Data may be preliminary.\nMethodsPROSPERO registration was performed prior to database searches on Pubmed, OVID, AMED,\nEmbase, Emcare, Ovid MEDLINE(R), Web of Science, PsycINFO, CINAHL, Cochrane Central to identify\nstudies that reported impact on mental health of hormones or hormone-blocking treatments for women with\nendometriosis. We then systematically searched and synthesised the available evidence.\nResults466 studies were identiﬁed from databases/registers; 464 studies screened, 101 studies sought for\nretrieval and 101 studies assessed for eligibility, 34 studies were excluded, and 67 studies were included in the\nreview. Of the 67 studies included 42 studies showed an improvement in mental health, 21 studies showed a\nworsening in mental health and 4 studies did not show any change.\nDiscussion\nMental health scores before and after treatment generally showed a signiﬁcant improvement in depression\nand anxiety and improved quality of life. However, severe cases of de-novo depression brought on by hormonal\nendometriosis treatments were reported. A subsect of patients with endometriosis appears to be susceptible\nto negative mental health side eﬀects.\nConclusion\nWe can reassure patients that overall hormonal treatments improve mental health, but some individuals\nwill have an intolerance to progestin or hormone-blockers, which can result in a decline in mental health.\nAdequately powered studies are needed to qualify this eﬀect. Accurate reporting of side eﬀects is mandatory,\neven for oﬀ-license indications.\nKeywords: Endometriosis, Hormonal treatment, mental health, depression, anxiety\nWhat is already known on this topic:\nFew large-scale studies exist of hormonal and hormone-blocking treatments for endometriosis which are po-\nwered to assess the eﬀect on mental wellbeing.\nWhat this study adds:\nThis study is the ﬁrst to systematically review all available information published on the eﬀects on mental\nhealth for patients receiving medical hormonal and anti-hormonal therapies for endometriosis and highlights\nknowledge gaps.\nHow this study might aﬀect research, practice or policy:\nOur study provides patients and health care practitioners with current evidence-based information regarding\nthe impact on mental health with these treatments. We call for improved side eﬀect reporting/monitoring for\nadverse mental health eﬀects by clinicians and adequate design for future studies.\nINTRODUCTION\nIn this study, the terms ’woman’ and ’women’ are used to refer to individuals assigned female at birth who\nidentify as women. However, we recognise and respect that gender identity is personal and diverse, and that\nnot everyone assigned female at birth identiﬁes as a woman. The use of these terms reﬂects the study’s focus\nand language, but we acknowledge that gender identity can be ﬂuid and not solely deﬁned by biological sex.\nEndometriosis is a neuro-inﬂammatory condition aﬀecting 1 in 10 women of reproductive age[1] equating to\n190 million people globally[2]. Endometriosis can cause debilitating pain and subfertility resulting in poor\nphysical, mental and emotional health. The economic burden of this disease is vast, with estimated costs of\n£1130 to £15,676 per patient per year[3, 4].\nTherapeutic approaches comprise of hormonal suppression either for primary disease control or secondary\nprophylaxis, alongside holistic management strategies, analgesia and surgical interventions. NICE[5] recom-\nmends hormonal treatments as ﬁrst-line management of the condition. Progestin based therapies suppress\nectopic endometrium-like tissue and thus help reduce inﬂammation and improve pain symptoms. Data may be preliminary.\ncan be given with or without oestrogen, as injections, tablets or an intra-uterine system. In addition, higher\nProgestin doses additionally lower oestrogen by supressing ovulation, leading to further endometrial suppres-\nsion, as do Gonadotrophin Releasing Hormone (GnRH) antagonists or continuous agonists. 30% of women\nare “dissatisﬁed”, and 10% “very dissatisﬁed”, with Progestins either due to lack of eﬃcacy or side eﬀects[6].