Abstract

Abstract Background: Coronavirus disease 2019 (COVID-19) has placed healthcare workers (HCWs) and administrative staff at increased risk of infection, making awareness of infection prevention and control (IPC) measures essential. Assessing awareness levels among healthcare personnel is critical for strengthening preparedness and response in healthcare settings. Objective: To assess the level of awareness and associated factors of COVID-19 infection prevention among healthcare workers and administrative staff in selected public hospitals of the Somali Region, Eastern Ethiopia. Methods: An institution-based cross-sectional study was conducted among 200 healthcare workers and administrative staff from Dhagahbur and Kebridahar General Hospitals. Data were collected using a structured self-administered questionnaire. Descriptive statistics were computed, and awareness levels were summarized using median scores. The Modified Bloom’s cut-off point was applied to categorize awareness levels. Results: Of the 200 participants, 120 (60.0%) were male and 80 (40.0%) were female. The overall awareness of COVID-19 infection prevention was moderate, with an overall median correct response rate of 60%. Higher awareness was observed among clinical nurse level IV staff (66.1%) and medical doctors (64.0%), while non-clinical/administrative staff (49.3%) demonstrated the lowest awareness. Female participants showed slightly higher median awareness than males. Although most respondents correctly identified hand hygiene moments and appropriate PPE use, notable gaps were observed in identifying the causative agent, defining close contact, appropriate isolation procedures, and preferred hand hygiene methods for visibly soiled hands. Conclusion: Healthcare workers and administrative staff demonstrated a moderate level of awareness regarding COVID-19 infection prevention. Targeted and continuous IPC training particularly for non-clinical staff is recommended to address identified gaps and strengthen infection prevention practices.

COVID-19 Infection prevention Awareness Healthcare workers Somali Region BACKGROUND Coronavirus disease 2019 (COVID-19) is a global public health emergency caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The disease was first identified in December 2019 in Wuhan, China, and rapidly spread worldwide due to its high transmissibility and lack of population immunity.( 1 ) SARS-CoV-2 is an enveloped, single-stranded, positive-sense RNA virus belonging to the Betacoronavirus genus of the Coronaviridae family.( 2 ) Clinically, COVID-19 commonly presents with fever, fatigue, dry cough, and sore throat, while severe cases may progress to hypoxemia, acute respiratory distress syndrome, septic shock, and multi-organ failure.( 3 ) The virus is mainly transmitted through respiratory droplets and close contact, with possible fecal-oral transmission, and has an incubation period ranging from 2 to 14 days.( 4 ) Globally, COVID-19 has resulted in millions of infections and substantial mortality, placing unprecedented pressure on health systems. Low- and middle-income countries, including those in East Africa, face additional challenges due to limited resources, weak health infrastructure, and shortages of personal protective equipment. In Ethiopia and neighboring countries, increasing numbers of COVID-19 cases have been reported among healthcare workers.( 5 ) Healthcare workers are at the frontline of COVID-19 prevention and response and are at heightened risk of infection because of their close and prolonged contact with patients. ( 6 ) To reduce this risk, the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC) developed infection prevention and control (IPC) guidelines and training programs aimed at improving awareness and safe practices among HCWs.( 7 ) Nevertheless, evidence indicates that inadequate awareness and poor adherence to IPC measures among healthcare staff can contribute to healthcare-associated transmission and compromise patient and worker safety. ( 8 ) Although several studies have assessed awareness of COVID-19 prevention among healthcare workers in high-income settings, evidence from pastoral and resource-limited regions such as the Somali Region of Eastern Ethiopia remains scarce. At the time of this study, no specific antiviral treatment or universally available vaccine was recommended, making effective infection prevention measures the primary strategy for controlling COVID-19 transmission.