Symptoms of the disease may be nonspecific or \nabsent. However, CE can be manifested by pain in the \nlower abdomen, abnormal bleeding and increased vag-\ninal discharge, as well as affect the susceptibility of the \nendometrium, which causes repeated pregnancy losses, \ninfertility and impaired embryo implantation [1, 2].\nChronic endometritis has been a subject of scientific \ninterest for many decades. Despite the significant prog-\nress in the study of chronic endometritis, the issue of op-\ntimizing diagnostic and therapeutic approaches remains \nrelevant [3]. The lack of uniform standards in treatment \nand diagnosis creates the need for new research to de-\nvelop more effective methods that can improve treat -\nment outcomes and increase the chances of successful \npregnancy in patients with this pathology [4].\nOne of the priorities of modern research is to cre-\nate specific and sensitive diagnostic methods that will \nhelp reduce the number of false-negative results [5]. \nThis is of great importance for improving treatment \noutcomes with antibiotic therapy and probiotics, which \nhave demonstrated the ability to restore the balance of \nendometrial microflora and reduce inflammation [2, 4].\nIn addition, a promising area is the study of the role \nof the immune system and proinflammatory cytokines \nin the formation of chronic endometritis, which can be \nthe basis for developing new therapeutic strategies [5, \n6]. Further research should be aimed not only at im-\nproving diagnosis and treatment but also at a deeper \nunderstanding of the individual characteristics of chron-\nic endometritis in different patients. This creates pros-\npects for personalized medicine, which will increase the \neffectiveness of treatment and significantly improve the \nchances of a successful pregnancy [7, 8]. \nThis approach can radically change the current meth-\nods of treating reproductive disorders associated with \nchronic endometritis and open up new opportunities for \nwomen who have difficulty realizing their reproductive \npotential [9].\nThe aim of the study.\nTo consider the main pathophysiological mechanisms \nof chronic endometritis, its impact on fertility, and mod-\nern approaches to diagnosing and treating this disease.\nObject and methods of the study.\nA systematic review and comparative analysis of \nscientific publications from PubMed, Google Scholar, \nResearchGate, and literature sources over the past five \nyears related to research in reproductive medicine on \nchronic endometritis and its impact on women’s repro-\nductive function was conducted. This inflammatory endometrial disease significantly affects reproduc-\ntive function. CE is associated with embryo implantation failures, reducing endometrial receptivity, which leads to a \ndecrease in the effectiveness of in vitro fertilization cycles. The disease is of significant medical and social importance \ndue to its increasing prevalence and difficulties in treatment, which worsen patients' quality of life.\nThe study aims to summarize current scientific data, including clinical guidelines, randomized trials, and rec -\nommendations, to improve the diagnosis and treatment of CE in women with infertility. The article describes the \npathophysiological mechanisms of the development of CE, including increased levels of proinflammatory cytokines \nand endometrial microflora disorders. The use of modern diagnostic methods, such as immunocytochemical detec -\ntion of CD138+ plasma cells and PCR, provides an accuracy of CE of up to 90%, significantly reducing the risk of false \nnegative results.\nTherapeutic approaches include antibiotic therapy with doxycycline and metronidazole, probiotics to normalize \nthe microflora, and immunomodulators to restore endometrial function. The integration of these methods can sig -\nnificantly reduce the frequency of failed IVF attempts and increase the chances of a successful pregnancy.\nIt is concluded that improving diagnostic approaches and therapeutic strategies is a key factor in improving \nthe effectiveness of infertility treatment. Optimizing cryoprotocols in patients with infertility of unknown genesis \nin the setting of chronic endometritis is a multifactorial process that includes correcting the endometrial condition, \nindividualizing treatment, and improving laboratory methods. This can improve the results of assisted reproductive \ntechnology programs and increase the pregnancy rate. The main causative agents of chronic \nendometritis are opportunistic pathogens, including vi-\nruses, bacteria and fungal infections. Among the most \ncommon pathogens are Escherichia coli, Enterococcus \nfaecalis, Mycoplasma, and various anaerobic bacteria \n[10, 11].