Although many studies have been conducted worldwide to find out the prevalence of job satisfaction among doctors, yet very few have been conducted in Ethiopia, especially regarding medical interns. Objective: This study was aimed to assess the prevalence of job satisfaction and its associated factors among medical intern students in University of Gondar comprehensive specialized hospital, Gondar, Ethiopia 2022. Method: An institution based cross-sectional study was conducted on 320 study participants from November 05-15, 2022. All medical interns registered in 2022 was assessed by using conventional sampling method and data were collected by using pre-tested and structured self-administered questionnaire. Data were entered into Epi-INFO version7 and exported to SPSS version 25.0 for analysis. Logistic regression were carried out to recognize factors associated with medical intern job satisfaction. Adjusted odds ratio with a 95% confidence interval were used to measure the strength of association. Variables having p-value less than 0.2 in bivariable analysis were entered in to multivariable analysis; variables with a p-value< 0.05 were declared significantly associated with job satisfaction. Results: The overall level prevalence of job satisfaction among medical interns was found to be 32.2% 95%. Being male [AOR=1.84; (1.02, 3.34)], working department [AOR=2.53; (1.2, 5.30)], place of origin [AOR=1.80; (1.01, 3.20)], and number of duties [AOR=2.98; (1.17, 7.59)] factors were significantly associated with job satisfaction. Conclusion: The prevalence of job satisfaction among medical interns was found to be high. Being male, place of origin, working department, and number of duty were statistically significant associated factors. Its better, managers and policy makers should give due considerations on medical interns’ job satisfaction if they genuinely want to attain institution’s objectives. Medical intern job satisfaction University of Gondar Figures Figure 1 Introduction Job satisfaction is a gratifying or positive emotional state resulting from evaluating one’s job or job experiences. Job satisfaction can be divided into extrinsic and intrinsic factors. The extrinsic factors relate to the architecture of the job (pay, working conditions, and hours of work). The intrinsic factors relate to psychological attributes of the job (nature, ability, and recognition)( 1 ). . Deficiency of trained health professionals is a global encumbrance and it is high in most African countries including Ethiopia( 2 ).According to WHO report by the year 2035 the health workers shortage will get up to 12.9 million; 47 and 25% of the shortfall will be in South-East Asia and Africa( 3 ). To preclude this challenge the health profession’s intention to stay should be augmented by increasing their job satisfaction level( 4 ). Studies conducted in Africa on job satisfaction showed a prevalence that ranges from 17.4%( 5 ) to 82.6%( 6 ). Prior studies identified some determinants for health professional’s job satisfaction such as sex, age, educational status, year of experience, working environment, workload, salary, recognition and opportunity for professional growth and development( 7 – 9 ). In Ethiopia, systematic review and meta-analysis was done to assess the pooled prevalence of health professional’s job satisfaction and its determinants and was 46.17%( 10 ). Salary, working experience and profession type were identified as factors for job satisfaction. Support from supervisor, working environment, staff relationship, sex and opportunity for professional growth were identified as significant determinants for health professional’s job satisfaction. Based on the international standard classification of occupations, health professionals include medical doctors, nurse professional, midwifery professionals, dentists, medical laboratories, and pharmacists( 11 ). In most countries, all doctors must complete a medical internship after completing four to six years of medical education, and before becoming generally licensed and registered with the medical board of that country( 12 ). This is a structured period where doctors in training transit from supervised learning in medical schools to rapidly assume clinical responsibility under supervision, and it can be challenging. Despite the different terminology from intern, house officer, foundation doctor to resident, they are all under huge pressure: highly demanding working hours, less satisfactory pay and a need for ongoing learning and assessment( 13 ). Depending on the context, interns may also experience low availability of resources, limited supervision and feedback, poor safety climate, lack of responsiveness to basic psychological needs that result in rapid burnout and dissatisfaction. The internship year/years are also the time where these trainees are about to make their first career decisions and they are important in informing opinions about whether they want to continue medicine in that organization and country, and if so which specialty career seems most attractive. Exits after internship