( 9 ) Given the high occupational risk faced by healthcare workers and the limited local evidence, assessing awareness of infection prevention measures is critical for strengthening pandemic preparedness and response. Therefore, this study aimed to assess awareness of infection prevention against COVID-19 among staff of two selected public hospitals in the Somali Region, Eastern Ethiopia, in 2020. METHOD AND MATERIAL Study area The study was conducted in the Somali Regional State , located in eastern Ethiopia. The region is the second largest in the country and borders Afar and Oromia regions and Dire Dawa city administration to the west, Djibouti to the north, Somalia to the north, northeast and east, and Kenya to the southwest. Jijiga is the capital city of the region. According to the 2007 national census conducted by the Central Statistical Agency (CSA) of Ethiopia, the Somali Region had a population of 4,445,219, of whom 14.0% lived in urban areas and approximately 38.0% were pastoralists. The population was projected to reach 5,748,998 in 2017. This study was carried out in Jarar Zone (Dhagahbur General Hospital) and Korahe Zone (Kebridahar General Hospital) . Dagahbur town is located approximately 650 km east of Jijiga and about 780 km from Addis Ababa. Kebridahar town is the administrative center of Korahe Zone. Both hospitals serve as referral centers for surrounding districts and pastoral communities. Dhagahbur General Hospital has a total of 164 staff members , including clinical and administrative personnel. Kebridahar General Hospital has 148 staff members , comprising nurses, midwives, physicians, laboratory professionals, pharmacists, health officers, and administrative staff. Study period The study was conducted from June to July 2020 . Study design An institution-based cross-sectional study design was employed Source of population All healthcare workers and administrative staff working in public hospitals of the Somali Region Study population All healthcare workers and administrative staff working at Dhagahbur General Hospital and Kebridahar General Hospital during the study period. Inclusion and exclusion criteria Inclusion:- All healthcare workers and administrative staff who were available during the study period and willing to participate. Exclusion criteria ; Healthcare workers who were engaged in critical duties during data collection (e.g., operating room staff, radiology staff) Sample size determination The sample size was calculated using a single population proportion formula , assuming a proportion of 84% for study conducted on Knowledge, attitude and practice toward COVID-19 among the public in the Kingdom of Saudi Arabia.( 10 ) n= \(\:\frac{{\text{z}}^{2}\text{p}\text{q}}{{\text{d}}^{2}}\) where z = 1.96 P = the proportion of hand-washing practice were 84% q = 1-p, 1-0.84 = 0.16 d = 0.05, degree of accuracy desired n= \(\:{(1.96)}^{2}\text{*}0.84\text{*}0.16/{0.05}^{2}\) , = 207 The calculated sample size was 207 . After adding a 5% non-response rate , the final sample size became 217 participants . Sampling procedure Two public hospitals were selected from the Somali Region using a purposive sampling technique. The total sample size was allocated proportionally to each hospital based on the number of staff. (Dhagahbur General Hospital:114 participants and Kebridahar General Hospital: 103 participants). Participants were selected via using systematic random sampling technique using staff registration lists. The sampling interval (k) was calculated as N/n, and the first participant was selected by lottery method, followed by every kth staff member until the required sample size was reached. Data collection instrument and validity Data were collected using a self-administered structured questionnaire adapted from the World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) COVID-19 infection prevention and control guidelines updated in March 2020. The questionnaire included sections on socio-demographic characteristics, awareness of COVID-19, Infection prevention and control practices, Hand hygiene knowledge based on WHO “Five Moments of Hand Hygiene” The questionnaire was translated from English to Somali and back-translated to maintain consistency. ( 4 ) Infection prevention and control (IPC):- is a scientific approach and practical solution designed to prevent harm caused by infection to patients and health workers. It is grounded in infectious diseases, epidemiology, social science and health system strengthening. IPC occupies a unique position in the field of patient safety and quality universal health coverage since it is relevant to health workers and patients at every single health-care encounter.( 11 ) Hcw’s:- a health care worker is a one who delivers care and services to the sick and ailing either directly as doctors and nurses or indirectly as aides, helpers, laboratory technicians, or even medical waste handlers. ( 11 ) Prevention:- any action taken to decrease the chance of getting a disease or condition. ( 12 ) Awareness:- is the state or ability to perceive, to feel, or to be conscious of events, objects ,or sensory patterns.