\nSex steroids however are powerful neurotransmitters, and their manipulation and ﬂuctuations are thought to\nbe a major biological factor causing psychological side eﬀects, such as depression and anxiety[7]. Women are\ntwice as likely as men to develop anxiety and depression[8, 9] and those with endometriosis have been shown\nto have poorer baseline mental health[10] likely resulting from the complex cause and eﬀect relationship\nbetween the depression, anxiety and living with chronic pain. Living with a chronic health condition that\nhas no known cause, no cure, and limited treatment options, compounded by a lack of mental health support,\nsigniﬁcantly worsens mental well-being[11].\nThe British National Formulary (BNF) report depressed mood as a “common or very common” side eﬀect\nof the combined oral contraceptive pill in the average population[12], therefore we would expect this to be\nat least this amongst patients with endometriosis, if not more prevalent given the known poorer baseline\nmental health.\nIn our large London-based tertiary endometriosis centre a considerable proportion of endometriosis patients\nreport mental health side eﬀects from hormonal or hormone-blocking treatments, or do not want to try them.\nThis is echoed by our work with focus groups[13].\nFurthermore, ‘hormonophobia’, fuelled by social media posts about adverse eﬀects from contraceptive hormo-\nnes[14] inﬂuences whether patients would accept this treatment. Conversely, we also observe in our patients\nthat mental health improves, when pain responds to treatment.\nWe systematically searched and synthesised the available body of evidence to understand how hormonal and\nhormonal blocking treatment may aﬀect the mental health of women with endometriosis. Understanding\nthat these women may already have adversely impacted mental health, we wanted to determine\nthe frequency and severity of mental health side eﬀects\nif mental health side eﬀects have been adequately studied to help women make informed evidence-based\nchoices\nMETHODS\nAsystematicreviewwasperformedusingaprospectivelyregisteredprotocol(PROSPEROCRD42023366826,\n25/01/2023). Findings are reported in line with PRISMA guidelines. The search was performed on numerous\ndatabases; Pubmed, OVID, AMED, Embase, Emcare, Ovid MEDLINE(R), Web of Science, PsycINFO,\nCINAHL, Cochrane Central to identify studies that reported impact of hormones and hormone-blocking\ntreatments on mental health for women with endometriosis. Searches commenced from inception to 15th\nFebruary 2025 without language restrictions and were limited to human studies.\nAn expert patient from the charity Endometriosis-UK co-authored our paper to ensure our narrative review\nadequately reﬂected lived experience.\nWe studied changes in mental health in women with endometriosis before and after receiving hormones or\nhormone-blocking agents, using the PICOS format[15]:Population: Women with endometriosis.\nIntervention:Hormonal and hormone-blocking treatment.\nComparators: Non-hormonal treatment, hormonal or hormone-blocking treatment other than the inter-\nvention, waiting list controls and placebo or no comparator.\nOutcomes: Mental health before and after treatment.\nStudy designs: Randomised control trials, non-randomised control trials, cohort studies, cross-sectional\nstudies, cohort series, case reports and before-and-after studies were included. Papers without\nmethodological details were not included.\nSearch terms were generated to include all terms relating to mental and emotional wellbeing with a focus on\ndepression and anxiety and a list of quality-of-life scoring systems to assess pre and post treatment outcomes\nby three reviewers (SMS, KM & EB). The search is detailed in supplementary material. A manual search\nof reference lists of included articles, as well as backward and forward citation tracking, supplemented the\ndatabase search. When clariﬁcation on data was required, authors were also contacted.\nAll references identiﬁed by the searches were uploaded into COVIDENCE and de-duplicated. Two reviewers\n(EB & SMS) screened titles and abstracts separately for eligible articles and two reviewers (SMS & AL)\nreviewed all full texts of these articles for ﬁnal study selection. Disagreements were resolved by discussion\nbetween reviewers and with a third senior reviewer (EB). Two-reviewers (EB & DM) performed a risk of\nbias (ROB) assessment using appropriate critical appraisal tools. Disagreements were resolved by discussion\nand a consensus was reached. The ROB assessment tools used for either type of study design used [16-22]\nare available in supplementary material. Findings were reported as a qualitative synthesis due to variation\nin reporting, which precluded meta-analysis.