Material and Methods Bibliometric analysis of the field of obesity and GERD was conducted based on database of the Web of Science Core Collection (WOSCC) from its inception to October 20, 2022. Several factors are analyzed and mapped using VOSviewer software, including the total number of publications, the number of resources and journals, countries, authors, and intellectual networks, as well as the co-citation analysis of keywords and references. Results A total of 2,306 papers from 480 journals with 10,010 authors from 2,628 institutes in 75 countries were included. There was an increase in publishing papers and citations each year. Most of the publications were authored by researchers from the United States and Italy. In the collaboration network map, Mayo Clinic, Karolinska Institute, the University of Washington had the largest cluster of nodes. Lagergren J, Corley DA, Whiteman DC, Vaughan TL, and Patti MG were the most prolific authors. There was a high concentration of research into bariatric surgery (BS), risk factors, body mass index (BMI), and Barrett's esophagus in this field. Future research may focus on weight gain, sleep apnea, sleeve gastrectomy (SG), and revisional bariatric surgery. Conclusions We summarized the research hotspots and trends, collaborative network, and future research directions in the field of obesity and GERD. In addition to help scholars understand essential information, our results will also enable them to identify research frontiers. Introduction Obesity is a chronic metabolic disorder which caused by various factors and is one of the most severe global public health issues[ 1 ]. It increases the risk of several diseases, such as cardiovascular diseases, diabetes, and GERD. These diseases negatively influence the productivity, quality of life, and healthcare costs[ 2 , 3 ]. GERD is a common disease in which stomach contents enter the esophagus, causing discomfort complaints [ 4 ]. The rapid increase in obesity worldwide has contributed significantly to the prevalence of GERD, which is one of the most important risk factors for the condition [ 5 ]. Obesity and overweight are typically defined by the BMI. The risk of developing GERD increases with an increase in BMI from normal to overweight [ 6 ]. There are many factors might contribute to the association between obesity and GERD, including improper esophageal clearance, higher prevalence of hiatal hernia, increased intra-abdominal pressure, altered transdiaphragmatic pressure gradient, and esophageal dysmotility[ 7 – 10 ]. For patients with obesity and GERD, most treatments focus on weight loss because it has consistent benefits in relieving symptoms and reducing the risks [ 4 , 11 ]. BS is the most effective treatment method to sustain weight loss compared to nonsurgical strategies [ 12 ]. More than half of all bariatric surgeries worldwide were SG, but the connection between this procedure and GERD was up for debate [ 10 , 13 ]. The pathophysiological mechanism underlying the connection between obesity and GERD remains unclear, various kinds of research have been performed to determine the problem [ 8 , 11 , 14 ]. On this basis, it is necessary to keep abreast of developments and emerging trends in certain areas. Bibliometric analysis is a method of quantifying and visualizing the published literature, giving an outline of the present status of studies, and identifying future studies in a certain area[ 15 ]. Only one bibliometric analysis of GERD has been published, in which the research topics, emerging themes, and knowledge structure of GERD during the last decade were clarified [ 16 ]. Collaboration patterns, research hotspots, and research trends in obesity and GERD have not been the focus of any bibliometric analysis. We conduct a bibliometric analysis to identify and map the information from published papers in order to clarify the current state of research and explore the directions for future studies. 2. Subjects, materials and methods 2.1. Data source and collection The retrieved records were based on the WOSCC from its inception to October 20, 2022. The retrieval method was as follows: TS= (“Gastroesophageal Reflux” OR “Esophageal Reflux” OR “Gastroesophageal Reflux Disease” OR “Gastro Esophageal Reflux” OR “Reflux, Gastroesophageal” OR “Gastric Acid Reflux” OR “Reflux Disease, Gastro-Esophageal” OR “GERD”) AND TS= (“obesity”). The type of articles and reviews was limited to those published in English. 