Eine hohe Rezidivrate bedingt die Notwendigkeit wiederholter therapeutischer Interventionen. Entsprechend ist die Erkrankung mit einem hohen Leidensdruck für die betroffenen Patientinnen assoziiert.\nMaterial und Methoden\nAnhand der aktuellen Literatur wird der pathophysiologische Wissensstand bezüglich der Endometriose bestimmt.\nErgebnisse\nNeue Forschungsergebnisse tragen zu einem besseren Verständnis der Schmerzentstehung, der posttranskriptionellen Regulation und der Beteiligung von Stammzellen an der Ätiologie bei. Sog. mikroRNAs sind mit hoher Spezifität und Sensitivität nachweisbare Nukleinsäuren, die nichtinvasive Biomarker für die Diagnostik und den Therapieerfolg bei der Endometriose werden könnten. Die Endometriose präsentiert sich darüber hinaus als östrogenabhängige neuroinflammatorische Erkrankung, die in präklinischen Modellen durch Steroide moduliert werden kann. In gleicher Weise kann die Funktion und Rekrutierung endometrialer Stammzellen steroidal beeinflusst werden.\nSchlussfolgerung\nDie in präklinischen Modellen vielversprechend erscheinenden Daten bieten einen Ansatzpunkt zur Entwicklung neuer diagnostischer und therapeutischer Konzepte.\nAbstract\nBackground\nEndometriosis is associated with pelvic pain, painful periods, pain during sexual intercourse, and subfertility. In addition, the disease is associated with high levels of psychological stress.\nMaterials and methods\nBased on the current literature, the pathophysiological knowledge of endometriosis is presented.\nResults\nNew research results contribute to a better understanding of the mechanisms of endometriosis-associated pain, posttranscriptional regulatory mechanisms, and the role of stem cells in the etiology of the disease. microRNAs can be detected with high specificity and sensitivity and may serve as future biomarkers for the diagnosis of endometriosis and for monitoring therapeutic success. Moreover, endometriosis presents as an estrogen-dependent neuroinflammatory disease, which has been successfully modulated by steroids in preclinical models. Similarly, the function and recruitment of endometrial stem cells can be influenced by steroids.\nConclusion\nThe promising results of preclinical studies on these novel etiological concepts provide the basis for the development of new diagnostic and therapeutic concepts for endometriosis.\nLiteratur\nGiudice LC (2010) Clinical practice.
