The authors have responded excellently to their task. It is now our great pleasure to leave the discipline of writing chapters that are controlled by references and data and simply speculate on where advances in our understanding and treatment of this disease may occur over the next five to ten years. This chapter will be a combination of both our views and should give the perspective from both the United States and Europe.