2.2. Research methods All of the records were imported into VOSviewer for the bibliometric analysis. SPSS (version 26.0) and Microsoft Excel (version 2021) were utilized for the statistical analysis. Prof. Nees J and Ludo W from Leiden University created VOSviewer (version 1.6.18). To construct and visualize co-authoring, co-citation, and symbiotic networks, the software's text mining capabilities extract key elements from a large number of scientific publications [ 17 – 19 ]. 3. Results 3.1 Document selection and identification A total of 2,665 records were recognized from WOSCC. Non-articles, reviews, non-English records, and duplicates were excluded. Eventually, 2,306 papers were included in this bibliometric analysis (Supplementary Figure S1 ). 3.2 Annual growth trend of publications Annually, there was an increase in the number of papers, and the total number of papers published over the past two decades generally appeared an upward trend (Fig. 1 ). The majority consisted of reviews and original articles (2,368/2,665; 88.9%), which reflected the changes and development trends in this field. Supplementary Figure S2 shows that the 2,306 records were cited 81,075 times and every paper had an average of 35.1 citations. 3.3 Analysis of countries and districts The articles related to the field of obesity and GERD were published by 75 countries or regions. The United States, Italy, and United Kingdom accounting for 57.6% (1,328/2,306) of the papers. The highest average number of citations was Sweden (52), followed by Canada (50.9), Australia (37.2), Italy (44.3), and the United States (42.7) (Table 1 ). Supplementary Figure S3A showed that these nations worked together actively, particularly the United States and Australia. Prior to 2014, the United States, Australia, and Canada were the primary contributors to research on obesity and GERD in terms of dynamics and trends. After 2014, other nations began to pay more attention to the field and direction (Supplementary Figure S3B). 3.4 Institutional analysis These publications were published by a total of 2,628 institutions. Five clusters were formed in a network and overlay visualization of the top 100 institutions; the largest node represented the institution that published the most articles in their cluster (Supplementary Figure S4A). Mayo Clinic (Cluster 1; n = 75; 2.9%), Karolinska Institute (Cluster 6; n = 59; 2.2%), the University of Washington (Cluster 11; n = 41; 1.6%), Fred Hutchinson Cancer Research Center (Cluster 8; n = 30; 1.1%), the University of Padua (Cluster 3; n = 22; 0.8%), and Tehran University of Medical Sciences (Cluster 5; n = 19; 0.7%) were the biggest nodes in their cluster. The largest cluster, with 47 nodes representing related universities or institutions, was Cluster 1, while the smallest cluster, with just four nodes, was Cluster 5. There were also 17 nodes in Cluster 2, 16 in Cluster 3, and 9 in Cluster 4 in the other clusters. In addition, the findings demonstrated that, between the years 2010 and 2016, the Mayo Clinic, the Karolinska Institute, and the University of Washington made the most significant contributions to this field. Other institutions, such as the University of Queensland and the University of Miami, have gradually increased research in this area since then (Supplementary Figure S4B). 3.5 Analysis of co-cited authors and authors There were 10,010 authors involved in the creation of these documents. The top 150 most prolific authors of the author visualization graph form six clusters (Fig. 2 ). The largest contribution was by Thomas TL (24/2306) affiliated with Fred Hutchinson Cancer Research Center. EI-Serag H (19/2306) had the second biggest cluster and was affiliated with Baylor College of Medicine and the Houston Veterans Affairs Medical Center. According to Supplementary Table S1 , Lagergren J (34/2306), who is affiliated with the Karolinska Institute and Karolinska University Hospital, frequently collaborated with other researchers and published the most papers. 3.6 Distribution of output in journals Articles on obesity and GERD were published in 480 academic journals. Obesity Surgery (n = 364) was the journal with the most citations (141,861 times). The American Journal of Respiratory and Critical Care Medicine had the highest average number of citations (n = 6; average citation numbers: 206). In Supplementary Table S2, the journals with the highest average number of citations, highest productivity, and highest citation counts are listed (n = 10). 