\nChronic endometritis is often associated with sexual-\nly transmitted infections, such as chlamydia and gonor -\nrhoea, as well as secondary infections that occur after in-\nvasive procedures, such as abortions or uterine surgery \n[12].\nImmune response disorders play an important role in \nthe pathogenesis of chronic endometritis, contributing \nto the persistence of infections and the development of \ninflammatory processes. These disorders can manifest \nin increased plasma cells in the endometrium, which is \na key diagnostic criterion for chronic endometritis [13, \n14]. Additionally, impaired local immunity in the uterus, \nchanges in the cytokine profile, and activation of specific \ninflammatory mechanisms contribute to the progression \nof the disease [15].\nThe uterine microbiota also plays a significant role \nin the occurrence of CE. Dysbiosis, which means a dis -\nruption of the normal bacterial balance in the endome-\ntrium, can increase the number of pathogenic microor -\nganisms that contribute to inflammation [16]. Recent \nstudies have shown that the uterine microbiome of \nhealthy women differs from that of women with chronic \nendometritis, which may be an important aspect for fu -\nture diagnosis and therapy [17].\nIt has been suggested that chronic pelvic inflamma-\ntory disease (CPID) is autoimmune, triggered by a specif-\nic infectious agent. Under prolonged exposure to bacte-\nria or viruses, these diseases can become self-sustaining \npathological processes [18, 19]. It should be noted that \nthe activation of local immune mechanisms in the endo-\nmetrium can significantly affect the process of embryo \nimplantation, which often leads to several reproductive \nproblems, such as infertility and repeated pregnancy \nlosses [20]. This process can be disrupted by a decrease \nin the sensitivity of endometrial receptors to hormones \nor changes in the local environment, which makes it less \nfavourable for embryo attachment [21]. This emphasizes \nthe need for a detailed study of the autoimmune aspects \nof CPID to better understand their impact on reproduc -\ntive health and develop effective therapies for women \nfacing these difficulties [22].\nClinical manifestations of chronic endometritis (CE) \nare often absent or have no specific signs, which greatly \ncomplicates the diagnosis process. Usually, the disease \nis asymptomatic or accompanied by minor complaints, \nsuch as irregular menstruation, lower abdominal pain, \nand sometimes dysuria or fever [23]. The most charac-\nteristic signs that suggest the presence of CE are prob -\nlems with reproductive function, including infertility and \nrepeated pregnancy loss [24].\nDiagnosis of chronic endometritis is based on the \nuse of both invasive and non-invasive methods. One of \nthe most accurate approaches is an endometrial biop -\nsy followed by morphological analysis of the tissue. The \ndetection of plasma cells in endometrial biopsies is con-\nsidered a key morphologic indicator of the presence of \nCE [25, 26].\nToday, one of the leading methods for confirming \nthe diagnosis is using immunohistochemical staining to \ndetect Syndecan-1 (CD138+), a proteoglycan present \non the surface of plasma cells and keratinocytes. This \napproach can significantly increase the sensitivity and \nspecificity of the diagnosis of CE, providing more ac-\ncurate detection of plasma cells [27, 28]. According to \nmany researchers, immunohistochemistry is the most \nsensitive and specific method for identifying plasma \ncells in endometrial tissue [29]. Immunohistochemical \nexamination demonstrates higher efficiency than other \nmethods, such as methyl green pyronin staining. For the \nmethyl green pyronin method, the sensitivity is 78%, \nand the specificity is 65%, while immunohistochemistry \nusing CD138 is considered the “gold standard” [30].\nChronic endometritis is manifested by nonspecific \nmorphological signs, including lymphocytic and leuko-\ncytic infiltration, localized fibrosis in the stroma, and \nsclerosis of the walls of the spiral arteries. One of the key \ndiagnostic markers of CE is the presence of plasma cells \nin endometrial biopsies obtained during the early phase \nof the menstrual cycle [31, 32]. These findings lead some \nresearchers to classify CE as a local inflammatory pro-\ncess characterized by atypical plasma cell infiltration in \nthe endometrial stroma, indicating prolonged inflamma-\ntion [33].