Kleine Herde an Serosa bzw. Muscularis\nsind meist asymptomatisch. Größere Herde führen zu einer Induration bzw. Stenose\ndes Darmlumens. Die Fortschritte der endoskopischen Chirurgie ermöglichen es, diese\nHerde im Sinne einer Teil- bzw. Vollwandresektion zu entfernen. Bei ausgedehnten\nBefunden größerer Darmabschnitte ist eine Segmentresektion ggf. unter Verwendung\nvon Staplersystemen laparoskopisch assistiert möglich.\nErgebnisse: Im Zeitraum von 3/96 bis 7/03 wurden 142 Patientinnen mit ausgeprägter Darmendometriose\nlaparoskapisch assistiert operiert. Tief gelegene Befunde wurden nach Mobilisation\nmittels Staplertechnik reseziert bzw. die Anastomose angelegt. Höher gelegene Befunde\nwurden mit loparoskopisch assistierter Handanastomose behandelt.\nDie Beschwerden wurden deutlich reduziert: Defäkationsprobleme 88 %, Dyspareunie\n87 %, Unterleibschmerzen 96 %, Sexualität 75 %. Die Komplikationsrate war gering:\nAnastomosseninsuffizienz n=4 (2,8 %), Abszess 2 (1,4 %), Bluttransfusion 1 (0,7\n%), ausgeprägte Anastomosenstenose 6 (4,2 %).\nDie medikamentöse Therapie wird vornehmlich bei Patientinnen mit geringer Symptomatik\nangewandt. Die postoperative Behandlung obliegt Patientinnen die inkomplett operiert\nwurden oder einen dringenden Verdacht auf eine ausgeprägte Adenomyosis uteri und\nKinderwunsch haben.\nAbstract\nPatients with severe endometriosis involving the rectovaginal septum and bowel makes\nsurgery necessary. The laparoscopic approach offers the possibility to perform the\ncomplete resection with minimal invasiv techniques. Small lesions can be removed\nby full-thickness-resection. Large nodules make a segmental resection necessary.\nDeep lesions are resected with stapling devices. Nodules in the sigmoid ore removed\nby laparoscopic assisted hand-sewn anastomosis.\nResults: Between 3/96 and 7/03 142 patients with severe endometriosis involving the bowel\nhave been treated laparoscopically. The pre-operative complains have been reduced\nas following: dyschezio: 88 %, dyspareunia 87 %, chronic pelvic pain 96 %, disturbance\nof sexuality 75 %.
