Predicting Cardiotoxicity in Breast Cancer Survivors: A Decade-Long Risk Model for Heart Failure and Cardiomyopathy

Abstract

Cardiotoxicity remains a significant concern in breast cancer survivorship, particularly among women receiving systemic therapies such as anthracyclines and HER2-targeted agents. The study published in JAMA Oncology (October 23, 2025) presents a validated 10-year risk prediction model for heart failure and cardiomyopathy (HF/CM) in women with early-stage breast cancer. Using a cohort of over 26,000 patients from Kaiser Permanente Southern California, the model integrates demographic, clinical, and treatment-related variables to stratify patients into low-, moderate-, and high-risk categories. With a time-dependent AUC of 0.79, the model demonstrates strong discrimination and calibration, offering a practical tool for guiding cardiac surveillance and treatment planning. This commentary evaluates the study's methodology, clinical implications, and translational potential, while highlighting the need for equitable access to cardio-oncology services and integration of predictive tools into routine care.