\nFor additional diagnostics, hysteroscopy is often \nused to visually assess the condition of the inner surface \nof the uterus. This method can detect specific changes \nin the endometrium, such as hyperemia or focal lesions \n[34]. In women diagnosed with CE, the likelihood of \npregnancy decreases even if in vitro fertilization (IVF) is \nused because the inflamed endometrium creates unfa-\nvourable conditions for embryo implantation [35]. Even \nwhen implantation occurs, patients with CE often expe-\nrience early pregnancy loss due to the inability of the \nendometrium to provide adequate support for fetal de-\nvelopment.\nAn essential aspect of the treatment of women with \nchronic endometritis in preparation for IVF is to take into \naccount the activity of this disease. Conducting thera-\npy to eliminate the manifestations of CE before starting \nan IVF cycle can significantly increase the likelihood of \nsuccessful embryo implantation and reduce the risk of \nearly miscarriages [36]. In some cases, it is advisable to \npostpone the IVF procedure until a stable improvement \nin the clinical picture of the disease is achieved.\nTherapy for chronic endometritis aims to reduce in -\nflammation, restore the functional capacity of the endo-\nmetrium, and improve the chances of successful concep-\ntion and pregnancy. The selection of specific therapeutic \ntactics depends on the severity of clinical manifestations \nand the identified cause of the disease.\nThe use of antibiotics is one of the main methods of \ntreating chronic endometritis, especially if a bacterial in-\nfection causes the disease. The use of broad-spectrum \nantibacterial agents, such as doxycycline and metronida-\nzole, helps to reduce the number of opportunistic patho-\ngens in the endometrium and reduce inflammation [37]. \nHowever, it should be noted that antibiotics often re -\nquire a combination with other treatments to achieve a \n\nISSN 2077-4214. Вісник проблем біології і медицини – 2024 – Вип. 4 (175) / Bulletin of problems in biology and medicine – 2024 – Issue 4 (175)\n156\nОГЛЯДИ ЛІТЕРАТУРИ / LITERATURE REVIEWS\nstable effect, as in some cases, they cannot completely \neliminate the chronic process.\nIn addition, immunomodulators play an important \nrole in the treatment of CE, which aims to correct the im-\nmune response disorders that accompany this disease. \nFor example, drugs that activate local immunity can \nhelp reduce the inflammatory response in endometrial \ntissues and improve the functional state of the uterine \nmucosa [38]. Several studies have shown that the use \nof immunomodulators can improve the chances of suc-\ncessful implantation and gestation in women diagnosed \nwith CE, reducing the incidence of early pregnancy loss \n[39].\nConclusions.\nChronic endometritis is a complex inflammatory \ndisease that significantly affects women’s reproductive \nfunction. The literature analysis has shown that this pa-\nthology negatively affects embryo implantation during \nIVF/ICSI cycles, causing impaired decidual lining matu-\nration and the development of endometrial hormonal \nresistance. Such complex relationships require further \nresearch to understand their nature and impact on \nwomen’s reproductive health better.\nProspects for further research.\nGiven the limited scientific data available on effec -\ntive methods of treating infertility in chronic endome-\ntritis, it is important to develop a new approach that \nconsiders the individual characteristics of patients and \nthe specifics of the inflammatory process. The recom-\nmended strategies should not only improve diagnostic \nmethods such as immunocytochemistry or PCR but also \ninclude the development of personalized therapy regi-\nmens based on the immune status of patients and the \ncharacteristics of their microbiome. ХЕ пов'язаний із невдачами імплантації ембріона, знижуючи \nрецептивність ендометрію, що призводить до зменшення ефективності циклів екстракорпорального \nзапліднення. Захворювання має значне медико-соціальне значення через зростаючу поширеність і \nскладнощі лікування, які погіршують якість життя пацієнток.\nМетою дослідження є узагальнення сучасних наукових даних, включаючи клінічні настанови, \nрандомізовані дослідження та рекомендації, для вдосконалення діагностики й лікування ХЕ у жінок із \nбезпліддям. У статті описані патофізіологічні механізми розвитку ХЕ, що включають підвищення рівнів \nпрозапальних цитокінів і порушення мікрофлори ендометрію. Використання сучасних діагностичних \nметодів, таких як імуноцитохімічне визначення CD138+ плазматичних клітин і ПЛР, забезпечує точність \nвиявлення ХЕ до 90%, що значно знижує ризик хибнонегативних результатів.