\nRESULTS\nThe initial search identiﬁed 466 studies. Two studies were removed as duplicates resulting in 464 studies,\nwhich were screened applying the inclusion and exclusion criteria for the study protocol. All studies were then assessed for eligibility at full-text level by two independent\nreviewers(SMSandAL).34studiesoverallwereexcluded;2wrongcomparator,3wrongintervention,5wrong\nstudy design, 3 unobtainable in full text, 2 no extractable data, 2 wrong patient population and 17 did not\nassess mental health when reviewed in more detail. 67 studies were therefore included in the systematic review\n[6, 23-87] . Nineteen randomised control trials (RCT) showed improvement in mental health and emotional\nwellbeing scores after treatment with progestin. Eight studies which assessed the impact on mental health\nof both progestins and CHC showed improved mental health with these treatments.\nEl Taha et al. (2021)[24] performed a RCT (n=70) comparing the eﬀects of Dienogest vs. Combined Oral\nContraceptivePill(COCP)giventopatientswithendometriosis. Theyassessedemotionalwellbeingusingthe\nEHP-30 scale performed at baseline and after 6 months of treatment. They found signiﬁcant improvement in\nemotional wellbeing for the dienogest arm compared with baseline scores, with a non-statistically signiﬁcant\nimprovement in the COCP group.\nTanmahasamut (2012)[25] assessed the mental health outcomes for patients undergoing surgical excision\nof endometriosis alone vs. surgical excision with insertion of a levonorgestrel-releasing intrauterine system\n(LNG-IUS) in an RCT (n=55). The SF-36 (mental health domain) was assessed in patients at baseline, after\n6 months and 12 months of treatment. Interestingly, the SF-36 scores with LNG-IUS insertion showed sta-\ntistically signiﬁcant improvement from baseline mental health scores (p 0.022) and statistically signiﬁcantly\nbetter scores than those with surgery alone.\nMaiorana et al. (2023) [30] assessed 64 patients with endometriosis at baseline (T0) and\nafter 18 months (T1) of treatment with either Dienogest (56% patients) or a COCP (44%\npatients) containing dienogest (Qlaira) using both SF-36 (mental health domains) and EHP-30\n(emotional wellbeing). EHP-30 emotional wellbeing scores showed statistically signiﬁcant improvement\nin both treatment arms F(1, 59)=4.45, p<0.05, whilst the SF-36 mental health scores showed no signiﬁcant\nchange.\nOzdegirmenci (2011)[38] performed a prospective RCT assessing 75 patients with endometriosis divided into\ntwo groups LNG-IUS (n=43) vs. hysterectomy (n-32). EHP-30 emotional wellbeing scores\nwere assessed at baseline, 3-months and 6-months post treatment. Oral GnRH antagonist and injectable\nGnRH analogues showed equivocal improvement in emotional wellbeing compared with placebo. The best\nimprovement in emotional wellbeing was demonstrated in the mid and higher dose of GnRH antagonist:\nBaseline EHP=30 Emotional wellbeing scores: 10 mg Relugolix 20.7 (20.28); 20 mg Relugolix 25.3 (19.72);\n40 mg Relugolix 21.0 (18.25); Leuprorelin 21.4 (20.08); Placebo 21.9 (20.43). Emotional wellbeing changes\nfrom baseline at 3-months: -8.3 (16.44), -8.9 (18.62), -10.4 (17.77), -8.9 (17.34). Emotional wellbeing changes\nfrom baseline at 6-months: -8.4 (15.92) -15.4 (17.86) -13.3 (16.32) -12.4 (18.33).\nA non-randomised control trial published in 2020 by Alshehre et al.[54] assessed mental health outcomes for\nendometriosis patients receiving a GnRH analogue Triptorelin SR (11.25 mg) with addback HRT Tibolone\n(2.5 mg) at baseline and after 24 months of treatment. Alshehre reported a statistically signiﬁcant improve-\nment in the median EHP-30 (mental health domain) score by 24 months when compared to baseline 8.3\n(0–50) and 54.2 (0–100) respectively (Z = 3.301, p < 0.001). This however signiﬁcantly deteriorated after\ndiscontinuation of treatment at the 30-month follow up 45.8 (0–70.8) (Z = - 2.51, p = 0.009) showing that\nmental health improved only for the length of duration whilst receiving treatment.