Abstract

症例は47歳，女性．3年前に他院で子宮内膜症と子宮筋腫に対し左付属器切除，子宮摘出術の既往がある．発熱と右下腹部痛を主訴に当院を受診した．血液検査で炎症反応の上昇を認めたが，腫瘍マーカーは正常値だった．腹部造影CTで上行結腸に接して，造影効果の高い約6cmの壁外発育性の腫瘤を認めた．大腸内視鏡検査では，明らかな粘膜病変は認めなかった．悪性腫瘍も否定できないことから開腹手術を行った．結腸間膜内に上行結腸と横行結腸に付着した球型の腫瘤を認め，所属リンパ節も目立ったことから，D3リンパ節郭清を含む結腸右半切除術を施行した．病理組織検査では，上行結腸の筋層内に子宮内膜組織が散在し，上行結腸子宮内膜症と診断した．所属リンパ節13個にも子宮内膜組織を認め，その分布からリンパ行性に内膜組織が進展したと考えられた．腸管子宮内膜症に所属リンパ節病変を伴うものは稀であり，文献的考察を加え報告する．