\nСеред терапевтичних підходів розглянуто антибіотикотерапію із застосуванням доксицикліну та \nметронідазолу, пробіотики для нормалізації мікрофлори, а також імуномодулятори для відновлення \nфункціональності ендометрію. Інтеграція цих методів дозволяє значно знизити частоту невдалих спроб \nЕКЗ і підвищити шанси на успішну вагітність.\nЗроблено висновок, що вдосконалення діагностичних підходів і терапевтичних стратегій є ключовим \nфактором у підвищенні ефективності лікування безпліддя. Оптимізація кріопротоколів у пацієнток \nіз безпліддям нез’ясованого ґенезу на фоні хронічного ендометриту є багатофакторним процесом, \nякий включає корекцію стану ендометрію, індивідуалізацію лікування та вдосконалення лабораторних \nметодів. Це дозволяє покращити результати програм допоміжних репродуктивних технологій та \nпідвищити частоту настання вагітності. Симптоматика захворюван-\nня може бути неспецифічною або взагалі відсутньою. \nОднак ХЕ може проявлятися болем у нижній частині \nживота, аномальними кровотечами та збільшенням \nвагінальних виділень, а також впливати на сприй -\nнятливість ендометрію, що викликає повторні втрати \nвагітності, безпліддя та порушення імплантації емб-\nріона [1, 2].\nХронічний ендометрит є предметом наукового \nінтересу вже багато десятиліть. Попри великий про-\nгрес у вивченні хронічного ендометриту, питання \nоптимізації діагностичних та терапевтичних підходів \nзалишається актуальним [3]. Відсутність єдиних стан-\nдартів у лікуванні та діагностиці створює необхідність \nнових досліджень для розробки дієвіших методів, які \nзможуть покращити результати лікування та підви-\nщити шанси на успішну вагітність у пацієнток з даною \nпатологією [4].\nОдним із пріоритетів сучасних досліджень є ство-\nрення специфічних і чутливих методів діагностики, \nщо допоможе знизити кількість хибнонегативних ре-\nзультатів [5]. Це має велике значення для покращен-\nня результатів лікування за допомогою антибактері-\nальної терапії та пробіотиків, які продемонстрували \nздатність відновлювати баланс мікрофлори ендоме-\nтрію та знижувати запальні процеси [2, 4].\nКрім того, перспективним напрямом є вивчен -\nня ролі імунної системи та прозапальних цитокінів \nу формуванні хронічного ендометриту, що може \nстати основою для створення нових терапевтичних \nстратегій [5, 6]. Подальші дослідження повинні бути \nспрямовані не тільки на вдосконалення діагностики \nта лікування, але й на глибше розуміння індивідуаль-\nних характеристик перебігу хронічного ендометриту \nу різних пацієнток. Це створює перспективи для пер -\nсоналізованої медицини, що сприятиме підвищенню \nефективності лікування та значному покращенню \nшансів на успішну вагітність [7, 8]. \nТакий підхід може радикально змінити сучасні \nметоди лікування репродуктивних порушень, пов’я -\nзаних з хронічним ендометритом, і відкрити нові \nможливості для жінок, які мають труднощі із реаліза-\nцією свого репродуктивного потенціалу [9].\nМета дослідження.\nРозглянути основні патофізіологічні механізми \nрозвитку хронічного ендометриту, його вплив на \nфертильність, а також сучасні підходи до діагностики \nта лікування цього захворювання.\nОб’єкт і методи дослідження.\nПроведено систематичний огляд та порівняльний \nаналіз наукових публікацій із наукометричних баз \nPubMed, Google Scholar, ResearchGate та літературних \nджерел за останні 5 років що стосуються досліджень \nв області репродуктивної медицини щодо хроніч -\nного ендометриту та його впливу на репродуктивну \nфункцію жінок. Критеріями відбору були оригінальні \nдослідження, оглядові статті та метааналізи, які охо-\nплюють аспекти патогенезу, діагностики та лікування \nХЕ, а також його зв’язок із безпліддям та результата-\nми лікування за допомогою ЕКЗ.\nОсновна частина.\nХронічний ендометрит є тривалою запальною ре-\nакцією ендометрію, що зазвичай викликана інфекцій-\nним ураженням. Основними збудниками хронічного \nендометриту є умовно-патогенні мікроорганізми, \nзокрема віруси, бактерії та грибкові інфекції. Серед \nнайбільш розповсюджених патогенів виділяються \nEscherichia coli , Enterococcus faecalis, Mycoplasma та \nрізноманітні анаеробні бактерії [10, 11].\nХронічний ендометрит часто пов’язують із інфек-\nціями, що передаються статевим шляхом, такими як \nхламідіоз і гонорея, а також із вторинними інфекція-\nми, що виникають після інвазивних процедур, таких \nяк аборти або хірургічні втручання на матці [12].