\nWorsened mental health with hormones and/or hormone-blocking agents\nTwenty-one studies showed worsening mental health for patients with endometriosis taking hormones or\nhormone-blocking agents: progestins only (8), progestins and CHCs (3), progestins and hormone-blocking\nagents (4), hormone-blocking agents only (5), CHC only (1). All studies assessing progesterone therapies\ninvolved synthetic progestins, no studies used a micronized progesterone. It is important to highlight that\nmicronized progesterone is associated with fewer side eﬀects than other synthetic progestin therapies due to\nits ‘bioidentical’ nature, which is thought to cause less of a metabolic impact[88].\nThe largest study in this group was The Visanne Post-Approval Observational Study (VIPOS) published by\nMohener et al in 2020[74]. The study assessed 26,430 people with endometriosis and observed the risk of\ndepression in patients using Dienogest (Visanne) 2mg orally per day; 11.4% used Dienogest (DNG), 12.8%\nused other approved endometriosis medications (OAED) and 75.7% used hormonal treatments not approved\nbut frequently used for endometriosis treatment (NAED) including progestins and CHCs. There were 139\nnew or worsening cases of depression in total. The incidence rate in Dienogest users was higher (35.7/10,000\nwomen years; 16 cases) compared to OAED (8.2/10,000 women years; 2 cases) and NAED users (17.0/\n10,000women years; 68 cases). The adjusted hazard ratios were 1.8 (95% CI, 0.3–9.4) for DNG vs OAED\nand1.5 (95%CI,0.8–2.8) for DNG vs NAED. Overall, the number of depression events were substantially lower\nthan expected. The power of the study was insuﬃcient to exclude risk of newly diagnosed or deteriorating\ndepression for DNG vs OAED or NAED. Whilst a slight increase in depression risk cannot be excluded, this\nmight be explained by baseline severity of endometriosis or unknown country-speciﬁc confounding variables.\nLi et al. (2014)[26] performed a RCT with 280 participants who underwent laparoscopic excision of en-\ndometriosis and were given one of two GnRH-analogues (Leuprorelin n = 142 or Triptorelin n=138) post\noperatively. Adverse mental health side eﬀects were assessed and at 4 weeks and at 9 weeks. In both groups,\npatients developed anxiety and depression however statistically signiﬁcantly more anxiety and depression\nwere noted in the triptorelin group vs. the leuprorelin group. Anxiety: Leuprorelin 6/142 (4.23%) vs. Trip-\ntorelin 28/138 (20.29%) P 0.001. Data may be preliminary.\nTwo randomised, double-blinded studies published by Giudice in 2022[47] (SPIRIT 1 and 2 trial) with a\ntotal of 1251 participants reported 9 cases of suicidal ideation across both studies subsequently leading to\nall 9 participants withdrawing from the trial. Relugolix n=5, GnRH-analogue n=2 and placebo n=2.\nInarecentcohortstudybyLaTorre(2024)[81]17.4%ofthe114participantsusingdienogestforendometriosis\ndeveloped “mood disorders” subsequently resulting in them having to discontinue the hormonal treatment,\nwith an average time of discontinuation of 7.2 months.\nMorotti et al. (2017)[61] observed 103 patients with rectovaginal endometriosis receiving an oral Norethin-\ndrone Acetate (NETA) (2.5 mg/day up to 5 mg/day) for 5 years and reported adverse reactions: of the 103\nparticipants (n=7) people developed depression (6.8%). Similar rates of depression were reported by Ver-\ncellini et al. (2010)[73] with CHCs (Vaginal ring ethinyl E 15mcg and 120mcg etonogestrel vs. transdermal\n20mcg ethinyl E and 150mcg norelgestromin) in a study assessing adverse side eﬀects after 12-months of\ntreatment; 6% using the vaginal ring vs. 5% with transdermal CHC patch. There was no statistically signif-\nicant diﬀerence between the two groups (p 0.76). R¨ omer (2018)[77] however reported the rate of depressed\nmood with Dienogest to be as high as 11% (4/37) in a 5-year retrospective cohort study when assessing the\ndrug’s long-term eﬃcacy and safety in clinical practice.\nStudies where mental health was the primary outcome investigated\nThe primary outcomes considered in the studies were assessed, 24/67 had mental health changes as the\nprimary outcome. Of the remaining studies, the primary outcomes were reduction in pain (26/67), tolerability\nof treatment (9/67), satisfaction of treatment (2/67), impact on sexual-wellbeing (1/67) and radiological\nreduction in endometriosis disease burden (4/67).