3.7 Citations and Co-citations 3.7.1 Analysis of the top 10 citations Table 2 displays the top ten publications out of the 2,306 records based on the number of citations. In 2005, Hampel et al.'s article was published in the Annals of Internal Medicine and received the most citations[ 20 ] The association between BMI and GERD, including its complications, was reported in the article, which received 774 citations—more than the paper with the second-highest number of 757 citations [ 21 ]. Long-term Results of Laparoscopic Sleeve Gastrectomy for Obesity had 515 citations, making it the third most cited paper [ 22 ]. A greater number of literature citations and a greater number of co-citations are indicated by thicker lines and larger nodes. Supplementary Table S3 displays the top ten papers with co-citations. The Annals of Internal Medicine, New England Journal of Medicine, American Journal of Gastroenterology, Gastroenterology, Gut, and Journal of the American Medical Association were the primary venues for the publication of these papers with co-citations. 3.8 Keywords analysis 5,202 keywords were extracted from the retrieved papers. The network diagram for the top 100 keywords was divided into three clusters (Supplementary Figure S5A). The red cluster contained the top 10 keywords. Obesity (n = 950), gastroesophageal-reflux disease (n = 608), and bariatric surgery (n = 577) were the top three keywords. Bariatric surgery, BMI, weight loss, risk factors, and Barrett's esophagus are the most recent hot research areas on obesity and GERD, as shown in Supplementary Figure S5B and Supplementary Table S4. 3.9 Research Frontier Analysis By looking at the keywords for the last two years, the research frontiers-related keywords were discovered (Supplementary Table S5). According to the table, weight regain, sleep apnea, revisional bariatric surgery, and pH monitoring may become the research frontiers in the future. 4. Discussion Unlike traditional literature reviews, bibliometric analysis allows for a systematic analysis of published papers in particular areas [ 23 , 24 ]. The research trends and hotspots, collaborative network, and future research directions on obesity and GERD are all summarized in this study. A total of 2,306 papers in 480 journals by 10,010 authors from 2,628 institutes in 75 countries were retrieved. The upward trend in the number of published articles, particularly in 2021, suggests that new researchers may pursue this field of study. The United States ranked first in article publication, followed by Italy, the United Kingdom, Turkey, and Australia. The degree to which other nations or institutions accept and acknowledge the results of a country's scientific research is reflected in the citation rate, which is an important indicator of a paper's academic impact[ 25 ]. The top two countries with the highest total citation frequency are the United States (n = 42,578) and Italy (n = 4,551). However, Sweden has a higher average citation frequency than these two countries, indicating that Sweden has a greater influence in this field. The degree of cooperation varies between districts and nations. In some areas, academic productivity is greatly aided by international cooperation [ 26 ]. The United States is most closely associated with the United Kingdom and Italy. In addition, these nations rank among the top five most prolific. As a result of the emergence of regional imbalances in various nations and regions, it is imperative that developed and developing nations worldwide work together and communicate more effectively. In addition, Among the 2,628 institutions, Mayo Clinic (United States), Karolinska Institute (Sweden), University of Washington (United States), Fred Hutchinson Cancer Research Center (United States), University of Padua (Italy), and University of Uppsala (Sweden) contributed significantly to this field and remained cooperative with other institutions. Lagergren J published the most articles over the past two decades, primarily on Barrett's esophagus and esophageal adenocarcinoma[ 27 – 29 ]. The American Journal of Respiratory and Critical Care Medicine had the highest average paper citation, while obesity surgery was the most cited and published journal. Additionally, the fact that all three of the top journals in terms of average citations in the indexed records were published in these top journals suggests that these journals are well-known and deserving of the attention of researchers in the field. These top-cited records were distributed in Obesity Surgery, Surgery for Obesity and Related