{"paper_id":"1ebc8d3c-c587-4808-abb1-6627d8576c05","body_text":"Abstract\nThe purpose of this chapter is to present the keen reader an updated information and the accumulated evidence on surgery in adolescent endometriosis (EM) and to discuss surgical indications, findings, outcomes, and specific aspects of the disease that relate to surgery. The presenting symptoms and surgical findings somewhat differ from the endometriosis in the adult, contributing to a particularly challenging diagnosis.\nIn the selected population of symptomatic adolescents not responding to medical treatment, a questioned diagnosis, suspected advanced stage disease, or with endometrioma, early laparoscopy has the advantage of diagnosis and treatment (as in some Mullerian anomalies). The full spectrum of disease severity is found, including advanced stage EM in a substantial percent, and ovarian endometrioma cysts, with a special concern for future fertility and special surgical aspects. Subtle surgical findings may be unidentified by the inexperienced surgeon leading to false-negative diagnosis; thus, we strongly encourage video recordings and biopsies.\nIn adolescents, laparoscopy is very effective in reducing pain, bearing in mind a higher risk for symptom recurrence which may lead to repeat surgical intervention. Postoperative follow-up and treatment are crucial to identify and reduce recurrence. Subfertility is an important concern even in this young population, correlated with disease severity.\nThe implications of the medical encounter between caregiver and patient are immense, especially in adolescents, in broad aspects including physical, social, and psychological. It is beneficial to offer a suitable support system as part of a holistic approach.