( 13 ) Definition of Awareness Level The level of awareness of infection prevention against COVID-19 was assessed using a structured questionnaire. Participants’ responses were scored and converted into percentages. The overall awareness level was classified using the Modified Bloom’s cut-off point , where a score of less than 60% was considered poor awareness , 60–79% was considered moderate awareness , and greater than or equal 80% was considered good awareness . ( 10 ) Data quality assurance Data quality was ensured through questionnaire pre-testing, training of data collectors, daily supervision, and immediate checking for completeness and consistency at the collection site. Data processing and analysis Data were entered into EpiData version 3.02 , cleaned, and exported to SPSS version 23 for analysis. Descriptive statistics were used to summarize variables. The chi-square test was applied to assess associations, and a p-value < 0.05 was considered statistically significant.. Ethical consideration Ethical approval for this study was obtained from the Jigjiga University Institutional Health Research Ethics Review Committee (No: IHRERC/58/2020 ). Prior to data collection, permission was secured from the respective hospital administrations. All study participants were informed about the purpose of the study, the procedures involved, and their rights as participants. Written informed consent was obtained from each participant before enrollment. Participation was entirely voluntary, and participants were assured of their right to withdraw from the study at any time without any consequences. Confidentiality of the information was strictly maintained, and no personal identifiers were included in the data collection tools or reports. The study was conducted in accordance with the ethical principles of the Declaration of Helsinki. Expected outcome of the study The study aimed to determine the level of awareness and infection prevention practices against COVID-19 among healthcare workers and administrative staff. RESULTS Socio- Demographic Characteristics of the study participants Out of the 217 hospital staff initially sampled, 200 participants completed the study , yielding a response rate of 92.1% . The majority of participants (70.0%) were aged 18–30 years, 25.5% were 31–44 years, and 4.5% were ≥ 45 years. The majority of respondents were Muslim (187, 93.5%) , and most were male (120, 60%) , with females accounting for 80 (40%) . The predominant ethnic group was Somali (182, 91%) . In terms of profession, BSc nurses (44, 22%) constituted the largest group, followed by BSc midwives (31, 15.5%) and clinical nurses level IV (28, 14%) . Public health officers represented the smallest professional group ( 8, 4% ). Regarding sources of information on COVID-19, social media was the most frequently reported source ( 72, 36% ) (Table: 1) 4.2 The awareness of infection prevention against COVID-19 among health care workers and administrative staffs Overall awareness of COVID-19 infection prevention among participants was moderate , with an overall median correct response rate of approximately 60%. Using the Modified Bloom’s cut-off point, awareness varied across demographic and professional categories. More than half of the respondents correctly identified respiratory droplets as the primary mode of COVID-19 transmission. However, only 12.5% correctly identified SARS-CoV-2 as the causative agent of the disease. Knowledge of close contact definition was limited, with only 36.0% of participants providing a correct response. Regarding hand hygiene, 70.5% of respondents correctly identified all recommended moments for hand hygiene. However, only 47.5% reported having received formal hand hygiene training in the past three years. A substantial proportion of participants incorrectly selected alcohol-based hand sanitizer as the preferred method for cleaning visibly soiled hands. Awareness of personal protective equipment use was relatively high, with more than 80% of respondents correctly identifying appropriate PPE for transporting suspected or confirmed COVID-19 patients. In contrast, awareness of recommended isolation room requirements was lower, with only 42.5% providing correct responses. During the early months of the pandemic, healthcare workers (HCWs) were at particularly high risk of infection due to frequent and close contact with patients, highlighting the importance of infection prevention and control (IPC) awareness and practices. This study assessed the level of COVID-19 IPC awareness among healthcare and administrative staff in two public hospitals of the Somali Region, Eastern Ethiopia, during June-July 2020. The results revealed a moderate overall level of awareness (median score 60%), indicating that while basic knowledge of COVID-19 transmission and prevention existed, critical gaps remained. These findings are consistent with early studies conducted in low- and middle-income countries, which reported moderate knowledge levels among healthcare workers during the initial phase of the pandemic.