症例は46歳の女性.月経時に右気胸を3回繰り返した後, 胸腔鏡下手術を施行した.横隔膜腱中心に欠損孔を認め, これを含む横隔膜領域を切除縫合した.病理組織検査の結果, 広範なヘモジデリン沈着を認め月経随伴性気胸と考えた.術後8ヵ月間リュープロレリンの投与を行ったが, 投与中止後7ヵ月目に気胸が再発した.そこで投与を再開したが, その後の再発はない.手術療法後のホルモン療法は再発予防に有用と考えられた.
