Whether endometriosis reduces a woman’s fecundability remains controversial. In asymptomatic infertile patients with patent fallopian tubes on imaging, there is no indication for diagnostic laparoscopy. It would require ~40 surgeries to result in one additional pregnancy. In patients with pelvic pain, adnexal masses consistent with endometriosis or tubal occlusion, operative laparoscopy is indicated and has been shown to result in improved fecundability.