Diseases, Surgical Endoscopy and Other Interventional Techniques, and Gastroenterology, which had been cited more than 317,231 times. The pathogenesis, epidemiology, diagnosis, and treatment of GERD, as well as its connection to obesity, were the most frequently mentioned themes in the top ten cited research articles in this field. Researchers can get direction from the co-citation analysis of references regarding the references that contributed significantly to a particular field[ 30 ]. The keywords of the top co-citation references, which merited a reference for researchers, were “bariatric surgery,” “body mass index,” “erosive esophagitis,” “esophageal adenocarcinoma,” and “Barrett's esophagus” in the co-citation analysis. In order to locate research hotspots in particular research areas, high-frequency keywords are frequently utilized. According to the network diagram of keywords, we found that recent studies focused on BS, BMI, weight loss, risk factors, and Barrett's esophagus. The co-occurrence keyword analysis in the past two years revealed weight regain, sleep apnea, sleeve gastrectomy, and pH monitoring as emerging topics. The most effective method for sustaining weight loss is BS[ 31 ]. Laparoscopic sleeve gastrectomy (LSG) is the bariatric surgery which carried out most frequently all over the world [ 32 ]. LSG has gained popularity because it is a relatively straightforward procedure that does not require surgical anastomoses or anatomical rearrangement. Additionally, it has been demonstrated to successfully induce weight loss. In addition, it appears to be safe over the long term, with a lower risk of internal hernias, marginal ulceration, and negative nutritional effects[ 33 ]. However, the possibility of developing or worsening GERD postoperatively is a major drawback of LSG's popularity [ 13 ]. Researchers could identify the research hotspots in this study, which might influence the future directions. 5. Limitations There are undoubtedly several limitations to the study. Firstly, we only analyzed papers from the WOSCC, which might cause selection bias. However, WOSCC covers a wide range of papers from multifarious fields and is suitable for bibliometric analysis [ 34 ]. Secondly, only English-language articles and reviews were analyzed, which may have led to selection bias. But there were sufficient numbers of retrieved papers to accurately reflect the research situation [ 35 ]. 6. Conclusions We conducted a bibliometric and visual analysis using retrieved records related to obesity and GERD. The total number of papers published has increased, and numerous countries and institutions have collaborated extensively. The frontiers of this field included weight regain, sleep apnea, revisional bariatric surgery, and pH monitoring. It is helpful for researchers to comprehend research hotspots, the patterns of collaboration, and frontiers through bibliometric analysis. There were also 17 nodes in Cluster 2, 16 in Cluster 3, and 9 in Cluster 4 in the other clusters. In addition, the findings demonstrated that, between the years 2010 and 2016, the Mayo Clinic, the Karolinska Institute, and the University of Washington made the most significant contributions to this field. Several factors are analyzed and mapped using VOSviewer software, including the total number of publications, the number of resources and journals, countries, authors, and intellectual networks, as well as the co-citation analysis of keywords and references.\\u003c/p\\u003e\\u003ch2\\u003eResults\\u003c/h2\\u003e \\u003cp\\u003eA total of 2,306 papers from 480 journals with 10,010 authors from 2,628 institutes in 75 countries were included. There was an increase in publishing papers and citations each year. Most of the publications were authored by researchers from the United States and Italy. In the collaboration network map, Mayo Clinic, Karolinska Institute, the University of Washington had the largest cluster of nodes. Lagergren J, Corley DA, Whiteman DC, Vaughan TL, and Patti MG were the most prolific authors. There was a high concentration of research into bariatric surgery (BS), risk factors, body mass index (BMI), and Barrett's esophagus in this field. Future research may focus on weight gain, sleep apnea, sleeve gastrectomy (SG), and revisional bariatric surgery.\\u003c/p\\u003e\\u003ch2\\u003eConclusions\\u003c/h2\\u003e \\u003cp\\u003eWe summarized the research hotspots and trends, collaborative network, and future research directions in the field of obesity and GERD.
