Its relevance in reconstructive surgery, particularly during umbilicoplasty, is underreported. A 37-year-old woman with prior laparoscopic procedures presented with periumbilical pain and a growing mass. Imaging was inconclusive, but symptoms suggested endometriosis. She underwent surgical excision, umbilical hernia repair, and umbilicoplasty. Histopathology confirmed endometriosis. Postoperatively, she recovered well with complete symptom resolution. Umbilical endometriosis should be considered in patients with prior abdominal surgery presenting with umbilical pain.