\nВ патогенезі хронічного ендометриту важливу \nроль відіграють порушення імунної відповіді, які \nсприяють персистенції інфекцій та розвитку запа -\nлених процесів. Ці порушення можуть проявлятися \nпідвищенням кількості плазматичних клітин в ендо-\nметрії, які є ключовим діагностичним критерієм для \nдіагностики хронічного ендометриту [13, 14]. Додат -\nково, порушення місцевого імунітету в матці, зміни \nв цитокіновому профілі та активація специфічних за-\nпальних механізмів сприяють прогресуванню захво-\nрювання [15].\nМікробіота матки також відіграє суттєву роль у \nвиникненні ХЕ. Дисбіоз, що означає порушення нор-\nмального бактеріального балансу в ендометрії, може \nпризводити до збільшення чисельності патогенних \nмікроорганізмів, які сприяють запаленню [16]. Нещо-\nдавні дослідження виявили, що мікробіом матки у \nздорових жінок відрізняється від мікробіому жінок з \nхронічним ендометритом, що може стати важливим \nаспектом для діагностики та терапії в майбутньому \n[17].\nІснує припущення, що хронічні запальні захворю-\nвання органів малого таза (ВЗОМТ) мають аутоімунну \nприроду, яку запускає певний інфекційний агент. Під \nтривалим впливом бактерій або вірусів ці захворю-\nвання можуть перетворитися на самопідтримувані \nпатологічні процеси [18, 19]. Слід зазначити, що ак-\nтивація локальних імунних механізмів в ендометрії \nможе суттєво впливати на процес імплантації емб-\nріона, що часто призводить до ряду репродуктивних \nпроблем, таких як безпліддя та повторні втрати вагіт-\nності [20]. Цей процес може порушуватися через зни-\nження чутливості рецепторів ендометрію до гормонів \nабо зміни в локальному середовищі, що робить його \nменш сприятливим для прикріплення ембріона [21]. \nЦе підкреслює необхідність детального вивчення ау-\nтоімунних аспектів хронічних ВЗОМТ для кращого ро-\nзуміння їхнього впливу на репродуктивне здоров’я та \nрозробки ефективних лікувальних методів для жінок, \nякі стикаються з цими труднощами [22].\nКлінічні прояви хронічного ендометриту (ХЕ) часто \nвідсутні або не мають специфічних ознак, що значно \nускладнює процес встановлення діагнозу. Зазвичай \nзахворювання має безсимптомний перебіг або су -\nпроводжується незначними скаргами, такими як не -\nрегулярні менструації, біль у нижній частині живота, \nа інколи і дизуричні явища або підвищення темпера-\nтури тіла [23]. Найбільш характерними ознаками, які \nдозволяють запідозрити наявність ХЕ, є проблеми з \nрепродуктивною функцією, включаючи безпліддя та \nповторні втрати вагітності [24].\nДіагностування хронічного ендометриту базуєть-\nся на застосуванні як інвазивних, так і неінвазивних \nметодів. Одним із найточніших підходів є біопсія \nендометрію з подальшим морфологічним аналізом \nтканин. Виявлення плазматичних клітин в біоптатах \n\nISSN 2077-4214. Вісник проблем біології і медицини – 2024 – Вип. 4 (175) / Bulletin of problems in biology and medicine – 2024 – Issue 4 (175)\n158\nОГЛЯДИ ЛІТЕРАТУРИ / LITERATURE REVIEWS\nендометрію вважається ключовим морфологічним \nпоказником наявності ХЕ [25, 26].\nНа сьогодні одним із провідних методів підтвер-\nдження діагнозу є використання імуногістохімічного \nфарбування для виявлення Syndecan-1 (CD138+), про-\nтеоглікану, який присутній на поверхні плазматичних \nклітин та кератиноцитів. Такий підхід дозволяє знач -\nно підвищити чутливість та специфічність діагностики \nХЕ, забезпечуючи більш точне виявлення плазматич-\nних клітин [27, 28]. На думку багатьох дослідників, \nімуногістохімія є найбільш чутливим та специфічним \nметодом ідентифікації плазматичних клітин в ендо-\nметріальній тканині [29]. Порівняно з іншими мето-\nдами, зокрема фарбуванням метиловим зеленим пі-\nроніном, імуногістохімічне дослідження демонструє \nвищу ефективність: для методу метилового зеленого \nпіроніну чутливість складає 78%, а специфічність – \n65%, тоді як імуногістохімія з використанням CD138 \nвважається «золотим стандартом» [30].\nХронічний ендометрит проявляється через не -\nспецифічні морфологічні ознаки, серед яких лімфо -\nцитарна та лейкоцитарна інфільтрація, локальний \nфіброз у стромі, а також склерозування стінок спі-\nральних артерій. Одним із ключових діагностичних \nмаркерів ХЕ є наявність плазматичних клітин у біоп-\nтатах ендометрію, отриманих під час ранньої фази \nменструального циклу [31, 32]. Ці знахідки змушують \nдеяких дослідників класифікувати ХЕ як місцевий \nзапальний процес, що характеризується атиповою \nплазмоцитарною інфільтрацією в стромі ендометрію, \nщо свідчить про тривалу активність запалення [33].