investigation with a relatively small sample size, its findings may have limited generalizability. Multicenter studies with larger cohorts should verify these results and better elucidate the mechanisms contributing to poor outcomes in this high-risk population. Additionally, preoperative MDRO decolonization strategies warrant further evaluation. In HF patients after cardiovascular surgery, MDRO infection and end-stage HF are independent risk factors for severe pneumonia, correlating with drastically increased mortality. These results underscore the importance of tailored preventive measures such as preoperative MDRO screening and intensified postoperative surveillance for high-risk individuals. Clinicians should focus on promptly detecting severe pneumonia to enhance outcomes in this susceptible patient group. Eligible participants were required to have undergone bacterial culture, drug sensitivity testing and respiratory pathogen nucleic acid testing during hospitalization. Multivariate logistic regression analyses were used to identify risk factors for severe pneumonia.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eOf the 194 patients, 55.67% had microbiologically positive results, with \u003cem\u003eKlebsiella pneumoniae\u003c/em\u003e (36.4%) and \u003cem\u003eAcinetobacter baumannii\u003c/em\u003e (21.2%) being the most common pathogens, and multidrug-resistant organisms (MDROs) accounting for 27.3% of isolates. Severe pneumonia occurred in 25.3% of HF patients, and these patients had a significantly higher mortality rate (51% vs. 8.3%, P \u0026lt; 0.001) compared to those without severe pneumonia.
