{"paper_id":"3753c9a3-3aba-4a85-8084-006f3f6dcabe","body_text":"Zusammenfassung\nUnter der Verdachtsdiagnose einer Rektumbeteiligung bei Endometriose wurden 36 gynäkologische Patientinnen zur weiteren Diagnostik der proktologischen Sprechstunde der Klinik für Allgemeinchirurgie und Thoraxchirurgie der Universität Kiel zugewiesen. Alle Patientinnen wurden mittels endorektaler Sonographie untersucht, um die Wertigkeit dieser Methode im Rahmen der präoperativen Diagnostik zu evaluieren.\nBei 13 Patientinnen wurde endosonographisch der Verdacht auf eine Infiltration des Rektums durch einen Endometrioseherd geäußert. In zwei weiteren Fällen lag der Endometrioseherd unmittelbar der Rektumwand an, ohne in die Lamina propria hineinzureichen. Bei den verbleibenden 21 Frauen konnte eine Rektumbeteiligung endosonographisch ausgeschlossen werden, und es erfolgte eine Laparoskopie in der Klinik für Gynäkologie. Lag eine Rektumbeteiligung vor, wurden die Patientinnen in die Chirurgische Klinik zur Laparotomie aufgenommen. Der intraoperative Befund bestätigte in allen 36 Fällen die endosonographische Diagnose; die Endometrioseherde wurden histologisch verifiziert. Bei Rektumbeteiligung wurde entweder eine tiefe anteriore Resektion (n=9) oder eine Rektummanschettenresektion (n=4) durchgeführt.\nSchlußfolgerung\nDie endorektale Sonographie ist in der Lage, bereits präoperativ mit hoher Genauigkeit eine Endometriosebeteiligung des Rektums festzustellen und somit das weitere therapeutische Prozedere maßgeblich zu beeinflussen. Zusätzlich kann die Endosonographie bei unklaren Beschwerden kleine Endometrioseherde lokalisieren und somit die laparoskopische Exploration vereinfachen.\nAbstract\n36 women with suspected rectal endometriosis were examined by endorectal sonography to evaluate the role of this technique for rectal wall involvement and to evaluate the position of preoperative diagnosis in the operative management. In 13 patients rectal wall involvement was detected by endorectal sonography. Those 13 women were operated by laparotomy and anterior rectal resection (n=9), particular excision of the involved wall (n=4) in the surgical department. In two cases the endometrial tumor was localized directly beyond the M. propria without infiltration. In 21 women rectal involvement could be excluded. All 23 patients without rectal involvement were treated laparoscopically in the gynecological department. Preoperative diagnosis was confirmed in all patients during operation.\nEndorectal sonography is able to visualize perirectal endometriomas and to access rectal wall involvement.