\nДля додаткової діагностики часто застосовується \nгістероскопія, яка дозволяє візуально оцінити стан \nвнутрішньої поверхні матки. Цей метод дає змогу ви-\nявити специфічні зміни ендометрію, зокрема, гіпере-\nмію або фокальні ураження [34]. У жінок із діагносто-\nваним ХЕ знижується ймовірність настання вагітності \nнавіть за умови застосування екстракорпорального \nзапліднення (ЕКЗ), адже запалений ендометрій ство-\nрює несприятливі умови для імплантації ембріона \n[35]. Навіть коли імплантація відбувається, у паці -\nєнток із ХЕ часто виникають ранні втрати вагітності \nчерез нездатність ендометрію забезпечити належну \nпідтримку розвитку плода.\nВажливим аспектом лікування жінок із хронічним \nендометритом під час підготовки до ЕКЗ є врахуван -\nня активності цього захворювання. Проведення те-\nрапії для усунення проявів ХЕ перед початком циклу \nЕКЗ може значно збільшити вірогідність успішної \nімплантації ембріона та зменшити ризик ранніх ви -\nкиднів [36]. В окремих випадках доцільно відкласти \nпроцедуру ЕКЗ до досягнення стабільного поліпшен-\nня клінічної картини захворювання.\nТерапія хронічного ендометриту спрямована на \nзниження рівня запального процесу, відновлення \nфункціональної здатності ендометрію та покращення \nшансів на успішне зачаття і виношування вагітності. \nПідбір конкретної терапевтичної тактики залежить \nвід тяжкості клінічних проявів та виявленої причини \nзахворювання.\nВикористання антибіотиків є одним із основних \nметодів лікування хронічного ендометриту, особли-\nво якщо захворювання викликане бактеріальною \nінфекцією. Використання антибактеріальних засобів \nширокого спектра, зокрема, таких як доксициклін і \nметронідазол, допомагає зменшити кількість умов -\nно-патогенних мікроорганізмів у ендометрії та зни -\nзити рівень запалення [37]. Проте слід враховувати, \nщо застосування антибіотиків часто потребує поєд-\nнання з іншими методами лікування для досягнення \nстабільного ефекту, оскільки у деяких випадках вони \nне здатні повністю усунути хронічний процес.\nКрім того, в терапії ХЕ важливу роль відіграють іму-\nномодулятори, які спрямовані на корекцію порушень \nімунної відповіді, що супроводжують це захворюван-\nня. Наприклад, препарати, що активізують місцевий \nімунітет, можуть сприяти зменшенню запальної ре -\nакції в тканинах ендометрію та покращувати функ-\nціональний стан слизової оболонки матки [38]. Ряд \nдосліджень демонструє, що використання імуномоду-\nляторів здатне покращити шанси на успішну імплан-\nтацію та виношування у жінок із діагностованим ХЕ, \nзнижуючи частоту ранніх втрат вагітності [39].\nВисновки.\nХронічний ендометрит – це складне запальне \nзахворювання, що суттєво впливає на репродуктив -\nну функцію жінок. Проведений аналіз літературних \nджерел показав, що ця патологія негативно впливає \nна імплантацію ембріонів під час циклів ЕКЗ/ICSI, \nспричиняючи порушення дозрівання децидуальної \nоболонки та розвиток гормональної резистентності \nендометрію. Такі складні взаємозв’язки вимагають \nподальших досліджень для більш глибокого розумін-\nня їхньої природи та впливу на репродуктивне здо-\nров’я жінок.\nПерспективи подальших досліджень.\nЗважаючи на обмеженість наявних наукових \nданих щодо ефективних методів лікування безплід -\nдя на фоні хронічного ендометриту, важливим є роз-\nробка нового підходу, що враховує індивідуальні осо-\nбливості пацієнток та специфіку запального процесу. \nРекомендовані стратегії повинні не лише вдоскона-\nлювати діагностичні методи, такі як імуноцитохімія \nабо ПЛР, але й передбачати розробку персоналізо-\nваних схем терапії з урахуванням імунного статусу \nпацієнток та особливостей їх мікробіому. Це дозво-\nлить підвищити ефективність лікування та збільшити \nшанси на успішне настання вагітності, зменшуючи \nчастоту невдалих спроб ЕКЗ.\nЗастосування нових знань про роль запалення та \nйого регуляції за допомогою сучасних терапевтичних \nпідходів, зокрема антибактеріальної та пробіотичної \nтерапії, може значно покращити результати лікуван-\nня безпліддя у пацієнток з хронічним ендометритом.\nReferences / Література\n1. Хронічний ендометрит (ХЕ) є запальним захворюванням ендометрію, що пов’язане з репродук -\nтивними порушеннями, включаючи безпліддя та невдалі спроби екстракорпорального запліднення (ЕКЗ). \nНа сьогоднішній день не розроблено єдиного стандарту або загальноприйнятих критеріїв для діагностики \n\nISSN 2077-4214. Хоча більшість фахівців сходяться на думці, що для гістопатологічної діагностики \nхронічного ендометриту важливим є виявлення множинних (двох і більше) ендометріальних стромальних \nплазмоцитів (ESPC), оптимальне граничне значення їх щільності або кількості залишається невизначеним. \nОглядова стаття висвітлює сучасні підходи до діагностики та лікування ХЕ, а також його патогенетичний вплив \nна безпліддя. У роботі розглядаються основні патофізіологічні механізми ХЕ, інноваційні методи діагностики \nта перспективи покращення репродуктивних результатів при вдосконалених методах діагностики та викорис-\nтанні сучасної терапії.