In this case, we present a 48-year-old female patient with a rare persistent bilateral ureteral obstruction and hydroureteronephrosis.\nImaging revealed the obstruction but was not helpful in illuminating the etiology. In the patient who underwent exploratory laparotomy, the affected ureter and surrounding fibrotic foci were resected, and ureteroneocystostomy was performed. During follow-up, the patient's clinical condition improved, and no recurrence was observed.\nConsequently, ureteral endometriosis is a rare pathology, and imaging may not be helpful in determining the etiology.