( 9 ):( 13 ). This suggests that although general IPC messages were disseminated widely, detailed knowledge regarding specific preventive measures was not fully internalized without targeted training. Profession-specific differences were observed in awareness. Clinical nurses (66.1%) and medical doctors (64.0%) demonstrated higher awareness compared with non-clinical and administrative staff (49.3%). The higher awareness among clinical staff likely reflects their direct involvement in patient care and prior IPC training. Conversely, non-clinical personnel, despite their exposure to healthcare environments, had lower awareness levels. Similar professional disparities were reported in early studies in Saudi Arabia and other resource-limited settings, emphasizing the need to include all categories of hospital staff in IPC programs. ( 6 ):( 10 ). Hand hygiene awareness was relatively good, with most participants identifying the recommended moments for hand hygiene. However, misconceptions persisted regarding the appropriate method for cleaning visibly soiled hands, and less than half had received formal training in the preceding three years. This finding highlights the need for continuous, practical IPC training rather than reliance solely on theoretical knowledge, echoing findings from studies in Ethiopia and Libya conducted in the first half of 2020. ( 13 ):( 14 ). Overall, these results underscore the importance of inclusive IPC strategies that address both clinical and non-clinical staff. Providing targeted education, regular refresher training, and clear guidance on hand hygiene, isolation procedures, and personal protective equipment (PPE) use is critical for strengthening hospital preparedness against COVID-19 and future outbreaks. Conclusion Healthcare workers and administrative staff in the two public hospitals of the Somali Region demonstrated a moderate level of awareness of COVID-19 infection prevention and control measures. Awareness of hand hygiene moments and PPE use was generally satisfactory; however, significant gaps were observed in knowledge regarding the causative agent, close contact definitions, isolation procedures, and appropriate hand hygiene practices. Professional, age, and gender differences were apparent, with non-clinical and administrative staff showing the lowest awareness despite frequent interaction with healthcare environments. These findings emphasize the need for inclusive and targeted infection prevention interventions that reach all categories of hospital staff, not only those directly involved in patient care. Limitations of the study This study had several limitations. The cross-sectional design limits causal inference between awareness and associated factors. Data were collected using self-administered questionnaires, which may be subject to recall and social desirability bias. The study was conducted in only two public hospitals, limiting generalizability to other settings. Additionally, rapidly evolving COVID-19 guidelines during the study period may have influenced awareness levels. Recommendations Inclusive Training Programs : National and regional health authorities should implement comprehensive IPC training that includes both clinical and non-clinical staff , ensuring all personnel understand key preventive measures. Continuous Education : Hospitals should provide regular refresher sessions and practical workshops on hand hygiene, isolation protocols, and PPE usage to reinforce knowledge and correct misconceptions. Monitoring and Evaluation : Establish routine assessments of staff awareness and practices to identify gaps and adjust training programs accordingly. Resource Provision : Ensure adequate supplies of PPE, hand hygiene materials, and isolation facilities, particularly in resource-limited hospitals, to support effective infection prevention. Public Health Preparedness : Strengthen communication and educational campaigns aimed at healthcare workers and the wider community to maintain awareness and readiness for future outbreaks. Data were collected using a structured self-administered questionnaire. Descriptive statistics were computed, and awareness levels were summarized using median scores. The Modified Bloom\u0026rsquo;s cut-off point was applied to categorize awareness levels.\u003c/p\u003e\u003ch2\u003eResults:\u003c/h2\u003e \u003cp\u003eOf the 200 participants, 120 (60.0%) were male and 80 (40.0%) were female. The overall awareness of COVID-19 infection prevention was moderate, with an overall median correct response rate of 60%. Higher awareness was observed among clinical nurse level IV staff (66.1%) and medical doctors (64.0%), while non-clinical/administrative staff (49.3%) demonstrated the lowest awareness. Female participants showed slightly higher median awareness than males. Although most respondents correctly identified hand hygiene moments and appropriate PPE use, notable gaps were observed in identifying the causative agent, defining close contact, appropriate isolation procedures, and preferred hand hygiene methods for visibly soiled hands.\u003c/p\u003e\u003ch2\u003eConclusion:\u003c/h2\u003e \u003cp\u003eHealthcare workers and administrative staff demonstrated a moderate level of awareness regarding COVID-19 infection prevention.