\nМетою дослідження було розглянути основні патофізіологічні механізми розвитку хронічного ендоме-\nтриту, його вплив на фертильність, а також сучасні підходи до діагностики та лікування цього захворювання.\nОб’єкт і методи дослідження. Проведено систематичний огляд та порівняльний аналіз наукових публі -\nкацій із наукометричних баз PubMed, Google Scholar, ResearchGate та літературних джерел за останні 5 років \nщо стосуються досліджень в області репродуктивної медицини щодо хронічного ендометриту та його впливу \nна репродуктивну функцію жінок. Критеріями відбору були оригінальні дослідження, оглядові статті та мета-\nаналізи, які охоплюють аспекти патогенезу, діагностики та лікування ХЕ, а також його зв’язок із безпліддям та \nрезультатами лікування за допомогою ЕКЗ.\nОсновна частина. Аналіз літератури виявив, що хронічний ендомерит характеризується підвищенням \nрівнів прозапальних цитокінів, що створює хронічне запальне середовище в ендометрії та ускладнює імп-\nлантацію ембріона, як наслідок наявність ХЕ може знижувати рівень рецептивності ендометрію на 25%, що \nсуттєво знижує ефективність циклів ЕКЗ у таких пацієнток. Найбільш чутливими методами діагностики ХЕ є \nімуноцитохімічне визначення CD138+ плазматичних клітин та молекулярна діагностика за допомогою ПЛР. \nВикористання цих методів дозволяє підвищити точність виявлення ХЕ до 90% та уникнути хибнонегативних \nрезультатів, які часто зустрічаються при традиційній гістології. Застосування антибіотикотерапії та пробіотиків \nу пацієнток з ХЕ сприяє відновленню нормальної мікрофлори ендометрію та знижує рівень запалення. Це \nпризводить до підвищення частоти живонароджуваності у пацієнток з попередньо невдалими спробами ЕКЗ. \nПроте, ефективність лікування значною мірою залежить від правильного підбору антибіотиків та тривалості \nкурсу терапії.\nВисновки. Аналіз сучасної літератури показав, що хронічний ендометрит є важливою причиною репродук-\nтивних порушень у жінок, що включають безпліддя та невдалі спроби ЕКЗ. Своєчасна діагностика та комплек-\nсне лікування ХЕ дозволяють покращити результати лікування безпліддя та підвищити ймовірність успішної \nвагітності. Використання сучасних діагностичних методів, таких як імуноцитохімія значно підвищує чутливість \nвиявлення ХЕ та забезпечує індивідуалізований підхід до терапії. Подальші дослідження необхідні для визна-\nчення оптимальних терапевтичних стратегій та довготривалих результатів лікування пацієнток з ХЕ.\nКлючові слова: хронічний ендометрит, кріопротокол, безпліддя нез’ясованого ґенезу, допоміжні репро-\nдуктивні технології, підготовка ендометрію.\nCHRONIC ENDOMETRITIS AS A CAUSE OF REPRODUCTIVE FUNCTION DISORDERS IN WOMEN (LITERATURE \nREVIEW)\nKhmil S. Chronic endometritis (CE) is an inflammatory condition of the endometrium associated with \nreproductive disorders, including infertility and unsuccessful attempts at in vitro fertilization (IVF). Currently, there \nis no single standard or universally accepted criteria for diagnosing chronic endometritis. While most experts agree \nthat the detection of multiple (two or more) endometrial stromal plasma cells (ESPC) is crucial for histopathological \ndiagnosis, the optimal threshold for ESPC density or number remains undetermined. This review article highlights \ncurrent approaches to the diagnosis and treatment of CE, as well as its pathogenetic impact on infertility. The study \nexamines the main pathophysiological mechanisms of CE, innovative diagnostic methods, and the prospects for \nimproving reproductive outcomes through enhanced diagnostic techniques and modern therapies.\nThe aim of the study  was to examine the primary pathophysiological mechanisms of chronic endometritis, its \nimpact on fertility, as well as current approaches to the diagnosis and treatment of this condition.\nObject and research methods.  A systematic review and comparative analysis of scientific publications from \nbibliometric databases such as PubMed, Google Scholar, ResearchGate, and literary sources over the past five years \nwas conducted, focusing on studies in reproductive medicine related to chronic endometritis and its impact on \nwomen’s reproductive function. . Selection criteria included original studies, review articles, and meta-analyses \ncovering aspects of pathogenesis, diagnosis, and treatment of CE, as well as its association with infertility and IVF \noutcomes.