have significant financial cost to the host organization and country, and evidence from low- and middle-income countries suggests substantial exits and migration to high-income countries immediately after qualification, causing “brain drain” and huge financial losses as medical education is heavily subsidized in most countries( 14 , 15 ). Interns are usually made responsible for non-medical activities which cause extra burden of work on them. A non-medical activity then would be an activity that does not require a great deal of judgment or decision making based on Interns knowledge or expertise and does not change based on the individual client or situation. As prevailing practices of institution medical interns are involved in doing time consuming work, they get very less opportunity to be involved in patient care. Hence interns become frustrated when they are unable to use their knowledge and skill for patient for which actually, they are trained for. Satisfied workers tend to be more productive, creative, and committed. Therefore, a highly satisfied and free from burnout interns’ will eventually be effective in rendering a quality medical care since their ultimate goal is the patient satisfaction. Medical interns can directly influence patient satisfaction in that their involvement and interaction with patients plays a significant role in quality perception. Intern job satisfaction has positive correlation with patients fall rate. This study was aimed to determine the magnitude of job satisfaction among University of Gondar comprehensive specialized hospital medical interns, and to identify factors associated to the outcome variable. Methods Study design and period Institution based cross sectional study design was employed from November 05–15/2022. Study area University of Gondar, until 2003 was known as Gondar College of Medical Sciences, is the oldest medical school in Ethiopia and established as the Public Health College in 1954. It is located in Gondar city, the former capital city of Ethiopia, is located 738 km from Addis Ababa, which is the capital city of Ethiopia. University of Gondar Comprehensive specialized hospital provides a full range of health care services including outpatient, inpatient and surgical services. This hospital is expected to serve for more than 5 million people in its catchment area. The hospital has 1040 health care professionals, 580 beds in five different inpatient departments and 14 wards, and 14 different units giving outpatient services to customers. Besides, this hospital serves as a referral hospital for Northwest Ethiopia( 16 ). The medical interns are part of the health care professionals those serve the community. Study Population All medical intern in the University of Gondar, who are registered at college of medicine and health science in the academic year of 2022 and who are available during the study period. Sampling Technique, and Sample Size Determination On the basis of conventional approach of sampling method, an attempt was made to recruit all interns (census) i.e. 327, working at University of Gondar comprehensive specialized hospital during the study period. Subjects were identified from their interns’ lists and intern rotation schedules, which were accessible from the school of medicine registry and batch representatives, respectively. Data were collected by using a pretested and structured interviewee administered questionnaire. The questionnaire was prepared in English. Assessment of Job satisfaction was measured by using thirty items each scored 5-point Likert scale with 1 denoting strongly dissatisfied (refers to strongly disagree) and 5 denoting strongly satisfied (refers to strongly agree) with Minnesota Satisfaction Questionnaire (MSQ) short form( 17 , 18 ). The questions related to factors associated with job satisfaction were prepared by reviewing previous similar studies ( 13 , 14 , 19 – 26 ). During the data collection period, a questionnaire was distributed to each intern along with an informed consent form. Operational Definitions Medical Intern Refers to doctor in training who completed their primary qualification training and spend usually one to two years working in accredited positions in hospital settings. This period is compulsory and may be regulated by the government and professional licensing board. This is the period before they are fully licensed and registered to practice medicine unsupervised. This is the most common term in most countries( 15 ). Job satisfaction Job satisfaction is general or global affective reactions that individual holds at their job( 27 ). Interns are going to be asked to indicate their level of satisfaction with each statement using a five-point Likert scale, with 1 indicating \"strongly disagree\" and 5 indicating \"strongly agree\". The overall job satisfaction was estimated by taking the sum score of all the subscales. Then, to measure the level of job satisfaction of each individual, respondents who scored more than 90 of the sum (150) of all the satisfaction scale items were considered as satisfied with