症例は既往歴のない36歳の女性．平成20年5月，月経初日の夕食後に腹痛が出現，翌日に腹痛の増悪と嘔吐を認めたために近医を受診．腹部CTにて血性腹水と小腸の拡張を認め，当院婦人科紹介となる．子宮・付属器に異常なしとのことで，回盲部の炎症による小腸イレウスとして外科紹介となった．右下腹部に反跳痛を認めたため，緊急手術を施行した．手術所見では，回腸末端部が相互の強固な癒着性変化にて一塊となっており，潰瘍穿通による瘻孔形成によるものと判断した．病変部口側にも線維化病変を数カ所認めたた…