\nMain part. Literature analysis revealed that CE is characterized by elevated levels of pro-inflammatory cytokines, \nsuch as IL-6 and TNF-α, creating a chronic inflammatory environment in the endometrium and hindering embryo \nimplantation. As a result, CE can reduce endometrial receptivity by 25%, significantly decreasing the effectiveness of \nIVF cycles in affected patients. The study also confirmed that women with CE who underwent a course of antibiotic \ntherapy experienced a significant increase in pregnancy rates compared to untreated patients. The most sensitive \nmethods for diagnosing CE include immunocytochemical identification of CD138+ plasma cells and molecular \ndiagnostics using PCR. These methods enhance the accuracy of CE detection up to 90% and help avoid false-negative \nresults, which are common with traditional histology. The use of antibiotic therapy and probiotics in patients with CE \npromotes the restoration of normal endometrial microbiota and reduces inflammation levels. This leads to higher \nlive birth rates in patients with previously unsuccessful IVF attempts. However, treatment effectiveness largely \ndepends on the proper selection of antibiotics and the duration of the therapy course.\n\nISSN 2077-4214. Analysis of current literature shows that chronic endometritis is a significant cause of reproductive \ndisorders in women, including infertility and failed IVF attempts. Timely diagnosis and comprehensive treatment \nof CE can improve infertility treatment outcomes and increase the likelihood of successful pregnancy. The use of \nmodern diagnostic methods, such as immunocytochemistry, significantly enhances the sensitivity of CE detection \nand ensures a personalized approach to therapy. V.\nMODERN PERSPECTIVE ON THE PREVALENCE AND DEVELOPMENT OF DISEASES \nUNDER THE INFLUENCE OF CHLAMYDIA TRACHOMATIS (A LITERATURE REVIEW)\nDnipro State Medical University (Dnipro, Ukraine)\nverashatornaya67@gmail.com\nChlamydial infections constitute a group of infectious diseases of anthropozoonotic and zoonotic origin, which \nare responsible for a wide range of diverse pathologies in more than half a billion people worldwide. According to \nthe World Health Organization (WHO), urogenital chlamydia is the second most prevalent sexually transmitted dis-\nease (STD) after trichomoniasis. It is estimated that over 90 million new cases of Chlamydia trachomatis infection \noccur annually worldwide, resulting in economic losses amounting to billions of dollars. Ukraine is no exception to \nthis trend, with thousands of new cases of diseases caused by this type of chlamydia registered annually in hospitals \nand laboratories. However, many cases remain undiagnosed due to incomplete diagnostics, the variety of target \nsites affected in the human body, or the long incubation period. Chlamydia trachomatis is an obligate intracellular, \ngram-negative, immobile bacterium that is the causative agent of trachoma, sexually transmitted diseases, certain \nforms of arthritis, conjunctivitis, and pneumonia in newborns. Furthermore, infection with Chlamydia trachomatis \nincreases the risk of HIV infection due to the weakened immunity of the patient.\nThe aim of this study was to search and analyse existing literature on the bacterium's life cycle, transmission \nroutes, the impact of Chlamydia trachomatis infection on the human body, and prevention methods. A review of \ncurrent medical and biological scientific publications revealed that the morphogenesis of Chlamydia trachomatis \nand its impact on the human body, particularly in chronic disease forms, remain insufficiently studied and require \nfurther research, including interdisciplinary approaches. Furthermore, the number of individuals infected with this \nmicroorganism is increasing, underscoring the necessity for educational initiatives among the general population, \nparticularly in high-risk groups.\nKey words: Chlamydia trachomatis, reproductive system, congenital chlamydia, pathogenesis, diagnosis, and \nprevention of chlamydial infection.\nConnection of the publication with planned re-\nsearch works. \nThe experimental study was conducted as part of \na scientific research project led by the Department of \nMedical Biology, Pharmacognosy, and Botany at Dnipro \nState Medical University.