their job. Those who scored 90 and below was taken as dissatisfied( 28 ). For each domain factors, the sum score of each variable under domains value of 90 was taken as a cut point value to determine whether a medical intern satisfied with his/ her job or not. As a result, medical intern who scored a value of 90(60%) and below was considered as dissatisfied and those who scored greater than 90(60%) was considered as satisfied( 28 , 29 ). Data Quality Assurance The data collection methods, tools and the way how to handle ethical issues were discussed with the data collectors. Before the beginning of data collection for this study pre-test was conducted on some medical interns (5% of the sample size) who were not included in the main study to identify potential problems in the proposed study tool. Pretest was conducted in Bahirdar University, College of medicine and health sciences. English version of the questionnaire was used. Regular supervision by the supervisor and principal investigator compel to ensure that all necessary data were properly collected. Each day during data collection, filled questioners were checked for completeness and consistency. The collected data was edited and processed. DATA PROCESSING AND ANALYSIS Once all necessary data are obtained, data was checked for completeness, edited, cleaned, coded and entered in to Epi-info software version 7 and analyzed using IBM Statistics SPSS version 25 for windows. Mean, mode, and median were used for continuous variables whereas; frequency and percentage for categorical variables. Descriptive results were presented using tables and figures. Model fitness was checked using a Hosmer-Lemeshow goodness-of-fit test. Crude odds ratios with their 95% confidence intervals was estimated in the bivariable logistic regression analysis to assess the association between each explanatory variable and outcome variable. In the bivariable logistic regression, variables with P-value < 0.2 was fitted into the multivariable logistic regression analysis. Finally, adjusted odds ratios with their 95% confidence intervals were estimated to assess the strength of association, and variables with P-value < 0.05 were considered statistically significant factors. Ethical Consideration Ethical clearance was secured from the University of Gondar College of medicine and health science research review board and official letter of support was issued from each department. After approval was granted by the College of Medicine and Health Science Research and Ethics Committee (CMHSREC) in November 02 of 2022, data was collected for this cross-sectional study from 05 to 15 November 2022 at University of Gondar comprehensive specialized hospital. Permission from the school of medicine was obtained before data was going to be collected using two parts written questionnaire, which was to be self-administered by each intern and returned to the study team within the ten-day data collection period. Results A total of 327 study participants were targeted in this study, 320 of respondents returned their fully completed questionnaire with a response rate of 98.8%. As depicted in [Table 1 ] The majority of respondents were male (66.5%), aged 23 to 25 years (68.4%), single (85.9%), Orthodox Christian (82.5%), from urban (73.6%), get income 6501-6600ETB (44.1%), joined medical school because of Passion for the profession(45.3%), 74.1% worked for 4–12 hours per day, 82.5% had 4–12 duty in the last one month, nearly equally distributed working department (Table 1 ). Over all prevalence of job satisfaction among medical interns The prevalence of job satisfaction among medical interns were 32.2% at 95%CI [26.9, 37.5]. (Fig. 1 ) Factors associated with medical interns job satisfaction A multivariable variable logistic regression analysis was performed to identify factors associated with job satisfaction of medical interns. Consequently, four variables were found to be statistically associated with job satisfaction after adjusting for confounders. These were sex, working department, place of origin, and number of duties in the last one month. In this study, male interns were 1.84 times more likely to be satisfied by their job as compared to female interns [AOR = 1.84; 95% CI (1.02, 3.34)]. The odds of job satisfaction were 1.80 times higher among study participants whose residence from rural compared to urban [AOR = 1.80; 95% CI (1.01, 3.20)]. Study participants who were in internal medicine rotation were 2.53 more likely to be satisfied than those of in gynecology and obstetrics department rotation [AOR = 2.53; 95% CI (1.2, 5.30)]. Medical interns who had 4–12 number of duties are 2.98 times more likely to be satisfied with their job as compared to those who had more than 12 number of duties in the last one month[AOR = 2.98; 95% CI (1.17, 7.59)](Table 2 ). Discussion This study determined the prevalence of job satisfaction and its associated factors among medical intern students in University of Gondar comprehensive specialized hospital, Gondar, Ethiopia 2022. The overall prevalence of job satisfaction and