症例は40歳，女性．虫垂炎の手術歴があり，20代の頃より月経困難を自覚していたが，子宮内膜症と診断されたことはなかった．食後の腹痛と腹満感を主訴に当院を受診．CTにて癒着性小腸閉塞と診断し入院加療としたものの，保存的治療により改善せず，手術の方針となった．骨盤内で回腸が癒着・屈曲し，狭窄を呈していたため，小腸部分切除を施行した．切除標本の病理組織学的検査にて腸管子宮内膜症と診断した．腸管子宮内膜症は画像診断で特徴的な所見が得られず術前診断は困難である．術前診断が困難である女性…