\nFourteen of the 24 studies (58.3%) showed an improvement in mental health with medications, 6/24 (25%)\nshowed worsening of mental health, 2/24 (8/3%) showed no change in mental health with progestins or\nCHCs, 1/24 (4.2%) study showed no change in mental health with hormone-blockers and 1/24 (4.2%) study\nshowed an improvement in mental health with progestins and worsened mental health with hormone-blockers.\nDISCUSSION\nWe report that most studies (n=42) observed improvement in mental health with hormonal or hormone-\nblocking agents for the treatment of endometriosis pain rather than deterioration of mental health (n=21).\nThirty-three studies show signiﬁcant improvement in mental health and overall quality of life with progestins,\nhormone-blocking agents and CHCs, given our experience of frequent reports of side eﬀects and the high\nrate of mental health co-morbidity in endometriosis[89].\nWhile many individuals with endometriosis may experience psychological beneﬁts from hormonal downregu-\nlation, because pain scores improve, some women, outside of their control, are sensitive to hormonal changes,\nand experience iatrogenic depression and anxiety. These experiences could be inﬂuenced by factors such as\ndisease severity, individual coping strategies, and any pre-existing mental health conditions.\nPatients may prioritise improved physical symptoms over mental health from hormonal downregulating\ntreatments, such as improved pelvic pain at the cost of mental health deterioration. This raises the question\nof whether managing one aspect of health may inadvertently exacerbate the other, presenting a challenging\nclinical dilemma.\nThe studies which showed improved mental health outcomes with hormonal or hormone-blocking treatments\nhad either a low (22/42; 52.4%) or moderate (11/42; 26.2%) risk of bias, with 9/42 (21.4%) having a high risk\nof bias. Six of these 9 studies were RCTs. Similarly, of those studies which reported worsening mental health\neﬀects with hormonal or hormone-blocking treatments; 11/21 (52.4%) had low risk of bias, 7/21 (33.3%) had\nmoderate risk of bias and 3/21 (14.2%) had a high risk of bias.\nThe incidence of mental health side eﬀects of hormonal and hormone-blocking treatments may be underre-\nported in patients with endometriosis:\nIn research settings:\n8\n\nPosted on 3 Jul 2025 | The copyright holder is the author/funder. Data may be preliminary.\n• Many researchers fail to deﬁne baseline mental health, quantify depression, or adequately power the\nstudy for this outcome.\n• There are published, designed and validated tools to assess side eﬀects[90] but none of the included\nauthors have used them.\n• Patients who experience adverse mental health eﬀects from prescribed medication often become non-\ncompliant or may drop out of studies.\nIn clinical settings:\n• Endometriosis patients often have had their symptoms minimized or dismissed by healthcare providers\n[91] and may not feel empowered to discuss mental health for fear of stigma and clinicians are not\nroutinely screening for this[92]\n• Patients may feel their concerns about the eﬀects of hormonal treatment on skin or weight are triviali-\nsed, despite the potential impact on mental health. This can lead to underreporting of such issues, as\nindividuals may fear being perceived as vain or ridiculed.\n• The high incidence of mental health comorbidity in women endometriosis may need to a normalisation\nof additional iatrogenic mental health deterioration from treatment by women and clinicians\n• Many hormones and hormone-blocking treatments are not licenced for the treatment of endometriosis,\nwhich could deter women and clinicians from yellow card reporting to the Medicines and Healthcare\nproducts Regulatory Agency (MHRA), who encourage reporting of unlicensed and oﬀ-license medica-\ntion side eﬀects.\nCONCLUSION\nWe can reassure patients with endometriosis that hormonal and hormone-blocking treatments have a bene-\nﬁcial impact on mental health in general but warn that there are individuals who may suﬀer deterioration\nof mental health, requiring monitoring and reporting to the MHRA. Adequately powered studies, including\nthe correct questionnaires are needed to qualify this eﬀect, and to understand its mechanism. We also need\nto understand which endometriosis patients are more vulnerable to those side eﬀects and if there is a way of\npredicting this.