its associated factors among medical intern students in University of Gondar comprehensive specialized hospital was found to be 32.2% at 95% CI [26.9, 37.5]. The prevalence of job satisfaction found in this study was almost comparable with previous cross sectional studies done among health care professionals in West Shoal zone, and Western Amhara region Ethiopia ( 30 ), that showed job satisfaction level of 31.7% ( 31 )and 34.9%( 10 ). On the other hand The prevalence of finding in this study was lower than health professionals satisfaction level reported in Addis Ababa (52.9%)( 32 ), West Ethiopia (41.5%)( 33 ), and Harare (44.2%)( 34 ). The possible reason for the above variation could be due to difference in the study participant’s i.e. rather than medical interns, the licensed health professionals including midwives, pharmacists, nurses, and others were the study participants. Specific to medical interns’ magnitude of job satisfaction, in our finding is lower than that of Malawi, Nigeria, India Christian medical college, and Delhi reports which were 56.7%( 14 ), 60.5%( 26 ), 63.4%( 35 ), and 44.8%( 36 ) respectively. The possible explanation for the above difference could be due to the difference in socio-economic status, organizational set-up, and patient burden. Regarding to the associated factors of job satisfaction among medical intern students, this study had find out that the factors significantly associated were sex, working department, place of origin, and number of duties. Being male medical interns were nearly two times more likely satisfied by their job as compared to their counterparts which is consistent with studies done in Karachi, Ayub medical college( 37 ) and Greece( 38 ) but inconsistent with studies done in Saudi Arabia( 37 ) and united Kingdom( 13 )which reported females are more satisfied than male. In addition to this, there were no significant differences in satisfaction level amongst Malawi( 14 ), Nigeria( 26 ), and India( 36 ) medical interns regarding sex. This might be because of the fact that internship is full of burden, prolonged work hours, and duties which may be counted as less tolerable situations for females than males( 37 , 38 ). Medical interns whose place of origin from rural were two times more likely to satisfy than as compared from urban dwellers. Notably, there was no significant variation in magnitude of job satisfaction regarding place of residence difference reported yet in literature we viewed. The possible explanation for our finding could be due to the fact that joining medicine and/or being there might be the underlining reason to be satisfied to them relative to those of who came from urban. Secondly, financial issues( 39 , 40 ) might be resolved for rural comers than that of urban comers, thus attributed to more satisfaction level. Study participants who were in internal medicine rotation were nearly three more likely satisfied than those of in gynecology and obstetrics department rotation. Notably, there were no significant variation in the magnitude of job satisfaction regarding working department difference reported yet in literature we viewed. The possible reason could be the difference in staff, working environment, and work load in our study area. Medical interns who had 4–12 number of duties were three tims more likely satisfied with their job as compared to those who had more than 12 number of duties in the last one month. This finding is consistent with studies done in Delhi( 36 ), and united Kingdom( 13 ). This could be explained by facing burn out usually is inevitable after long work hours, that in turn end up with dissatisfaction. Limitation of the study Since our participants were limited to university of Gondar medical interns, the generalization of our findings could be difficult. The analysis of results in this study was entirely quantitative. No open-ended questions were posed to the study participants, and this also may be considered as a limitation of the study. Conclusion The overall prevalence of job satisfaction among medical interns working at university of Gondar was high. Sex, place of origin, working department, and number of duty were statistically significant associated factors. All medical interns registered in 2022 was assessed by using conventional sampling method and data were collected by using pre-tested and structured self-administered questionnaire. Data were entered into Epi-INFO version7 and exported to SPSS version 25.0 for analysis. Logistic regression were carried out to recognize factors associated with medical intern job satisfaction. Adjusted odds ratio with a 95% confidence interval were used to measure the strength of association. Variables having p-value less than 0.2 in bivariable analysis were entered in to multivariable analysis; variables with a p-value\\u0026lt; 0.05 were declared significantly associated with job satisfaction.\\u003c/p\\u003e\\n\\u003cp\\u003e\\u003cstrong\\u003eResults: \\u003c/strong\\u003eThe overall level prevalence of job satisfaction among medical interns was found to be 32.2% 95%.
